3 Release Notes Product Versions Lifecycle Mediware currently supports a five year lifecycle for our software versions. Upon initial version release, a version is named for the year in which it is released. For example, the software is named Ascend Should there be a need for an additional release during the year, the subsequent release is still named for the year, but the year will be followed by R2 (Such as Ascend 2018 R2). Initial releases of software will contain product enhancements, including new features. NOTE If a planned release is close to the end of a calendar year, the release could be named for the following year. During the first two years following the initial release of the software version, service packs will be released, if needed. Service packs will contain defect corrections only. No new product enhancements will be included in a service pack. At the end of the two years, maintenance on the release ends. No further service packs will be issued for the release. However, support services will continue for years three through five. At the end of year five, Mediware will no longer support the release. 6 Ascend 2018 Release Notes Ascend 2018 Ascend 2018 Release Notes The Release Notes include information for all changes and enhancements in the Ascend 2018 release and are organized by the following categories. If more than one category is affected by an enhancement, the information is provided in the Multi-Category Enhancements section. Detailed information for each enhancement is also provided in the Help file. Upgrade Considerations Ascend Release 2018 requires Microsoft s.net version 4.6 or newer. If the client workstation does not have.net 4.6 installed, you will be prompted to install.net version 4.6 when upgrading to Ascend When you confirm to install.net 4.6, the Ascend installer automatically installs.net version 4.6 along with Ascend Ascend Release 2018 requires Microsoft SQL Server 2008 or higher. Important! Mediware no longer tests updates against Microsoft SQL Server 2005 or older. If you are still running Ascend on SQL Server 2005 or older, you will be unable to apply the Ascend 2018 update. Microsoft has ended support for SQL Server 2005 and will be ending support for SQL Server 2008 in the near future. If you are running on SQL Server 2000, 2005 or 2008, please consider updating to SQL Server 2012 or 2014 as soon as possible. If you are not sure which version of Microsoft SQL Server you are running, go to Help > About in Ascend. If you use the Ascend Interface Engine, you must update Ascend first before updating the Ascend Interface Engine. The Ascend Interface Engine also requires Microsoft's.NET version 4.6 or newer. If you receive electronic prescriptions, contact Customer Care for upgrade support as the e-prescribe web service will need to be updated. Page 1 Proprietary and confidential

7 Release Notes Ascend 2018 Release Notes Windows 10 Certification Ascend 2018 is certified to run on the Windows 10 Operating System. Client Responsibilities Clients are required to maintain their records for a minimum of two years. Mediware advises clients to back up their databases on a daily basis. 8 ASAP Changes Ascend 2018 ASAP Changes The following ASAP changes have been made. Case / Compound Order Formatting Error When the Compound field was set to Yes for a narcotic order, a formatting error occurred resulting in the patient information (PAT segment) being included before each ingredient in the narcotics export. This issue has been resolved so the patient information is included only once for a compound order. Page 3 Proprietary and confidential

9 Release Notes Billing and Financials Billing and Financials The following enhancements / bug fixes relate to Billing and Financials functionality in Ascend. Case / Allow EFT Payment Type Option for Claim Receipts An EFT # (electronic funds transfer) payment type option has been added to the Description drop-down list on the New Receipt screen and the Edit Receipt screen. To use this new payment type option, select EFT # from the Description drop-down list then enter the transfer number directly into the Description field immediately after 'EFT #'. If you tab into the Description drop-down list when either EFT # or Check # is selected, the cursor will be positioned immediately after the "#" symbol so the check or transfer number can be entered. 11 Release Notes Billing and Financials Case / Preserve New Claims Tab Sort Order The New Claims tab has been enhanced to preserve the user's most recently selected sort order. The default sort order is by Claim #, but the user's sort preference will now be saved even after navigating away from the tab or logging out of the application. 12 Interface Changes Ascend 2018 Interface Changes The following enhancements / bug fixes relate to the Ascend Interface Service (AIS). Case Valid Allergy Displayed as Unlisted When AIS received one of the following valid allergy listings for a patient, the allergy displayed on the Ascend patient profile in curly brackets indicating the allergy was unlisted. NKA NKDA NKFA This issue has been resolved. Valid allergy listings display correctly in the patient profile. Page 7 Proprietary and confidential

13 Release Notes Interface Changes Case Incorrect Dose Scheduling for Orders with a Frequency and Administration Times Changes have been made to accomodate frequency-based orders that include prescribed administration times. For example, an order to be given every three weeks at 06:00 would result in doses being scheduled daily at 06:00 as the frequency (interval of 21 days) would be disregarded. This has been corrected and doses are now scheduled using the SIG code, frequency, and administration times. Similarly, if a specific day of the week was included in this scenario (give every three weeks on Tuesday at 09:00), the order would be scheduled every Tuesday at 09:00 but not include the interval of every 21 days. 17 Release Notes Interface Changes Case / Order Created Before Admission Under certain circumstances, an orphaned order was created in Ascend when a HL7 message was received for a patient that did not already exist. The following modifications have been made to handle a HL7 message for a patient that does NOT already exist in Ascend. AIS will check the Don't Create Patients for the following Message Types setting. If the message type has been configured as Don't Create Patients, then AIS will NOT create any records in Ascend. Instead it will return an ACK message to the sending system. When the message type is NOT configured as Don't Create Patients, then AIS will create records in the appropriate sequence. In the case of an order, AIS will create the patient first, the admission second, and the order last. The Do not create patient on non-adt message check box has been removed from the ADT tab. The Don't Create Patients for the following Message Types setting should be used instead. 19 Release Notes Interface Changes Case / AIS - Reestablish Severed Database Connection An enhancement has been made to the interface service (AIS) to test the database connection before attempting to process an interface transaction. If the database connection fails, AIS will terminate the current connection and create a new connection. This will help prevent AIS from getting a database connection error and help ensure that messages are processed more quickly. Document Number: ASPNR2018R1_01 Revision Date: February 27, 2018 Page 14

20 Interface Changes Ascend 2018 Case Inbound HL7 Order Not Processed When Drug ID Contains Hyphen Previously, when an inbound HL7 order included a Drug ID that contained a hyphen, the interface service (AIS) was unable to process the order. This issue has been resolved. When an order is received where the Drug ID contains a hyphen, the system will attempt to match the corresponding Drug ID value in the inventory record. If a match is found, the order is created and linked to the matched inventory item. If a match is not found, the order is created with an unmatched inventory item and AIS continues processing additional order messages. The Drug ID is established in the AIS configuration as Charge Code, Alternate Charge Code, Unique ID, NDC, or Inventory Ref. Page 15 Proprietary and confidential

21 Release Notes Inventory Management Inventory Management The following enhancements / bug fixes relate to Inventory Management functionality in Ascend. Case Unable to Clear / Remove NDC from Inventory Item Previously, when an inventory item was edited to remove the NDC value, the inventory record was saved with two zeros in the NDC field. This issue has been resolved. The removed NDC is saved as a null value and displays as blank. Document Number: ASPNR2018R1_01 Revision Date: February 27, 2018 Page 16

22 Labels and Forms Ascend 2018 Labels and Forms The following enhancements / bug fixes relate to Labels and Forms functionality in Ascend. Case Outpatient Multi-Part Label Option A new Outpatient Multi-Part Label option is available for printing labels and priority warnings onto a single multi-part label sheet. This version of the Outpatient Multi-Part Label includes limited financial information, and its intended use is for customers who do not bill prescriptions. Future enhancements will be made to include patient payment amounts and other claim and financial information. When the Outpatient Multi-Part Label is selected for printing or previewing, the output will include the following. Prescription Label - contains all of the information required for an outpatient order label. The information included is based on the option settings for the Outpatient Multi-Part Label. Prioritized Warning Labels - includes up to five prioritized warnings from FDB. For multi-ingredient drugs, the priorities for each ingredient are evaluated and the five highest priority warnings are printed. 23 Release Notes Labels and Forms The Outpatient Multi-Part Label has been designed using a universal form supplied by Pharmacy Automation Supplies (Form/Item #PASLM014). The form dimensions are illustrated above. NOTE You do not have to purchase labels from a specific supplier as long as the dimensions on the form are similar. Nothing prints below the tear-off point on the sheet. The drug monograph will not fit within the area available below the tear-off point and must be printed separately. The Outpatient Multi-Part Label can be set as the default label for a specified order type on the Order Type Defaults screen. Label options can be set for the Outpatient Multi-Part Label in the usual manner on the Utilities > Options > Printing tab. Document Number: ASPNR2018R1_01 Revision Date: February 27, 2018 Page 18

25 Release Notes Labels and Forms Case / Sort Extemporaneous Labels List The Labels list on the Extemporaneous Labels screen has been enhanced to display labels alphabetically, making it easier to find and print the correct label. 26 Multi-Category Enhancements Ascend 2018 Multi-Category Enhancements The following enhancements / bug fixes relate to multiple categories throughout Ascend. SCR Alternate Outpatient Order Entry Screen An alternate Outpatient Order Entry screen is now available and can be configured to display instead of the traditional view which is an infusion-oriented view. The alternate view provides relevant fields for dispensing outpatient non-infusion prescriptions and includes a refill summary. An Outpatient Order View drop-down list has been added to the Order Entry tab of the Ascend Program Options screen. Either Infusion or Outpatient can be selected from the drop-down list. When Infusion is selected, the traditional view of the Order Entry screen will display when entering and viewing an outpatient order. When Outpatient is selected, the alternate view of the Order Entry screen will display when entering and viewing an outpatient order. An application administrator should choose the view that best suits the outpatient order entry work flow. NOTE The Outpatient Order View value will be set to Infusion by default. 27 Release Notes Multi-Category Enhancements A read-only Refill history has been added near the bottom right corner of the Outpatient view. One row displays in the Refill grid for the initial prescription fill and for each subsequent refill. A vertical scroll bar will appear if needed to view all of the rows. The following information displays in each row. Refill # Fill Start Date Fill Stop Date Quantity NDC Manufacturer Status Verified By The Refill grid columns can be resized as needed. If a cell value exceeds the column width, the user can hover the cursor over the cell and view a tool tip with the full cell value. 28 Multi-Category Enhancements Ascend 2018 Case New Rx Transfer Feature A new Rx Transfer feature is available for recording the transfer of an outpatient prescription and remaining refills to an external pharmacy. This feature allows a pharmacist to document the receiving pharmacy, receiving pharmacist, date/time of transfer, and other relevant information. New Pharmacies List for Rx Transfer A new Pharmacies List screen has been created for maintaining a list of external pharmacies where an outpatient prescription can be transferred. Use the Pharmacies List screen to view, add, edit, delete, or archive pharmacy records. Pharmacy information entered through this screen is saved to the Pharmacy database table. All active (unarchived) pharmacies are available for selection on the Rx Transfer screen when recording an outpatient prescription transfer. Select Utilities > Pharmacies to open the Pharmacies List screen. Page 23 Proprietary and confidential

29 Release Notes Multi-Category Enhancements View Pharmacies By default, all active pharmacies display alphabetically in the selection list on the left side of the screen. To view both active and archived pharmacies, check the Show Archived check box located below the selection list. Select a pharmacy from the list to display the pharmacy's information (Name, Address, Phone, DEA #) in the input fields. You may search for a specific pharmacy by entering the pharmacy name in the text box located above the selection list then pressing the Enter key. If a match is found, the pharmacy is automatically selected and the information displays. Click the Close button to exit the Pharmacies List screen. Add Pharmacy To add a pharmacy, click the Add button then enter the pharmacy Name, Address, Phone, and DEA # in the appropriate input fields. Name is the minimum information required to add a pharmacy. All other fields are optional. Click the Save button to finish. Document Number: ASPNR2018R1_01 Revision Date: February 27, 2018 Page 24

30 Multi-Category Enhancements Ascend 2018 Edit Pharmacy To edit a pharmacy, first select the desired pharmacy from the list then click the Edit button. Change the information as needed then click the Save button. Delete Pharmacy When you delete a pharmacy, it is removed from the Pharmacies List and is no longer available for selection on the Rx Transfer screen. To delete a pharmacy, first select the desired pharmacy from the list then click the Delete button. A message prompt will display asking if you are sure you want to delete the pharmacy. Click the Yes button to delete. Click the No button to close the prompt without deleting. A pharmacy cannot be deleted if a prescription has been transferred to that pharmacy. If you attempt to delete a pharmacy that has received a Rx transfer, the following message displays, Pharmacy will be archived as there are associated transferred prescriptions. Click the OK button to archive the pharmacy. Archived pharmacies are not selectable on the Rx Transfer screen. Archive Pharmacy You may archive a pharmacy so it is no longer selectable on the Rx Transfer screen. First select the desired pharmacy from the list then click the Edit button. Click the Archived check box then click the Save button. Archived pharmacies can still be viewed on the Pharmacies List screen by clicking the Show Archived check box. You may also unarchive a pharmacy by editing it then clearing the Archived check box. Page 25 Proprietary and confidential

31 Release Notes Multi-Category Enhancements Transfer an Outpatient Order To open the Rx Transfer screen and record an outpatient prescription transfer, select an outpatient order from the Orders tab then click Orders > Rx Transfer. Only outpatient orders with a status of active or hold can be transferred. If you select an order that is not eligible for transfer, you will receive a pop-up message stating that the order cannot be transferred. Only one order can be transferred at a time. Use the Pharmacies drop-down list to select the pharmacy where the prescription will be transferred. The address, phone, and DEA # for the selected pharmacy displays in the appropriate fields. These values cannot be edited. NOTE If the pharmacy you need to transfer the prescription to is not available in the Pharmacies drop-down list, click the Pharmacies link to open the Pharmacies screen and add the pharmacy information. When you are finished adding the pharmacy information, click the Close button. The newly added pharmacy is available for selection from the Pharmacies drop-down list on the Rx Transfer screen. Enter the name of the pharmacist who will receive the transfer in the Receiving Pharmacis text box. The Transferring Pharmacist field automatically displays the name of the currently logged in user and cannot be edited. Document Number: ASPNR2018R1_01 Revision Date: February 27, 2018 Page 26

32 Multi-Category Enhancements Ascend 2018 The grid in the bottom half of the Rx Transfer screen displays the following read-only information for the selected outpatient order. Rx # Description Start Date Stop Date Fill Date (Last Fill) Refills Remaining OrderRef To see additional order details for an order that has been selected for transfer, double-click the Rx item in the grid section on the bottom portion of the Rx Transfer screen. A read-only view of the Order screen displays. Click the Transfer button to record the transfer. To exit the Rx Transfer screen without transferring the order, click the Close button. Important! When an order is transferred, the order status is changed to discontinued and all remaining refills are removed from the order. Once transferred, the order status cannot be changed. However, a transfer can be cancelled. See the Cancelling a Rx Transfer section below for more information. Viewing a Transferred Rx To view a Rx that has already been transferred, select the appropriate order from the Orders tab then click Orders > Rx Transfer. The Rx Transfer screen displays the transfer information including the date/time of the transfer. All of the fields are read-only. Cancelling a Transferred Rx To cancel a transfer, select the appropriate order from the Orders tab then click Orders > Rx Transfer. The Rx Transfer screen displays the transfer information. Click the Cancel Transfer button to cancel the transfer. Page 27 Proprietary and confidential

34 Multi-Category Enhancements Ascend 2018 Prescription Transfer Report A new Transfer Rx report option is available on the Report List screen. After setting the desired filter values, click the Preview (or Print) button to view (or print) the report. The report is grouped by Facility, Patient, Rx # and includes the following information. Patient name and contact info Prescribing physician name and contact info Rx # and order information Refill information Comments including transfer date, receiving pharmacy name and contact info Page 29 Proprietary and confidential

35 Release Notes Multi-Category Enhancements NOTE The Prescription Transfer report can also be run from the Orders tab by selecting one or more transferred orders then selecting Print from the right-click menu. When the Labels/Forms screen opens, check the Rx Transfer check box then click the Print or Preview button. Security Enhancements A security enhancement has been made to allow an application or security administrator the ability to limit a user's access to the Rx Transfer option on the Orders Menu and the Pharmacies option on the Utilities menu. Select Form - Profile from the Security Object Types drop-down list on the Permissions tab of the Security screen. Once selected, a user's permission to the following new security objects can be set. 36 Multi-Category Enhancements Ascend 2018 Menu Orders Transfer Rx Menu Utilities Pharmacies Unless explicitly set, the permissions default to Full Permissions. Assigning No Permissions to a security object will result in the corresponding menu option being inactivated (dithered). When a user is assigned View Permissions to the Menu Utilities Pharmacies security object, the Add, Edit, and Delete buttons on the Pharmacies List screen will be inactivated (dithered). 38 Options Ascend 2018 Options The following enhancements / bug fixes have been made to the Options in Ascend: Case Default Form per Order Type An enhancement has been made to allow a default form to be configured per order type in addition to setting a Default Label form. For example, you may wish to have a Drug Education form pre-selected in addition to the label when printing an order. Previously, one default label could be configured for an order type on the Order Type Defaults screen. An enhancement has been made that also allows one default form to be configured for an order type. Follow the steps below to configure a default form for an existing order type. 1. Select Utilities > Order Types to open the Order Type Defaults screen. 2. Select an order type from the drop-down list on the left side of the screen. 3. Click the Edit button. 39 Release Notes Options 4. Select a form from the new Default Form drop-down list. 5. Click the Save button. Once a default form is configured for an order type, it will automatically be selected for printing on the Labels/Forms popup screen when an order of the correct type is entered. Additional labels and forms can be selected for printing in the usual manner, and the default form can be deselected as needed by clearing the associated check box. 40 Options Ascend 2018 NOTE Printing options configured on the Printing tab of the Ascend Program Options screen will remain in effect. For example, if the Drug Education form is configured as the default form for an Outpatient Prescription order type but is also configured as Do not make this label/form available for printing, then the form will not display on the Labels/Forms screen or be available for printing when an order is entered. 41 Release Notes Options Case / Patient Care Area - Edit Area Error Message When the Edit Area button on the Patient Care Areas screen is clicked, the Area popup window displays. Previously, if the user clicked the Edit Area button again while the Area window was already open, an unhandled exception error occured. The Area window has been modified to display as a modal window which will prevent the user from clicking outside of the Area window until it is closed via the OK button or Cancel button. 42 Order Entry Ascend 2018 Order Entry The following enhancements / bug fixes relate to Order Entry functionality in Ascend. Case Clinical Reviews Dialog Behavior The behavior of the X button when used to close the Clinical Reviews window during order entry or order verification has been modified. When the X button is clicked in this scenario, a Clinical Review Warning message displays. Click the OK button to close the message then use the appropriate button (Edit Review, Edit Order, etc.) to resolve the problem. The Clinical Review Warning also displays if you click the X button after adding a patient allergy that triggers a clinical review. Page 37 Proprietary and confidential

43 Release Notes Order Entry Case Error Message When Verifying Orders When verifying inpatient CPOE orders using Batch Modes - Verify Orders, the Order Note pop-up window displays when the order is opened. 44 Order Entry Ascend 2018 Case / Do Not Allow Order Verification When No Allergies Have Been Documented A new patient safety enhancement is available for alerting the pharmacist and / or preventing order verification when patient allergies have not been documented. Previously, the When No Allergy Data drop-down list on the Ascend Program Options screen could be configured to determine how order entry was handled for a patient who does not have any documented allergies. This feature has been enhanced to apply to both order entry and order verification. The When No Allergy Data options behave as follows during order verification. No Action - When the patient does not have any documented allergies and a new or existing order is opened for verification, the order can be verified without prompting the pharmacist for additional actions. No New Orders - When the patient does not have any documented allergies and a new or existing order is opened for verification, a popup message displays indicating that the order cannot be verified. When the pharmacist clicks the OK button, the patient's profile is opened so the pharmacist can document patient allergies. Allow Override - When the patient does not have any documented allergies and a new order or existing order is opened for verification, a popup message displays indicating that there are no allergies documented for the patient. 48 Order Entry Ascend 2018 Case Process Diagnosis Code Received in erx In an effort to combat the opiate abuse and addiction epidemic, states are implementing new opiate prescribing and dispensing guidelines and rules. States are now requiring a diagnosis code to be included in an e-presciption. The Ascend e-prescribe web service has been enhanced to populate the order Indication with the ICD code provided in the erx transaction. When the erx web service receives a prescription that includes an ICD code, the code will be looked up in the Indications table and the associated text description will be displayed as the Indication in Ascend. If the code is not found in the Indications table, then the ICD code from the HL7 message will be displayed instead. In addition, the formatted Note that displays on the erx Messages tab and once upon order verification in a popup message will include the ICD code from the message. NOTE More than one diagnosis code can be sent in the transaction. Ascend allows only one Indication, and the back-end reporting to the Prescription Drug Monitoring Programs via ASAP allows one diagnosis code. 50 Order Entry Ascend 2018 Case Retain Order Location When Refilling Outpatient Order When an outpatient order is created for a patient, the Location set on the Patient Admission Information screen is assigned to the order. The Location can be changed by selecting a different value from the Location drop-down list on the Other tab of the Order Entry screen. Previously, when the Location was changed in this manner and the order was refilled, the Location reverted back to the Location set on the Patient Admission Information screen. An enhancement has been made so that each subsequent refill retains the Location that was set on the prior order instead of reverting to the Location set on the Patient Admission Information screen. Case e-prescribe Patient Matching Should Consider Admission Status Previously, when an e-prescription was received via Surescripts, the patient's admission status was not taken into consideration when matching the prescription to a patient admission in Ascend. Occasionally, this resulted in a prescription being created on an old or discharged patient admission instead of the active admission. This issue has been resolved. After a patient is matched, the admission date of each admission will be checked in reverse chronological order (newest to oldest) to determine if the admission is active or discharged. When an active admission is found (discharge date is today or a future date), the prescription will be created on that admission. Case New emar Security Settings The Ascend emar (electronic medication administration records) is an optional web-based application available to Ascend users. An enhancement has been made to Ascend security to allow emar administrators the ability to restrict access to certain emar features to only authorized users and groups. The new emar security options are only applicable to the Ascend emar application. A new emar option has been added to the Security Object Types drop-down list on the Permissions tab of the Security screen.
