HOW TO SIGN UP While in your doctor s office, inform staff that you wish to enroll in the Patient Portal. We will require you to complete the Ezra Medical Center Portal Consent Form prior to using the Patient Portal. If you are requesting access on behalf of another Ezra Medical Center patient (e.g. as a legal guardian) then you must complete the relevant Authorization Form and provide documentation in accordance with our policies and procedures to verify that you are such patient s personal legal representative. After completing the EMC Portal Consent Form and if applicable an additional Authorization, an invitation email will be sent to you at the e-mail address you provide on the consent form. After reading the instructions select the appropriate Patient Portal login link contained in the email to complete the enrollment process. Follow the prompts which will guide you through the enrollment process. Create and remember a password, you will be using it each time you log into the site. We suggest you keep your password in a secure location, if you forget your password you must request a password reset from your login screen. New to Nextgen Patient Portal Link Or Already have a Nextgen Patient Portal Account 2
New to NextGen Patient Portal If you are new to NextGen Patient Portal, review the Terms and Conditions then click I Accept. Next enter your information include the email where all portal notifications will be sent, and click Submit. 3
New to NextGen Patient Portal (Cont.) Next create your enrollment credentials and click Submit.!DON T FORGET TO ENTER THE CAPTCHA INFORMATION WHICH WILL BE ON THE BOTTOM RIGHT OF THE SCREEN! After all your information is submitted, a notification will pop up informing you that you request has been submitted to our office and you will receive an email once its been approved. 4
Existing Portal Account Log-in If you have an existing portal account, select the appropriate link from your enrollment notice. There are two log in options: Option 1 Review the Terms and conditions and click Accept. Enter username and password and click Login. Enter the answer to your security question and click Submit. A pop-up will appear stating that you are already enrolled, click Take Me to My Account Option 2 Enter your user name and password in the Already a member; and click Log-in. Review the Terms and conditions and click Accept. Enter the answer to your security question and click Submit. 5
Resetting Username and/or Password If you forgot your log on information click Need help with your username and password? and follow the instructions to reset. 6
Congratulations! You re in the Portal! This is how the portal home screen will appear. Please note: When a new message is available in the Portal, you will also receive an alert sent via email to your personal email account. How to Change your Account Settings From the right hand upper navigation bar under settings click Account Settings. From here you can change your username, password, security questions and more. When finished click Submit 7
Navigating the Home Page To begin you may navigate through the tabs at the top right of your page. Home clicking on this will take you back to the main homepage Mail This will display a dropdown to access your inbox and to compose new messages. Schedule This will display a dropdown to view or request appointments. My Chart This will display a dropdown where you can view your basic clinical history or to request records. Renew Medications This will display a dropdown to submit a medication renewal request. Patient Education This is a separate entity not run by Ezra Medical Center, it s operated through 1995-2016 Healthwise, Incorporated. Here you ll be able to find some educational materials. But remember the information displayed does not replace the advice of a doctor, so any issues or concerns you may have schedule yourself an appointment with your Physician. 8
Navigating the Home Page (cont.) To begin you may navigate through the tabs at the top right of your page, or select from the options from the main home screen page. Practice information Click view profile page for our office contact information. Inbox Click the word inbox to view all documents in your mailbox including appointment requests, record requests and all other communications with our office. You can also click compose an email to communicate with our office regarding medication refills and records requests. Upcoming Appointments This will display all upcoming appointments in our office that were scheduled via the portal. If you d like to reschedule the appointment please request via the portal so it can be properly updated and displayed. Upcoming Appointments if you click on the words it will show you all your upcoming appointments. In your history if you click the words include past history your full appointment history will be displayed. 9
What do the icons in my Inbox mean? - Unread message - Read message - Appointment response - Medication response - Document - Template to be completed - IMH form to be completed - Statement to be paid - Personal Health Record Unread Message The unread message icon will be seen in your inbox when you receive a new message via the portal that has not been opened. Read Message The read message icon will be seen in your inbox after you have opened and closed a new message. Medication Response The medication response icon will be seen in your inbox after you have requested a renewal of existing medication requests. Attachment The attachment icon will be seen in your inbox when you receive a message that has documents attached to the message. Documents The documents icon will be seen in your inbox when you receive a document via the portal. How to Send a Secure Message On the home screen or upper right navigation bar, under Mail, click Compose Message. Fill in your message just as you would with other common email applications and click Submit. Replies from your physician will appear in your inbox. 10
How to Request an Appointment On the home screen or upper right navigation bar, under Schedule, click Request Appointment. Fill out the request form with your preferred appointment information. When you re finished entering your information, click Submit. You ll receive a call from our office and a portal inbox confirmation when your appointment is booked. 11
How to Renew Medication On the Home screen or upper right navigation bar, click Renew Medication. You will see all active medications prescribed by your physician. OR 12
How to Renew Medication (Cont.) Fill in your medication and pharmacy information and click Submit. 13
How to Request your Labs/Reports If you are requesting specific test results/reports from previous visit(s) in the Mail category select Compose Message, select Category: Medical records. Compose your email accordingly and click Submit. 14
