All controlled substances will require the prescriber to electronically sign the prescription prior to transmitting it to the pharmacy. Emdeon has partnered with Verizon to provide the digital signing using their 2 factor authentication process. This document will walk the prescriber through the Verizon registration process. Verizon Registration Process: 1. Determine the mobile device you will use to generate the unique passcode that will be needed each time you submit an eprescription for a controlled substance a. On this same mobile device download the Verizon Universal Identity app via the App Store or Google Play b. Keep this mobile device near you as you complete this registration process. 2. Go to the email the prescriber, who is the DEA Registrant, received from UIS@betrusted.net and click on the here hyperlink a. This will launch the Universal Identity Services website where the prescriber will complete the online registration 21073.1.Rev004 06.17.2016 1
3. Enter contact information a. Name must be the prescriber s legal name found on their DEA License. This is the name that you had also provided Patterson during enrollment. If this is incorrect, please contact eservice Sales at 1-800-294-8504. b. The address must be the prescriber s home address. 4. Choose a suggested Username or create one of your own a. Make note of username as it will be needed should you need to access this portal in the future 5. Create a password a. Make note of the password as it will be needed should you need access to this portal in the future and it will also be used in Step 22 6. Create your own 4 digit PIN a. 8. Select Setup listed in the Smart Phones, Tablets and other Devices section 9. Select the device type that you downloaded the app on in Step 1, enter in the Device Name that best describes that device (i.e. Dr. cell phone) a. If you have multiple devices you will be using then choose Add Another and enter in the Device Type(s) and Device Name(s) b. It is highly recommended that you add at least two devices in the event something happens to one or the other you are still able to access the app. 10. 12. On the device you installed the Verizon Universal Identity app, open the Verizon Universal Identity app. 13. From the Verizon portal select Get Activation Code next to the device you have the opened the app on 14. The below screen will appear, enter this Activation Code in your app on your device. You need to do this within 5 minutes or the Activation Code will expire. 15. Within the app you will also need to enter in the PIN you created in Step 5 16. You can now close the app on the device and you can choose Close to the message which displays the activation code in the portal 17. Repeat steps 11-16 for each device you added in Step 8 18. 20. Select View Certificate 21. You will be prompted to enter in a 6 digit soft token. You will obtain this by going to one of the devices you registered earlier and you will open the Verizon Universal Identify app and a 6 digit code will appear. Verification was Successful 1. You will receive the below message if your registration was successful. 2. This is the email you will receive if registration was successful. a. Received Medium Level certification 1. If you received medium level certification you will receive the below message and email. 2. Although you received medium level certification you need high level certification in order to submit prescriptions electronically for controlled substances. 3. From the message indicating your certification is at medium level you will need to do one of the following. a. Option 1: Choose Select here to download the PDF version of the notary form i. You will need to complete this form and then bring to a notary b. Option 2: From within the Verizon portal go to Increase my Security under Universal ID on the left hand side of the screen i. Select on that screen select to verify your data again and attempt to answer the questions a second time ii. Verification was Unsuccessful: 1. If online verification was unsuccessful the prescriber will see the below screen with the following message appearing in the red highlighted box. UIS was unable to issue you a Verizon signing Certificate at this time. You can log into Profile Manager to retry this step. More instructions will be provided in the next page. Please click on Next to proceed. 2.
