1 All products and services ordered through Travisa Visa Service must have a completed Global Service Order Form. This form tells Travisa what you are ordering, how you are paying, and where the documents are going upon completion. It also provides Travisa Visa Service with your contact information, in the event that the Consultant handling your case needs to follow up with you. This is the first screen of the Global Service Order Form. The and Date of Birth entered here will act as a username for checking on your order during its process. 1

2 This first screen is where you list all the people applying for visas. If you require Passport processing, you can add that to the order here. This may include renewing a passport, or adding Visa pages to your current passport. You can list multiple persons on a single Global Service Order Form. These dates let Travisa know when you need your documents back by. 2

3 On this page, you will list all the visas that you will be applying for. This page should begin by displaying BRAZIL. Select TOURIST for Purpose of Trip. Your location should automatically load in the next field, based on your State of Residence. Select BRAZIL REGULAR for processing speed. Once you ve added the information for your Brazil visa, please click the, Add Another Travel Visa button. 3

4 Select RUSSIA for Country Select TOURIST for Purpose of Trip Select SINGLE ENTRY All Semester at Sea applicants should select SAN FRANCISCO as the processing location of their Russian visa. Select RUSSIAN REGULAR for processing speed. After adding your Russian visa to your order, again click the Add Another Travel Visa button. 4

5 Select GHANA for Country Select TOURIST for Purpose of Trip Select MULTIPLE ENTRY All Semester at Sea Ghana visas should be processed in WASHINGTON, unless you need Rush Processing. All Rush processing Ghana applications should go to HOUSTON. All applicants will require 4 recent passport photos for their visa applications. If it is more convenient, applicants can upload a jpeg photo here, for Travisa to print for you. Once you ve added all your visas to this page, click Continue. 5

6 List the primary contact information here. This is how Travisa will contact you, if necessary. This is where you select how your documents will get to and from Travisa. You can use Travisa s FedEx account to ship in one or both directions, or opt to ship it yourself, and provide a pre-paid return air bill. Travisa highly recommends using FedEx for shipping your passport. If you opt to ship your documents to Travisa using their FedEx account, a prepaid air bill will print along with the Global Service Order Form. 6

7 If you have a different shipping address than your contact address, list it here. Travisa will notify you your documents are being shipped via , if you d like a text message as well, complete this field. Choose your payment method in this field. Please note that Travisa will not charge a credit card listed here until all documents have been processed and Travisa is ready to ship your documents back. 7

8 Click one of these two options to print your Global Service Order form If you opted for a prepaid air bill to ship your documents to Travisa, click here to generate the shipping label. 8
