The FUJIFILM MEDICAL SYSTEMS U.S.A. (FMSU) training schedule i s s h o w n from January 1 st 2019 through December 31 st Disclaimer: All dates and fees are subject to change based on resource availability as determined by FMSU management. Registration is not final until you receive a confirmation from FMSU. FMSU is not responsible for costs associated with travel or lodging in the event a course has been rescheduled. If a scheduled class is cancelled by FMSU or the registrant, students can be rescheduled to the next available or next most convenient date. No refunds will be issued. If the requested class has prerequisites, the prerequisite classes must be completed before you can attend the class originally requested unless you have specific confirmation of a waiver or temporary waiver of the prerequisite course through the training team. 5 2019 COURSE DESCRIPTIONS FDX Console - Course # (Recommended Prerequisites: Basic X-ray Training or equivalent field experience) This course will provide training on Software Installation, Configuration, Setup, Network Configuration and Image Processing for Synapse as they pertain to the FDX Console. Image Acquisition & Distribution, User Utility & Service Utility Functions and DICOM Tag Edit Tool are among the numerous subject matters discussed and applied. (Classroom Lecture/PowerPoint and Hands-On Lab, Written Test). Dry Pix Course # This course introduces Fuji's newest addition to the DryPix family and provides an overview of the DryPix Topics include Equipment Installation, Film Handing and Transport, Equipment Service, Preventative Maintenance, Equipment Operation and Network Printing. (Classroom Lecture/PowerPoint and Lab, Written Test). Dry Pix 4000/5000/ Course # This course introduces Fuji's newest additions to the DryPix family. In this course we will explore the following topics of Fuji's fastest dry laser imagers: Equipment Installation, Film Handing and Transport, Equipment Service, Preventative Maintenance, Equipment Operation, Film Sorting and Network Printing. (Classroom Lecture/PowerPoint and Lab, Written Test). Fuji CR Readers - Course # (Recommended Prerequisite Training: FDX Console) This course provides an overview of the Fuji FCR Carbon, XG5000 and the ClearView CS. Topics include: Machine Description, Software Control, Installation, FDX Console Configuration, Image Plate Handling, Patient Registration, Image Data Flow, Client-PC Software, Hands-On Subassembly Removal & Reinstallation, Parts Replacement, Calibration, Troubleshooting and Preventive Maintenance. (Classroom Lecture/PowerPoint and Lab, Written Test). FDR D-EVO and D-EVO 2 - Course # (Recommended Prerequisite Training: FDX Console) calibration, troubleshooting and maintenance of the D-EVO and D-EVO 2 products including the various peripheral components associated with them. The course curriculum will also provide details for setup of the Interface Box and FDX Console settings required to connect to both. FDR D-EVO GL long panel - Course # (Recommended Prerequisite Training: FDX Console, FDR DEVO/DEVO 2) calibration, troubleshooting and maintenance of the FDR D-EVO GL product including the various peripheral components associated with it. The course curriculum will also provide details for setup of the FDX Console settings required to capture scoliosis and long length images.

6 FDR Aqro - Course # (Recommended Prerequisite Training: FDX Console and FDR D-EVO/DEVO 2) The FDR Aqro course is designed to provide a system level approach to Digital Radiography Mobile X-Ray system installation and maintenance. We start with an overview of the system design, and train the engineer to install and setup the X-Ray System. For system maintenance, our training and lab work covers complete system adjustments, alignments, and calibration, fault isolation to the Field Replaceable Units, reviewing the battery removal and replacement procedures, interfacing the FDX console to the hospital network and preventive maintenance. (Classroom Lecture/PowerPoint and Lab, Written Test). FCR GO 2 - Course # (Recommended Prerequisite Training: FDX Console and Fuji CR Readers) The FCR Go class is designed to provide a system level approach to Mobile X-Ray. Beginning with an overview of the system architecture and trains the engineer to installation and setup of a mobile X-Ray System Laboratory work entails complete system adjustments, alignments, and calibration of a Mono X-ray Tube (KV and MAS). Fault isolation to the Field Replaceable Unit along with battery removal and preventive maintenance procedures are also covered. Interface to the hospital network is covered through service documentation. (Classroom Lecture/PowerPoint and Lab, Written Test). FDR GO and FDR Go Plus - Course # (Recommended Prerequisite Training: FDX Console and DEVO/DEVO II) The FDR Go and Go Plus course is designed to provide a system level approach to Digital Radiography Mobile X- Ray system installation and maintenance. Beginning with an overview of the system architecture, we train the engineer to install and setup the mobile X-Ray System. For system maintenance, our didactic training and laboratory work entails complete system adjustments, alignments, and calibration, fault isolation to the field replaceable units, reviewing the battery removal and replacement procedures, interfacing to the FDX console to the hospital network and preventive maintenance. (Classroom Lecture/PowerPoint and Lab, Written Test). Aspire HD Plus (FDR MS-2000/2500 System) - Course # (Recommended Prerequisite Training: FDX Console calibration, troubleshooting and maintenance of the FDR-MS-2000/2500 Aspire HD Plus System and the various peripheral components associated with it such as the FDR-2000AWS Console. Cristalle FDR MS-3500 System - Course # (Recommended Prerequisite Training: FDX Console calibration, troubleshooting and maintenance of the FDR-MS-3500 Cristalle System and the various peripheral components associated with it such as the FDR-3000AWS Console.

7 FDR Clinica - Course # (Recommended Prerequisite Training: FDX Console and DEVO/DEVO 2) calibration, troubleshooting and maintenance of the FDR Clinica and the various peripheral components associated with it (table, floor mounted tube stand, and wall stand). The course curriculum will also provide details for setup of X-ray Control (XCON/SpeedLink) as it relates to the FDR Clinica x-ray generator. FDR D-EVO Suite II - Course # (Recommended Prerequisite Training: FDX Console and DEVO/DEVO 2) calibration, troubleshooting and maintenance of the D-EVO Suite II and the various peripheral components associated with it. The course curriculum will also provide details for setup of X-ray Control (XCON/SpeedLink) as it relates to the Polydoros GEN RF x-ray generator. FDR AcSelerate (FDR 200 System) - Course # (Recommended Prerequisite Training: FDX Console, Basic X-ray CBT) calibration, troubleshooting and maintenance of the AcSelerate and the various peripheral components associated with it. The course curriculum will also provide details for setup of X-ray Control (XCON/Speed Link) as it relates to the AcSelerate device, EMD Epsilon EPS80RF generator and the DX Console. FDR AcSelerate - Advanced Applications (second week of FDR 200) (Recommended Prerequisite Training: FDX Console, and FDR AcSelerate Week 1) This course is designed to train the engineer on the installation, setting to work adjustments alignments and calibrations, and maintenance of the FDR AcSelerate advanced applications. This includes the Tomosynthesis application; Energy subtracted imaging, Linear Tomography and the Digital Scoliosis stitching function. FDR Visionary Suite DR-ID 900 system - Course # (Recommended Prerequisite Training: FDX Console), FDR DEVO/DEVO II calibration, troubleshooting and maintenance of the FDR Visionary Suite and the various peripheral components associated with it. The course curriculum will also provide details for setup of X-ray Control (XCON/Speed Link) as it relates to the FDR Visionary Suite device, Visionary Suite X-ray generator and the FDX Console. FDR Visionary Suite- Advanced Applications (second week of DR-ID 900) (Recommended Prerequisite Training: Console, and FDR FDR Visionary Suite Week 1) This course is designed to train the engineer on the installation, setting to work adjustments alignments and calibrations, and maintenance of the FDR Visionary Suite advanced applications. 8 Digital Radiography - Course # This course is designed to train Fuji Digital Radiology users to become Super Users and provide them the information and tools to be on-site experts. This focused off-site training offers benefits beyond our customer on-site offerings by providing a focused learning environment with uninterrupted access to the complete line of Fuji CR and DR products, Fuji printers and Synapse PACS. The curriculum incorporates many didactic sessions supported by hands-on clinical sessions using operational X- ray equipment and phantoms. Students will learn all aspects of Digital Radiology imaging, from image capture to image processing and display. A complete day is dedicated to the QA process for image capture and display. Advanced user options, such as security settings and image stitching are taught as well as site accessible User Utility equipment functions. The student must have at least 3 months experience with Digital Radiology products. (Classroom Lecture/PowerPoint and Lab, Written Test). Stamford Learning Institute Education Programs provided by Professional Services.

9 FUJIFILM MEDICAL SYSTEMS USA, INC. DISTRIBUTOR / CUSTOMER SERVICE TRAINING AGREEMENT As a condition to attending a Fujifilm Medical Systems USA, Inc. ("FMSU") specialized service training course, I, (" the attendee") an employee of agree to the following terms and conditions: 1. All information (including but not limited to data, know-how, trade secrets, methods and procedures, designs) provided to the Attendee in connection with the service training course, are proprietary to FMSU and / or Fuji Photo Film Co., Ltd. (collectively "Fuji"). 2. This information shall remain the property of Fuji and shall only be used by the Attendee in connection with the operation, service, repair or maintenance of Fuji brand medical diagnostic imaging equipment. 3. Attendee understands the proprietary nature of the information and agrees to take every reasonable precaution to protect such information from disclosure to third parties. Attendee will not copy or reproduce any written material provided to Attendee in connection with the service training course. 4. Attendee agrees to indemnify FMSU against any losses incurred by FMSU, including reasonable counsel fees resulting from the breach of any provision of this agreement by Attendee. 5. In the event that the Attendee's employer's equipment is sold, destroyed or otherwise disposed of, the above described information shall be returned immediately to FMSU. 6. The Distributor shall comply with all export and re-export restrictions and regulations imposed by the government of the United States and other relevant countries or regions ( Export Restrictions ). 7. The Distributor shall not knowingly transfer, directly or indirectly, any restricted software or technical data received hereunder or the direct product of such data, to any country or region or to those entities or persons listed on the website of Bureau of Industry and Security of U.S. Department of Commerce as Denied Persons List, Unverified List, Entity List, Specially Designated Nationals List and the Debarred List; identified as an embargoed destination or country in the Export Restrictions, unless prior written authorization is obtained from FMSU and each appropriate United States or other government agencies. 8. Attendees will not solicit, or assist in any way in the solicitation of, business from any of FMSU s or its affiliates customers, either for their employers or their own benefit or the benefit of anyone other than FMSU, unless the business being solicited is not competitive with the services or products provided by FMSU or its affiliates. 9. I understand all of the requirements and responsibilities that come with Fujifilm Medical Systems, USA service training and have read the above conditions and cautions on the export control of the United States and by signing below, confirm to follow the instructions.
