1 How to Login Transaction Management Open a web browser and go to 2. Input User ID and Password, then press Login. In case forget your password, please press Forgot Password for assistance. 1

3 How to issue New Test Request Step 1 Step 1: TRF Form Selection Click Service Requests, then select Testing Services. 2. Select appropriate option in Form Type, Lab Location, Program Client and Production Line (When selects Form Type Specific Form of the Client, additional field Program Client can be selected.) 3. Select appropriate Form Name for testing 4. Press Next to process OR Cancel to return. 3
4 How to issue New Test Request Step 2 Step 2: Input Details 5. Input at least the *mandatory information. 6. Click Back to go back to previous step for form selection. 7. After inputting, click Save and Continue to go to Step 3. **If you did not input all mandatory fields, the system will not bring you to Step 3. Message box will pop up at the beginning of the form to remind you to input all mandatory fields. 4
5 How to issue New Test Request Step 3 8. Step 3: Data Verification 8. Verify the information that you inputted. You may scroll down the cursor to verify all of them. If you want to modify, click Edit and it will bring you to Step 2. Then you can start modification from Step 2. If no modification is needed, click Confirm to go to Step 4. ** Once the TRF is confirmed, it cannot be modified anymore 5
6 How to issue New Test Request Step 4 Step 4: Service Agreement 9. Read the terms and conditions. You may scroll down the cursor to read all. Select option I agree the Terms and Conditions and click Accept to accept the agreement and go to Step 5. 6
7 How to issue New Test Request Step 5 Step 5: TRF Confirmation 10. To preview and print the TRF, click Print TRF. The Test Request Form will show on another web browser. Click to print the form. 11. To keep check the created Test Request Form, click Go to Dashboard. If you found any typo mistake on the form, you could not modify now. You may go to Main Menu and work from Step 1 to re-create a new TRF. 7
8 How to issue New Test Request Supplementary The TRF ID which is used for: Status tracking Operation can input it when create test submission to populate corresponding information to BVLab. **Operation need to confirm below before continuous the login process: A valid and printable TRF form with TRF# has been received (ie, Online TRF has been submitted in Transaction Management) Test sample has been received. 8
9 How to keep track TRF/Order Status This lists the Test Request Forms (TRF) that had been created successfully. You can use to keep track the request status. Click the header for sorting Download TRF Copy Order View Sample Description 9
10 How to keep track TRF/Order Status (Con t) Click the column header for sorting criterion. Sort by descending order Sort by ascending order Click for download the TRF form in PDF format Click for copy existing form and create a new form with pre-filled appropriate information. (Remarks: only appropriate for copy and create a TRF with same form template) Click and enter filter criteria to list out corresponding TRF records. 10

13 How to Change Password Click Edit Profile 2. Click Change Password Input your existing password and new password. Press Change Password to confirm
14 How to customize your default login page To customize your default login page, you can change the default value under the User Preference page 14
