1 Waste Pickup Form User s Guide Updated: 3/13/12
2 Introduction: Welcome to the Cal State University Fullerton Online Waste Pickup Form User s Guide. In this guide you will learn what you can use the form for, how to fill out the form, and what you need to do once you have submitted your request. What is the form used for? : 1. To request the pickup of spent hazardous chemicals (i.e. spent chloroform, spent acetone, etc) 2. To request a pickup of spent biological or radioactive chemicals or solutions (i.e. spent phosphorus-32) 3. To request a pickup of universal waste including light bulbs, aerosol cans, batteries, mercury thermometers 4. To request delivery of a chemical waste container(s) (i.e. container for used solvents or laboratory solid debris bags) 5. To request delivery of spent chemical labels ***EHS DOES NOT PICKUP E-WASTE INCLUDING: COMPUTERS, KEYBOARDS, SPEAKERS, MONITORS, CALCULATORS, TVs, OR ANY OTHER ELECTRONICS. CALL PHYSICAL PLANT AT X3494 FOR PICKUP OF THESE ITEMS***
3 How to fill out the form: STEP 1: Determine what type of waste you have (Chemical, Radioactive, Biological) and choose the correct form using the radio tabs at the top of the page. The correct form must be chosen for your waste to ensure proper disposal. For instance, if you have a radioactive carbon-14 liquid that needs to be picked up, press the radioactive button to be able to fill out the Hazardous Radioactive waste pickup form. You will notice a color change when changing forms (Chemical = white, Radioactive = yellow, Biological = red). Push the button next to the waste type to choose that form.
4 STEP 2: Fill out the first eight fields. These eight fields are required for you to be able to submit the form: Name of Person Requesting (i.e. John Smith) Requestor s (i.e. jsmith@fullerton.edu) Requestor s Phone (or 4 digit campus extension) (i.e. x9999) Department Name (i.e. Chemistry) Building Name (i.e. DBH) Room Number (i.e. 213) Principle Investigator (PI) (i.e. Jane Smith) Location of the Waste in Lab (i.e. right corner of eastside fumehood) This section MUST be completely and accurately filled out to insure proper disposal Figure 1 - Picture of required fields filled If your building or room number is not listed, click on the box next to other room or other building. An entry field box will open up next to other room or other building. Enter your building and or room number into the field. After you enter your building name and room number your info will now be in the drop down menus for future pickup requests. 5 STEP 3: Enter the chemicals/waste items you would like to have picked up. Scroll down the page and click on the button. After clicking on the button a new window will pop up where you will be able to enter all the information necessary to ensure safe and efficient disposal including: All the chemical constituents of the waste in terms of percentage of total weight/volume (i.e. Acetone 50%, Water 50%) Container type (i.e. Glass, plastic, metal) Weight or Volume Measurement Unit # of Containers (i.e. amount of same containers) Hazard Categories (i.e. flammable, toxic, etc) States (i.e. liquid, solid, gas) To add a chemical, first click on the drop down menu that says select a chemical. Once you have clicked on the drop down menu, start to type the name of your chemical (i.e. type acetone). You will see the menu start to move toward a name close to or even matching your chemical. You may also select a chemical by scrolling through the list. Figure 3: Once the drop down menu is clicked, start typing the chemical name to quick scroll to your chemical
6 ***If your chemical does not show up on the list, please select a similar chemical and state that your chemical is not on the list in the comments section. EHS will add your chemical for future requests. *** In the comments section type in any additional information on the chemical pertinent to a safe and secure pickup. If you don t have any comments type none or N/A into the comments section. If you have a chemical mixture, first select your primary chemical. Then write in the percentage of the remaining constituents into the comments section. To add a hazard category and physical state you must first click on the drop down menu and make a selection. Once you have selected the correct choice you MUST click and you will see your choice in the main box. If you have chosen the wrong selection click your selection and click the button. After entering all of the above information, your completed form should look like this: Figure 4 : Picture of Completed "Add Chemical" form
7 Once your form is completely filled out, click on the button to submit the waste item. If you have made a mistake and want to go back to the main form click the button. After clicking on button you will be taken back to the main form which should now look like this: Figure 5: Successful addition of a waste item If after you have submitted the waste item, you no longer want to have it picked up, click the button to remove the item. Repeat these steps until all of your waste items have been added.
8 STEP 4: Fill out the additional comments section of the form. Scroll down to the additional comments section. Any additional comments, requests, time restraints, specific needs, and spent chemical labels should be listed to ensure proper and safe disposal of the waste. If you are requesting additional waste containers or replacements for the containers being picked up, please note them here. Your form should now look like this: Fill in any additional comments or requests HERE Figure 6: Additional Comments Section Filled Out
9 STEP 5: Review your form and submit. After completing steps 1-4 your form should be completely filled out. Please spend a few moments reviewing entries to make sure everything is correct. ***Forms with inaccurate information WILL be delayed or not taken at all (if we can t find the waste in the location specified, it won t be picked up). You must click on the box stating that you have read this guide and agreed to the before pickup terms (STEP 6). Failure to fill out required fields or click on the box stating you have read the guide and agree to the terms will block you from submitting. First CLICK this box Then CLICK SUBMIT If you find you cannot click the button, it is because you either didn t fill out the required fields or you failed to check mark the box agreeing to the terms of this guide. If you find you have made too many mistakes on your form and would like a blank form, click the button. After submitting the form your waste is almost ready to be picked up. An confirmation with your request number will be sent to you after submittal. Please go down to STEP 6 for the before pickup terms.
10 STEP 6: BEFORE PICKUP TERMS: YOUR WASTE/SPENT CHEMICALS MUST BE PROPERLY LABELED, HAVE A SCREW TIGHT LID AND BE PLACED TOGETHER IN THE LOCATION SPECIFIED ON THE FORM BEFORE PICKUP. Your waste must be properly labeled with the following information before pickup including: Generator s name (name of person generating the waste) Building and room number Accumulation start date (date you started collecting waste in the container) Hazard category (i.e. flammable, corrosive, etc) Physical state (i.e. liquid, solid, gas) List of chemical contents with percentages (no shorthand or nicknames and percentages can be approximate) Figure 7: Properly filled out Spent Chemical Label Waste containers must be in good condition and have screw tight lids. Parafilm, aluminum foil, paper, tape, wax paper, and broken or non tightening lids are NOT acceptable for pickup.
11 All of the waste requested to be picked up MUST be placed into the same location as specified by the form. Figure 8: Picture of appropriate lid/cap (left). Waste grouped and ready for pickup (right). Once your waste has been properly labeled, has a screw tight lid/cap and is grouped together, it is ready for pickup! A confirmation will be sent to you once the waste has been picked up. ***WASTE THAT DOES NOT MEET THESE REQUIREMENTS WILL NOT BE PICKED UP UNTIL THE CORRECTION HAS BEEN MADE.
