1 Purpose Laptop for Teachers Program Cathedral High School The Laptop for Teacher s Program is designed to help Cathedral teachers create educational materials that are laptop/mobile friendly to enhance technology integration in the classroom. Eligibility Any teacher, full-time or part-time employed at Cathedral High School is eligible to submit an application. Application Process Teachers must submit an application letter to the Department of Information Services no later than Monday, January 19, Selection Process A selection committee will review the applications and will select seven (7) winners. The following questions will help determine winners of the program: Please identify how a laptop will enhance your current curriculum and benefit students at CHS. How will using a laptop further technology integration in your classroom? What do you think those benefits will do for you as a teacher? Are you willing to mentor other teachers to help them improve the curriculum or to enhance their technology integration into the classroom? How will this program enhance your personal life? Winners will be announced on Wednesday, January 28, 2009 during the Technology Professional Development Day. Winners are also not eligible to participate in the program until the end of term of their program contract. Warrantees All laptops will be under the warrantee through Lenovo for hardware and Cathedral High School for software use for the duration of the Laptop Program agreement. Lost, damaged, or stolen laptops will not be covered under any warrantee or policy by Lenovo or Cathedral High School. In addition, winners of the laptop program are not entitled to a replacement laptop if repairs or a replacement cannot be covered under any existing policy. Specifications Lenovo ThinkPad T U P8400(2.26GHz), 1GB RAM, 80GB 5400rpm HD, 15.4in 1280x800 LCD, Intel X4500HD, CDRW/DVD, Intel agn wireless, Bluetooth, Modem, 1Gb Ethernet, UltraNav, Secure chip, Fingerprint reader, 6c Li- Ion, WinXP Pro Training/Mentoring Cathedral High School will provide training and support on the effective use of the laptop throughout the duration of the laptop program. Teachers will be expected to provide peer mentoring and support when needed for future laptop award winners.
2 Laptop Program Evaluation Process Laptop winners will meet formally and informally to evaluate the Laptop Program and its impact on technology integration at Cathedral High School. Technical Support Cathedral High School Information Services will maintain and support a laptop image of software that is consistent with current software programs available on teacher s computers at Cathedral High School. Upon approval, award winners will be allowed to install personal software that is properly licensed for their personal use. However, Cathedral High School, and the Department of Information Services, will only support the original software installed on the laptop from the Department of Information Services. Any addition software will not be support by either party. How is this program funded? The Laptop for Teacher s Program is currently funded through the IBM Matching Donors Grant Program. Employees of IBM/Lenovo can donate up to $5,000 per year to Cathedral High School. IBM will match 4:1 for every dollar donated, providing up to $20,000 to Cathedral High School per individual, per year.
3 Laptops for Teachers Program Laptop Use Agreement Cathedral High School I confirm that I am eligible to accept Cathedral High School s Laptop for Teacher s Program offer. I confirm that I have read, understood and agree to the attached Terms and Conditions of Use and am willing to participate in the Program subject to these Terms and Conditions of Use and such other policies as are determined by Cathedral High School Information Services (I am aware that the related policies can be located on the Cathedral High School Information Services website and that they may be updated from time to time at the discretion of the Department of Information Services). By signing the Laptop for Teachers Program Registration Form, I agree to the following terms and conditions and accept delivery of the laptop. Lenovo ThinkPad T500 Serial Number: Asset Number: Employee name (please print) Employee signature & Date Contact Numbers: Work: Home: Mobile: DEPARTMENT OF EMPLOYMENT, EDUCATION AND TRAINING Laptop for Teachers Program Terms and Conditions of Use 1.1. I agree that the laptop at all times remains the property of Cathedral High School and that the Laptop is provided for my use as a teacher to assist me in developing educational learning materials for classes taught at Cathedral High School I may use the laptop for a 4-year academic period from the date of delivery of the Laptop package I undertake to keep the laptop in good working order and to notify the Cathedral High School Help Desk immediately of any defect or malfunction of the laptop while in my care I will use the services of the Authorized Repair Agents (as advised by IBM and knowing that these may change from time to time) for any repairs or maintenance that may be required I will use the laptop package lawfully and in accordance with Cathedral High School Acceptable Use policies which may change from time to time, regarding the ethical use of technology, use of legal software, use of the Internet and the protection of personal data I will not sell, assign, transfer or otherwise dispose of the laptop package If my employment status changes with Cathedral High School, or if I breach any of these terms or conditions, Cathedral High School may revoke this arrangement by giving me written notice In the event that I cease or vary my employment with Cathedral High School I will return the laptop package to the school in good working order on or before my last day of employment.
4 1.9. The consent of Cathedral High School is required to adjust or alter the laptop package by the addition or deletion of any hardware (excluding peripherals). I will rectify any such adjustments or alterations before the laptop package is returned (if required) I acknowledge that I cannot substitute the laptop package I have been provided with any other laptop package (which is now or may subsequently become available) during the term of the 4-year period I will take due care of the laptop package at all times, including Not leaving the laptop unattended in a public place. Not leaving the laptop unattended or unsecured in a classroom or other place in the School. Not leaving the laptop in plain view in an unattended or unsecured vehicle. Not allowing the laptop to be accessed by any other person (unless authorized by you). Not allowing the laptop to be interfered with, tampered with or altered by a third party or otherwise except in accordance with clause 1.9. Ensuring due care is taken in the handling, transporting and usage of the laptop I will not remove, conceal or alter any laptop package markings, tags or plates or engrave or mark the Laptop in any way that will reduce the value of the laptop If the laptop is lost, stolen or damaged I will advise the Principal and the Department of Information Services and/or the Police as soon as possible I will participate in any evaluation/s of the Laptop for Teachers Program I will not allow my network user account and password to be used by anyone other than myself, unless required by the Department of Information Services I understand that, due to the Department of Information Services current software licensing arrangements covering home use, the laptop package cannot be used by me for any commercial purpose.
