60 Days to Clinical Communications Success! Session 6, March 6, 2018 Joseph Wolfgram, Chief Technology Officer, Mission Health 1
Conflict of Interest Joseph Wolfgram, NLP-P, MCHt Has no real or apparent conflicts of interest to report. 2
Learning Objectives Share how to establish an effective clinical mobile communications pilot for enterprise deployment Identify technologies that are effective and proven in the real world Compare quality and efficiency metrics, both pre- and postimplementation 3
THE SITUATION THE PLAN AGENDA THE PILOT THE RESULTS
WESTERN NORTH CAROLINA 6 HOSPITALS BEDS AT FLAGSHIP 120 LOCATIONS ACROSS 18 COUNTIES About Mission Health 15 SPECIALTIES & SUB-SPECIALTIES 12,000 CARE PROVIDERS 1,000 PHYSICIANS $1.8 BILLION IN ANNUAL REVENUE 75%+ - MEDICARE/AID, SELF-PAY TOP 15 HEALTH SYSTEM (TRUVEN) 5 TH TIME IN 2017
The Situation Clinical environment had multiple modalities for communicating Multiple applications need integration with a communications device There were competing devices and multiple middleware platforms The scenario was rapidly getting more complex, and confusing Prior unsuccessful deployments of smartphone and Wi-Fi handsets Care providers had lost confidence in IT s ability to deliver reliably 6
What We Wanted A single communications device across the enterprise Reliable Voice Calls over our existing Wi-Fi network Secure Messaging that works like existing SMS Texting apps Nurse Call and Clinical Monitoring Device Alerts Bar Code scanning for Rx Administration / positive patient ID Staff Duress alarm, Real-Time Location/Presence, Proximity Aware A simplified middleware communications platform Able to integrate with multiple systems Reduce complexity of managing multiple platforms 7 Standard Device Enterprise-Wide Communications Platform EHR BioMed Alerts
What We Wanted to Avoid Dedicated Device Per Use Case Communications Platform Dedicated Device Per Use Case Communications Platform Dedicated Device Per Use Case Communications Platform Telemetry EHR BioMed Alerts 8
Get Clear on the Device Device Attributes Healthcare-Grade Hardware; No need for additional cases or clips One-handed operation / light-weight, fits in pocket of scrubs Long battery life / quick-swap battery OR quick-swap device Simple to assign to staff, with NO NEED to login Technical Requirements Reliable, Scalable, and Manageable 9
Device Success Factors ADOPTION IS GOAL #1 Device must be used: ON, ASSIGNED, and LOGIN complete Device assignment MUST be: Fast, Easy, and Reliable If it s not being used universally, the project is dead Partner with clinical leadership to remove old communication devices: pagers, WiFi phones, etc. to promote adoption RELIABILITY IS EQUALLY IMPORTANT Wi-Fi connects, re-connects, and alerts when out of signal range Super fast & easy device swap for any reason User data, preferences, and Directory Updates - AUTOMATIC 10
The PLAN IDENTIFY a single DEVICE that will meet all requirements Conduct a PILOT with a limited number of eager adopters Identify desired outcomes in advance and track metrics Determine Go / No-Go post-pilot based on actual outcomes Start with limited features, and expand functions over time 11
The PILOT Communicate & Set Expectations Limited features to start, expanding over time Structured process with defined steps at each phase Established success criteria in advance User adoption and device utilization metrics Reliability of features (call quality) and hardware (durability) Track decline of undesired actions (overhead pages, locating staff) Training requirements and ease of use as reported by care providers 12
The RESULTS Feature Utilization Over the 60-day pilot period, the Ascom Myco and Context solution was used more than 75,000 times, as seen in the adjacent chart. This activity level corresponds to 471 communications per trained caregiver during the pilot period. 13
The RESULTS Feature Utilization Immediately after launch, there were 13.9 texts per call. Within two weeks, the ratio had climbed to 21.9 texts per call, and has stayed at about that level ever since. 14
The RESULTS 77% Doubling of caregivers who agree that their unit has a rapid, reliable means of communication, from 37% to 77%. 13% The increase in caregivers who concur that the communication environment allows for timely communication of codes, from 61% to 74%. 20% The reduction in nurses who believe communication is a barrier to successful patient care, from 66% to 46%. 15
The RESULTS Satisfaction with the time it takes to page and get a call back from a doctor has tripled, as has satisfaction with the time it takes to locate and speak to a CNA. Satisfaction with the time it takes to locate and speak to an RN has nearly quadrupled. Satisfaction with the time it takes to locate and speak to security or a HUC increased substantially, quadrupling and up 20% respectively. 16
The RESULTS 63% of respondents agreed that the Ascom solution helped them reduce the number of steps they take in a day 43% say that overhead paging is drastically reduced, creating a quieter environment for both patients and caregivers. 17
Next Steps We are expanding our deployment Goal of more than 2,000 devices enterprise-wide Standardized platform across all major functions Clinical Nursing, Physicians, Environmental Services, Transport, etc. We are planning our feature expansion Clinical Engineering / BioMed device integration Bedside Telemetry monitoring / Results and Alerting Bedside Rx Administration & Positive Patient Identification Staff Duress Alerting and Proximity Sensing for advanced applications 18
