1 D/B/A CENTURYLINK QC (C) INTERCONNECTION AND TITLE PAGE UNBUNDLED ELEMENTS Sheet 1 ISSUED: July 19, 2011 DEFINITIONS, RATES AND CHARGES Applicable to Interconnection and Unbundled Elements within the operating territory of QWEST CORPORATION d/b/a CenturyLink QC (C) whether offered under that name, or the trade or brand name CenturyLink (N) in the State of OREGON as provided herein. ADOPTION NOTICE Effective April 4, 2011, Qwest Corporation registered the fictitious name CenturyLink QC. Effective August 8, 2011, Qwest Corporation began operating under the name CenturyLink QC. As such, Qwest Corporation d/b/a CenturyLink QC hereby adopts, ratifies, and makes its own, in every respect as if the same had been originally filed by it, all schedules, rules, notices, concurrences, schedule agreements, divisions, authorities or other instruments whatsoever, filed with the Public Utility Commission. By this notice, Qwest Corporation d/b/a CenturyLink QC also adopts and ratifies all supplements or amendments to any of the above schedules, etc., which Qwest Corporation has heretofore filed with said Commission. 2 UNBUNDLED ELEMENTS Sheet INTERCONNECTION The provisions in this section apply when unbundled elements are used in conjunction with interconnection to the Qwest network for the exchange of traffic. Interconnection is the linking of Qwest's and the Carrier's networks for the mutual exchange of traffic and for the Carrier's access to unbundled network elements. Interconnection does not include the transport and termination of traffic. Interconnection is provided by entrance facilities or Mid-Span Meet arrangements. Qwest will provide interconnection at the lineside of the local switch, the trunkside of the local switch, trunk interconnection points of the tandem switch, and central office cross-connect points. 1.2 ACCESS TO UNBUNDLED ELEMENTS A. Qwest will provide the following unbundled network elements in the following categories: Local Loops Cross Connect Facilities Switch Port Interconnection Network Elements Switching Interoffice Transport Switching Features Channel Performance Ancillary Services B. Unbundled Loops 1. An unbundled local loop establishes a transmission path between the Qwest distribution frame (or equivalent) and Qwest s network interface device (NID). An unbundled loop is also called a Network Access Channel (NAC). Descriptions of the types offered as unbundled network elements are set forth in E., following. For existing loops, the inside wire connection to the NID will remain intact. 2. Basic unbundled loops are available as a two-wire or four-wire, point-to-point configuration suitable for local exchange type services within the analog voice frequency range of 300 to 3000 Hz. For the two-wire configuration, the Carrier is required to specify loop start or ground start option. The actual loop facilities that provide this service may utilize various technologies or combinations of technologies. Basic unbundled loops will provide an analog facility to the Carrier.

Basic Installation Installation of an unbundled loop requires wiring a cable pair to the requested position (e.g., CFA) at the Carrier s presence in the central office. The work is done at a time determined by Qwest on the agreed to due date. b. Basic Installation with Conformance Testing Results Provided Installation of an unbundled loop requires wiring a cable pair to the requested position (e.g., CFA) at the Carrier s presence in the central office. The work is done at a time determined by Qwest on the agreed to due date. The loop installation will include specific testing (e.g., 3 tone slope, C-message noise, cable free of electrical fault, 1004 Hz loss, less than or equal to 8.5 db). c. Coordinated Installation with Cooperative Testing Installation of an unbundled loop requires wiring a cable pair to the requested position (e.g., CFA) at the Carrier s presence in the central office. The loop installation will include specific testing (e.g., 3 tone slope, C-message noise, cable free of electrical fault, 1004 Hz loss, less than or equal to 8.5 db). The installation will take place at a designated time (within the normal Monday thru Friday 8 a.m. to 5 p.m. work day) previously agreed to by Qwest and the Carrier. If the Carrier requests installation outside of normal business hours, additional labor charges will apply. d. Coordinated Installation without Cooperative Testing Installation of an unbundled loop requires wiring a cable pair to the requested position (e.g., CFA) at the Carrier s presence in the central office. The installation will take place at a designated time (within the normal Monday thru Friday 8 a.m. to 5 p.m. work day) previously agreed to by Qwest and the Carrier. If the Carrier requests installation outside of normal business hours, additional labor charges will apply. This option is available only when converting an existing Qwest end user to another Carrier or when another Carrier s end user changes carriers. 4. For DS1, Primary Rate ISDN Capable and DS3 unbundled loops, installation of service is available with testing results provided to the Carrier.

The switching network element includes facilities that are associated with the line (e.g., the line card), facilities that are involved with switching the call, and access to the facilities used for customized routing. The switching network element encompasses the facilities of the switch (e.g., DS0 Switched Lineside Port), plus the functions and capabilities of the switch. 2. The access point for lineside local switching is the distribution frame (DF) of the Qwest designated SWC. 3. Network Elements: a. Switch Ports The Switch Port (also referred to as Network Access Channel Connection ) provides the physical connection to the Qwest switch and includes non-traffic sensitive equipment such as the line card. This includes the function of connecting lines to lines, lines to trunks, trunks to trunks, and trunks to lines. Descriptions of the unbundled Switch Ports are set forth in G., following. The Switch Port is rated on a flat, per month basis. Basic capabilities of the switch included with the unbundled switching element are: Telephone Number Dial Tone Signaling (loop start or ground start) On/Off Hook Detection Audible and Power Ringing Automatic Message Accounting (AMA) Recording Access to 911, Operator Services and Directory Assistance Blocking Options (e.g., 900 services) b. Switching Switching is the function of switching calls on a line to line, line to trunk, trunk to line or trunk to trunk basis. This component is described as set forth in I., following, and is rated on a per switch, per minute of use basis for all calls. c. Switching Features Features are available from the end user s serving wire center and ordered individually. These features are set forth in H., following, and are rated on a per item, per month basis.

Interoffice Transmission Facilities Unbundled Interoffice Transmission Facilities are provided in accordance with the specifications, interfaces and parameters described in the appropriate Technical Reference Publications. Interoffice Transmission Facilities are described as set forth in J., following. E. Local Loops A Local Loop, also referred to as a Network Access Channel (NAC) is the transmission path between the Network Interface Device (NID) at a customer (end user) location and the main distribution frame or equivalent of Qwest s serving wire center (switching office), or any other point of interconnection of the Qwest s network that may develop. The following types of Local Loops are available as unbundled network elements. Basic Local Loop - 2-wire ISDN Capable Local Loop - 2-wire Basic Local Loop - 4-wire DS1 Local Loop Primary Rate ISDN Capable Local Loop DS3 Local Loop OC-N Capable Loop Dark Fiber Local Loop Following is a list of interfaces associated with the Local Loop: Digital Facility Interface (DFI) Network Interface Device Integrated Digital Loop Carrier (IDLC) Interconnection

Basic Local Loop (2-Wire) A two-wire facility that provides connectivity from the serving central office distributing frame to the end user's network interface. This channel supports a Hz analog service. The Carrier is required to specify loop start or ground start. Unbundled Channel Performance Unbundled Network Elements are available but may be facility dependent. 2. ISDN Capable Local Loop (2-Wire) A two-wire facility that provides connectivity from the serving central office distributing frame to the end user s network interface. This channel meets the design requirements for Basic Rate ISDN standards of 144 kbps. Ordering of the ISDN capable NAC may require coordination (loop design and/or engineering) between the Carrier and Qwest. 3. Basic Local Loop (4-Wire) A four-wire facility that provides connectivity from the serving central office distributing frame to the end user's network interface. This channel supports a Hz analog service with send and receive transmission paths. The Basic Local Loop (4-Wire) provides no signaling functionality unless ordered with Unbundled Channel Performance Network Elements. Unbundled Channel Performance Network Elements are available but may be facility dependent. 4. DS1 Local Loop A facility that provides connectivity from the serving central office DSX-1 Cross Connection Frame or equivalent to the end user s network interface and meets the technical parameters for DS1. 5. Primary Rate ISDN Capable Local Loop A facility that provides connectivity from the serving central office DSX-1 Cross Connection Frame or equivalent to end user s network interface and meets the technical parameters for Primary Rate ISDN.

DS3 Local Loop A facility that provides contiguous connectivity from the serving central office DSX-3 distributing frame to the end user's network interface device and meets the design requirements for a DS3. 7. OC-N Capable Loop Definition to be developed in Docket UM Digital Facility Interface (DFI) A termination that provides a digital interface for a high capacity (1.544 Mbps) facility which terminates to a central office based switching system. It provides for the connectivity of 24 circuits within a Centrex system. 9. Dark Fiber Local Loop A pair of unused fiber strands (two fiber strands) from the Fiber Distribution Panel (FDP) at a Qwest central office to the customer s premises FDP. The fiber optic terminations at each FDP are included. The customer premises must be located within the serving area of that central office. The FDP provides a point for terminating in the Qwest central office for the outside plant. An inquiry procedure will determine availability of existing unused fiber strands at no charge. 10. Network Interface Device (NID) The NID provides an interface between the Local Loop and the customer s inside wire (IW). The NID is divided into two compartments: one containing the over voltage unit (protector), buried service wire and drop terminals; the other containing the customer s IW, the IW terminals and a modular plug which connects the IW to the Local Loop. Qwest retains sole ownership of the NID. 11. Integrated Digital Loop Carrier (IDLC) Interconnection Definition to be developed in Docket UM 1025.

Cross-Connect Facilities Following is a list of Cross-Connect Facilities: ITP-DS0 ITP-DS1 ITP-DS3 electrical ITP-OCN Definition to be developed in Docket UM G. Switch Ports The Switch Port is a component of the Unbundled Switching function, also known as a Network Access Channel Connection (NACC). The Switch Port subcategory of Unbundled Network Elements refers primarily to various configurations of terminating electronics. Switch Ports are the central office equipment portion of the switch and are nontraffic sensitive. Following is a list of unbundled Switch Ports: DS0 Switched Lineside Port - (Analog Lineside Port) DS0 Analog Trunk Port DS1 Switched Lineside - (DID/DOD/PBX Capable) DS1 Local Message Trunk Port Digital Lineside Port (Supporting BRI ISDN) The Basic Switch Port provided with standard signaling and transmission level capabilities suitable for a wide variety of network services. Unbundled Basic Switch Ports provide the customer a variety of options and applications including switched lineside or trunkside voice and data connections as well as dedicated private line and special access connections. The following provides Extension Capability for ISDN 2-wire: ISDN Extension Technology Switch Port - (ISDN Capable 2-wire) The following provides Customized Routing for Unbundled Ports: Customized Routing

Switch Port DS0 Switched Lineside - Analog Lineside Port A two-wire analog line side connection (e.g., a Lineside Port) into the Qwest end office which allows customers access to the following switch based features and services. Telephone Number Directory Listing Dial Tone Loop or Ground Start Signaling On/Off Hook Signal Detection Audible and Power Ringing Automatic Message Accounting (AMA) Recording Access to 911 Access to Operator Services Access to Directory Assistance Blocking Options (e.g., 900 Services) 2. DS0 - Analog Trunk Port An analog trunk side connection (e.g., Trunkside Port, used for DID, PBX) into the Qwest end office which provides customers access to switch trunking features. 3. DS1 Lineside Port (DID/DOD/PBX) A DS1 lineside connection (e.g., 24 Lineside Ports) into the Qwest end office which allows customers access to switch based features and services. 4. DS1 Local Message Trunk Port A DS1 trunk side connection (e.g., 24 Trunkside Ports) into the Qwest end office which provides customers with access to trunkside switch features. 5. Digital Lineside Port (Supporting BRI ISDN) A two-wire Basic Rate ISDN capable lineside connection into the Qwest end office which allows customers access to switch based features and services. Minutes of use will also be billed for those minutes that originate from the unbundled switching element.

Customized Routing Customized Routing will combine end office switching (line ports) and dedicated trunk ports, groups and members with specific routing to allow Carriers the ability to route traffic by class of service to specific dedicated trunks (local message, operator services and directory assistance). Capacity constraints vary from switch to switch for local message routing and Customized Routing orders will be evaluated on a customer by customer and switch by switch basis. H. Interconnection Network Elements Following is a list of Interconnection Network Elements. Interconnection Tie Pairs (ITP) Multiplexing DS1 to DS0 Multiplexing DS3 to DS1 IDLC Interconnection Intra-Premises Riser Cable 1. Interconnection Tie Pairs (ITP) Definition to be developed in Docket UM Multiplexing DS1 to DS0 The Unbundled Network Element provides the Carrier with the option of having their DS1 signal demultiplexed to the DS0 level (24 DS0s) by Qwest. In addition to the multiplexing, channel plugs will be required. 3. Multiplexing DS3 to DS1 The Unbundled Network Element provides the Carrier with the option of having their DS3 signal multiplexed to the DS1 level (28 DS1s) by Qwest. 4. IDLC Interconnection Definition to be developed in Docket UM 1025.

Intra-Premises Riser Cable Intra-Premises Riser Cable provides a two-wire or four-wire connection between points within a building where Qwest owned facilities are utilized. This Unbundled Network Element includes the building type cable and the associated terminations. Access to the Intra-Premises Riser Cable originates at the Qwest Main Point of Presence (MPOP) and extends to the Additional Point of Presence (APOP) where the customer s inside wire terminates. I. Switching Switching establishes a temporary transmission path between two or more Local Loops in the same switching office, or between a Local Loop and a DSX-1 facility in the switched transport termination. Following is a list of Switching unbundled elements: Tandem switching End office switching-originating End office switching-terminating End office switching-intraoffice 1. Tandem Switching Switching between dedicated and switched transport facilities when a tandem switch is used as the first point of interface to the switched network. It includes the facilities connecting the trunk distribution frames to the switch and all the functions of the switch itself including those facilities that establish a temporary transmission path between two other switches. It also includes the functions that are centralized in tandems rather than in separate end office switches such as call recording and the routing of calls to operator services. 2. End Office Switching - Interoffice (Originating and Terminating) Switching between Local Loops and either incoming or outgoing switched transport facilities connected to different switching offices. 3. End Office Switching-Intraoffice Switching between two or more Local Loops served from the same switching office.

12 UNBUNDLED ELEMENTS Sheet ACCESS TO UNBUNDLED ELEMENTS (Cont'd) J. Interoffice Transmission Facilities Interoffice Transmission provides the transmission facilities between Qwest wire centers. There are two types of Interoffice Transmission Facilities, Switched Transport and Dedicated Transport. Interoffice Transmission Facilities is a temporary time sensitive interoffice transmission path between switching offices and/or serving wire centers (SWC) of Qwest. Dedicated Transport is a full period, bandwidth specific (DS0, DS1, DS3) interoffice transmission path between switching offices and/or SWCs of Qwest. Following is a list of Interoffice Transmission Facilities unbundled elements: Transport Termination-Switched Transport Termination-Dedicated DS0 Transport Termination-Dedicated DS1 Transport Termination-Dedicated DS3 Transport Termination-Fiber Optic Transport Facilities-Common Transport Facilities-Dedicated DS0 Transport Facilities-Dedicated DS1 Transport Facilities-Dedicated DS3 Transport Facilities-Dark Fiber 1. Transport Termination-Switched An Interoffice Transmission Facilities network element consisting of a trunkside switch termination (e.g., DFI) for a DS0 or multiplexed DS1 facility. 2. Transport Termination-Dedicated DS0 An Interoffice Transmission Facilities network element consisting of a 56 Kbps voice grade interface or up to 64 Kbps digital data interface between the interoffice transport facility and other network elements. The Transport Termination-Dedicated DS0 provides up to 64 Kbps capability for a Carrier to interconnect with Qwest s interoffice network. It consists of a channel on a M10 (e.g., D4) in Qwest s central office but does not include the Transport Facility.

Transport Termination-Dedicated DS1 An Interoffice Transmission Facilities network element consisting of a Mbps DS1 termination interface between the Interoffice Transmission facility and other network elements. An Interoffice Transport Termination-Dedicated DS1 provides full Mbps capability to a Carrier to interconnect with Qwest s interoffice network. It does not include the Transport Facility. 4. Transport Termination-Dedicated DS3 An Interoffice Transmission Facilities network element consisting of a Mbps termination interface between the interoffice transport facility and other network elements. An Interoffice Transmission Facilities Termination-Dedicated DS3 provides full Mbps capability for a Carrier to interconnect with Qwest s interoffice network. It does not include the Transport Facility. 5. Transport Termination-Fiber Optic A charge to terminate Interoffice Transmission Facilities-Dark Fiber at an FDP at both ends of the interoffice fiber path. 6. Transport Facilities-Common An Interoffice Transmission Facilities element consisting of a switched DS0, DS1 or DS3 facility between Qwest s end offices or between Qwest s end offices and a Qwest s local tandem. Transport Facilities-Common provide a temporary, timesensitive interoffice transmission path between Qwest s switching entities. Common facilities, also referred to as shared interoffice facilities, are used to combine message trunk traffic of multiple customers and are typically separated by traffic class.

Transport Facilities-Dedicated DS0 An Interoffice Transmission Facilities network element providing a transmission path up to 64 Kbps between Qwest central offices. This transmission path does not provide metallic circuit capability. 8. Transport Facilities-Dedicated DS1 An Interoffice Transmission Facilities network element providing a Mbps transmission path between Qwest central offices. 9. Transport Facilities-Dedicated DS3 An Interoffice Transmission Facilities network element providing a Mbps transmission path between Qwest central offices. 10. Transport Facilities-Dark Fiber An interoffice element providing a pair of unused fiber strands (two strands) between FDPs at Qwest central offices. A separate charge applies for Transport Termination-Fiber Optic. If intermediary central offices are used to complete the requested route, a cross connection OC-N will be required in each of the intermediary central offices. An inquiry procedure will determine availability of existing unused fiber strands.

Switching Features Switching Features is a category of network elements that provides for call processing beyond the simple connection of a Network Access Channel to a Network Access Channel, a Network Access Channel to outgoing Transport facilities or incoming Transport facilities to a Network Access Channel. Switch features are associated with Custom Calling, Centrex, CLASS and ISDN. The following switching features are available as unbundled network elements: Call Waiting A function that provides a tone to the party using the telephone to indicate another call is waiting on the line. Successive transfers between calling parties can be accomplished through switchhook operation. Call Waiting Deluxe Allows the customer to control the disposition of incoming calls while in an offhook condition via a visual display unit. A customer provided visual display unit is required to interact with this feature. The customer must subscribe to Caller Identification-Number or Name and Number. Call Waiting Identification A function that provides a tone to the party using the telephone to indicate another call is waiting on the line and allows the called party to receive information on a call that is waiting. The customer must subscribe to Caller Identification-Number or Name and Number. Dial Call Waiting Allows a customer with the feature to direct a Call Waiting tone or a Distinctive Alert signal to a line equipped with Distinctive Alert. The feature is activated by dialing a preset access code and the telephone number of the line to which the signal is directed. Call Forwarding-Busy Line (External) Allows a customer to have incoming calls forwarded to another predetermined number outside the customer's system, within the same central office switch, when the called number is busy.

Switching Features (Cont'd) Call Forwarding-Busy Line (Expanded) Allows a customer to have incoming calls forwarded to another predetermined number in a different central office switch if the called number is busy. Call Forwarding-Busy Line (Overflow) Allows a customer to have incoming calls forwarded to another predetermined number within the customer's system if the called number is busy. Call Forwarding-Busy Line (Programmable) Allows a customer to have incoming calls forwarded to another number if the customer does not answer after a preset number of ringing cycles. The customer can activate and deactivate the forwarding feature by dialing a code as well as establish or change the number to which calls are forwarded. Call Forwarding-Don't Answer (External) Allows a customer to have incoming calls forwarded to another number within the same central office switch if the customer does not answer after a preset number of ringing cycles. Call Forwarding-Don't Answer (Expanded) Allows a customer to have incoming calls forwarded to a predetermined number outside the serving central office switch if the customer does not answer after a preset number of rings. Call Forwarding-Don't Answer (Programmable) Allows a customer to have incoming calls forwarded to another number if the customer does not answer after a preset number of ringing cycles. The customer can activate and deactivate the forwarding feature by dialing a code as well as establish or change the number to which calls are forwarded.

Switching Features (Cont'd) Call Forwarding Busy/Don't Answer (External) Allows the customer to have incoming calls forwarded to another predetermined number within the same central office if the called number is busy or does not answer after a preset number of ringing cycles. Call Forwarding Busy/Don't Answer (Expanded) Allows the customer to have incoming calls forwarded to another predetermined number in a different central office if the called number is busy or if the customer does not answer after a preset number of ringing cycles. Call Forwarding Busy/Don't Answer (Overflow) Allows the customer to have incoming calls forwarded to another predetermined number within the same central office or to another line within the same customer group of lines within the customer system if all lines are busy or do not answer after a preset number of ringing cycles. Call Forwarding-Variable A function which allows incoming calls to be forwarded to another telephone number. The number the calls are forwarded to may be changed as required by the customer. An option is also available that allows a customer to activate the feature without completing a call to the forward-to number. Remote Access Forwarding A function which allows incoming calls to be forwarded to another telephone number. It allows the customer to remotely change the termination of their incoming calls. From any tone signaling telephone, the customer can activate, deactivate or change the destination numbers.

18 UNBUNDLED ELEMENTS Sheet ACCESS TO UNBUNDLED ELEMENTS K. Switching Features (Cont'd) Scheduled Forwarding A function which allows incoming calls to be forwarded to another telephone number. It allows a customer to remotely change the termination of their incoming calls and base the termination upon a time schedule. From any tone signaling telephone, the customer can activate, deactivate or changes the times, days and destination number. Customized Routing Functions An option which enable a customer to direct particular classes of calls to particular outgoing trunks. Customized Routing can be used to direct calls to 411, , or 0- to it own directory assistance or operator services platform. Speed Calling (Long and Short) A function that allows a customer to assign and dial abbreviated codes to frequently called numbers. It is available in 8 number or 30 number capacity. Three-Way Calling A feature providing the capability to add a third party to an existing conversation. Hunting Allows incoming calls to hunt to the next number in a hunting sequence if the number is busy Call Transfer Enables a customer to transfer an incoming call to a third party or add a third party to an existing call, forming a three party connection, and then to leave the connection without disconnecting the call.

Switching Features (Cont'd) Directed Call Pick Up Allows a customer to answer a call, during the ringing cycle, that is directed to another line, by dialing a preset code and the telephone number of the line to be answered. Both the originating line and the line to be answered must be equipped with the feature. Directed Call Pick Up with Barge-In Allows a customer to answer a call directed to another line which has been answered or is ringing by dialing a preset access code and the telephone number of the line to be answered. Both the originating line and the line to be answered must be equipped with the feature. Distinctive Alert (Distinctive Ringing) Allows a customer to receive an audible Call Waiting tone or Distinctive Ringing signal from a line equipped with Dial Call Waiting. If the called line is idle, a Distinctive Ringing signal will be heard. If the called line is busy, the called line receives a Call Waiting tone. Hot Line Allows equipped station to automatically place a call to a preassigned number by going off-hook Warm Line Service Allows a customer to establish a switched connection to a predetermined number if the customer does not dial a number within a specified length of time after going off-hook. When the customer's telephone goes off-hook and dialing begins within a specified time delay period, the call will proceed normally as dialed. If dialing has not started before the end of the predefined time delay period, a predetermined stored number is automatically dialed by the central office switching equipment.

Switching Features (Cont'd) Caller Identification - Name and Number (Calling Name and Number Delivery) Allows for the automatic delivery of a calling party's name and telephone number (including nonpublished and nonlisted telephone numbers) to the called customer, which gives the called customer an opportunity to decide whether to answer the call immediately or not. The name and number are displayed on customer provided equipment. The name displayed shall be the name associated with the calling telephone number as shown on the Company's records. The Company in its discretion, may abbreviate or limit that name for display purposes. The Company does not assure name accuracy, and it shall not be liable to any party for errors, omissions or mistakes. The Company's sole and only obligation shall be to reasonably correct errors in names when notified in writing of such errors. Caller Identification customers who do not wish to receive blocked calls can activate Anonymous Call Rejection by pressing *77 (1177 on rotary phones). The code to deactivate is *87 (1187 on rotary phones). While the feature is activated, incoming blocked calls are routed to an announcement in the central office. Anonymous Call Rejection is automatically available to residence subscribers of Caller Identification and to business subscribers where technically feasible. Caller Identification Blocking - All Call Caller Identification Blocking - All Call provides a permanent private indicator on a customer's line. Once blocking is established on the customer's line, the private status can be deactivated by the customer by dialing a code, *82 or 1182 on rotary phones before each call, to change the indicator from private to public. This one call unblock allows the name and number to be sent for that one call only. All lines in CLASS equipped central offices will have access to the feature. If a line is equipped with Caller Identification Blocking - All Call, the name and number of that line will not be delivered to any subscriber of Caller Identification. Poison control centers, hospitals, medical centers and others who might use Caller Identification will not be able to identify callers with Caller Identification Blocking - All Call who need assistance. E9-1-1 is not affected.

This service is available on a usage or subscription basis. Call Trace (Customer Originated Trace) Allows a called party to initiate an automatic trace of the last call received. Call Trace is available on a usage basis only. After receiving the call which is to be traced, the customer dials a code and the traced telephone number is automatically sent to the Company for action. The calling number is transmitted to a designated agency (telephone company or law enforcement agency). Last Call Return Allows a customer to dial a code that will cause the feature to automatically redial the number of the last incoming call to that line, whether the call was answered or not. The customer does not have to know the number of the calling party. If the called number is busy, the feature will redial the called number for a limited period of time. A distinctive ring alerts the customer when the called line is available. The feature does not return calls to numbers that have been blocked. This service is available on a usage or subscription basis. Last Call Return customers who do not wish to receive blocked calls can activate Anonymous Call Rejection by pressing *77 (1177 on rotary phones). Selective Call Forwarding Allows a customer to specify a special list of a maximum of fifteen telephone numbers. Incoming calls placed to the customer from telephone numbers on that list will automatically be forwarded to a predefined telephone number. All other calls will be handled normally. Call Rejection (Selective Call Rejection) Enables a customer to reject call attempts from up to fifteen numbers of calling parties by dialing a code and the telephone numbers of calls to be rejected. Any call attempts to the customer from these numbers will be prevented from terminating to the customer and will instead be connected to an announcement informing the caller that the call is not presently being accepted by the called party. A customer may also reject future calls from the most recent call received by dialing a code after completing the call.

Call Forwarding Busy Line Provides for forwarding of incoming calls to a preselected telephone number, when the called station is busy. Call Forwarding Don't Answer Provides for forwarding of incoming calls to a preselected telephone number, when the called station does not answer after a predetermined number of ringing cycles. Call Forwarding-Variable Allows a user to automatically forward incoming calls to any other number. Three-Way Calling/Consultation Hold/Call Transfer User can connect a third line to an established connection. A user can depress the switchhook and consult in private with a third party and return to the original call by using only one line. A user of a line can transfer any established call to another line within or outside the Centrex Plus System. Touch-Tone Allows for customer provided equipment to place calls on the Centrex Plus system. Intercom 30 A user of the Intercom 30 service can dial up to twenty-nine other lines in the same package by dialing an access code followed by two digits. Two user stations with the same line number cannot access each other utilizing the Intercom feature. 1.3 ANCILLARY SERVICES AND ARRANGEMENTS INTERCONNECTION TO LINE INFORMATION DATABASE (LIDB) Line Information Data Base (LIDB) stores line number information and Special Billing Number (SBN) information for use in processing Alternately Billed Services (ABS) calls. Operator Service switches have the capability of launching a query on the billing telephone number for validation of a calling card number, or for screening on collect or third number billed calls. These queries usually consist of a TCAP message and response which accesses the LIDB and retrieves information used for call processing INTERCONNECTION TO 8XX DATABASE QUERY SERVICE A. 8XX Database Query Service is an originating service which provides for the forwarding of the Carrier s end user dialed 8XX-NXX-XXXX calls to a toll carrier, based on the dialed 8XX number. When an 8XX call is originated by a Carrier s end user, the Carrier s SSP (SS7 equipped end office) will send a 8XX query to the Qwest 8XX Service Control Point (SCP) through the Qwest Signaling Transfer Point (STP). The Qwest SCP will perform the Carrier Identification Function based on the dialed digits to determine the toll carrier trunk group to which the call should be routed in accordance with the SMS/800 information residing in the Qwest SCP. The SCP will transmit the results of the Carrier Identification Function back to the Carrier s SSP through the Qwest STP. The results of the Carrier Identification Function will be the Carrier Identification Code (CIC) and/or the vertical features associated with the 8XX number. The 8XX Data Base query will be billed to the toll carrier whose CIC is returned from the 8XX Database Query.
