5 Overview The System utilizes chemical categories to provide you with important information about the chemicals in your workplace. assigns the chemicals in your workplace to one of 157 chemical categories called Safe Use Guides, which inform a user of the level of danger associated with the chemical. This program fully complies with the proposed GHS addendums to the OSHA Hazard Communication Standard. Take an active role in your safety and discover how the System works to provide you with tools that have simplified technical chemical information. Make certain that you are comfortable with the procedures for accessing chemical information. The more you know about the chemicals you are working with, the safer you will be. 8 Logging into MAXCOM To log into, please go directly to: Figure 1 Login Screen To use: 1. Enter in your username given to you by MAXCOM. 2. Enter in the password given to you by MAXCOM 3. Click Remember me on this computer for MAXCOM to remember your logon and automatically log you into MAXCOMGHS. 4. If you forget your password, click Forgot Password? to reset your password. (Please note if your administrator did not setup an address for you, this function will not allow a reset of your password. General user account passwords cannot be reset using this function) 1/9/2018 4

Click the link. b. Enter in a new password and re enter the password for the new password to take effect. 10 FRAME NAVIGATION TOOLS Google Translate allows you to choose a language that will transform the language of to your preferred language. To change, select the dropdown and find the language desired. Cancel the translation by clicking the Show original button or clicking the X on the top right hand corner for the Google Translate toolbar. Use the Select Location button to change the main location the Chemical List searches and shows results from. Submit MSDS Opens the informational screen with instructions on how to add products to MAXCOM. Only visible on administration and superuser level. Opens the Chemical Approval Request Window if you have chemical approvals activated. Gives information on the Fax on Demand service by (charges apply) Opens a new browser window to MSDSs on the Web with over 2000 Manufacturer websites and location of their MSDSs. This link opens your e mail client to send an e mail message to technical support. This link opens our Help and User Guide location. Clicking this link opens the login box for the administrative login. Clicking this link will log you out of the System and return you to the 1/9/2018 6

11 login screen. LANGUAGE Use Languages to change to the preferred language of your choice. To choose a language, use the drop down arrow to view the list of languages provided by Google Translate. Click to select your language. Figure 3 Google Translate The system will show in the desired language. When you would like to view back in the original language, click the Show original button to change the language back to English (default). To remove the Google translate toolbar, click the X on the top right hand corner of the toolbar. NOTE: Due to browser differences, the Google Translate may not be available for all browsers. 1/9/2018 7

12 SELECT LOCATION Use the Select Location button to change the main location the Chemical List searches and shows results from. Figure 4 Select Location Box To Use: 1. Click the Select Location button at the top of the Chemical List to change the default location name viewed. The highest level of the tree is considered the Company Level. If you have used previous versions of MAXCOM, this is the equivalent of ALL. Figure 5 Location Tree 2. Use the Plus sign to expand to the sub locations below the location. Continue to expand until you find the location you want to filter by. 1/9/2018 8

The Chemical List will change to reflect the products in that location. Submit MSDS Submit MSDS shows you information on how to submit products to MAXCOM to add to your repository. This only is accessible if you are a superuser or administrative user. 1/9/2018 9

15 Figure 10 Fax on Demand MSDSs ON THE WEB MSDSs on the Web is a website maintained for MAXCOM Services Division customers that lists all manufacturers and their locations on their websites for MSDSs. To use: 1. Click on MSDSs on the Web on the left hand navigation. 2. You will be navigated to a separate window. Figure 11 MSDSs on the Web 3. Use the numeric alpha list to look for manufacturers that start with that letter. 4. You can also type in a manufacturer or distributor name and choose a Boolean starts with or contains and click the Start Search button. 1/9/

16 5. The Search Results will appear with a hyperlink to that manufacturer s website for obtaining MSDSs from them and gives the format of the documents found. Figure 12 MSDSs on the Web Results 6. We try our best to give you the most up to information, however, those websites are not maintained by MAXCOM Services Division nor can we claim those websites are in working order and no problems can be associated with them. 7. SYSTEM MESSAGES If your Administrator created a message to you for your facility, this message will be displayed in a pop up window when you first log into. This message is by location so you may have a different message depending on what location you are currently displaying. 1/9/

Administrative Login The Administrative Login re opens the MAXCOM GHS login page for an administrator to log in. LOGOUT LOGOUT will log you out of the System and return you to the login screen. 1/9/

18 GENERAL USER VIEW CHEMICAL LIST SDS is short for Safety Data Sheet and is used extensively to describe this item in this menu. This is the first item you will see after logging into MAXCOM GHS. By default, this opens to the Company Level view of all MSDSs available for your username. You will see the following once logged into the system : Figure 16 Chemical List View General User Login Navigating the Chemical List Window The Chemical List is your starting point of the System. In this location, all other functions can be accessed using the left hand navigation as well as items below the list and ability to view system messages. Each of these abilities will be discussed separately. Summary Button The Summary button brings up the Summary tab from the chemical details and defaults to the Summary of the Material. MSDS Button The MSDS Button brings up the SDS tab from the chemical details and defaults to the MSDS of the material. Safe Use Guide Button The Safe Use Guide button brings up the Safe Use Guide tab from the chemical details and defaults to the Safe Use Guide tab. 1/9/

19 Safe Use Guide Icon The Safe Use Guide icon tells you the level by color of the material. The Safe Use Guide Number indicates what Safe Use Guide is assigned to that level. The number does not signify how dangerous or safe the chemical is. To describe the levels: 1. Red Corresponds with the GHS Signal Word Danger 2. Yellow Corresponds with the GHS Signal Word Warning 3. Green Does not correspond with a GHS Signal Word but material can be considered relatively safe. GHS Pictogram If a material has a GHS Pictogram attached to it on the chemical list, hover over the pictogram to obtain more information of what that pictogram represents. Figure 17 - Pictogram Hover description. Search: To search for an SDS or product, use the Search box at the top of the Chemical List. The area searched is filtered by the location selection box at the top of the screen. Figure 18 Chemical List Search Click on the Enter Search Text box and enter the desired information. You can search by the following values with this same box: 1 Product Name 1/9/

20 2 Product Synonym 3 Manufacturer Name 4 Manufacturer Part Number Once you enter an item in the search box, the Search and Reset buttons will enable. Click the Search button to start the search or press <Enter> on your keyboard. To reset and start over, click the Reset button. Figure 19 Chemical List Buttons Advanced Search: You can perform an advanced search to find products using additional information. To use: 1. Click on Advanced Search. 2. A pop up of additional options displays. 1/9/

21 Figure 20 Advanced Search Options 3. Choose the options from the bottom for national lists or type in the input boxes the information you would like to search for. 4. Click the Search button. 5. To cancel and close the Advanced Search, click the Cancel button. 6. After performing an advanced search, to reset the chemical list, select the Reset Advanced Search button. Location Filter: To change the location you want to search by, select the Location button. Figure 21 Select Location The location checkbox will appear. Use the hand cursor to pick what location you want to view. Click the Close button to select the location. 1/9/

22 Figure 22 Select Location Treeview Sorting the Chemical List: Product Name Lists the Product Name or the Synonym if listed on the SDS. Clicking on drop down arrow to the right the product name heading in the column will provide a menu to choose if you want to sort by ascending or descending order. The arrow directly to the right of the Product Name Heading will show which direction the list is currently sorted. Figure 23 Sort Menu Product Name If you click on Columns, there is a sub menu allowing you to choose which of the two or both columns you would like to have displayed in the Chemical List. 1/9/

23 Figure 24 Columns submenu Manufacturer Name Lists the Manufacturer Name if listed on the SDS. Clicking on drop down arrow to the right the manufacturer name heading in the column will provide a menu to choose if you want to sort by ascending or descending order. The arrow directly to the right of the Manufacturer Name Heading will show which direction the list is currently sorted. Figure 25 Sort Menu Manufacturer Name If you click on Columns, there is a sub menu allowing you to choose which of the two or both columns you would like to have displayed in the Chemical List. Figure 26 Columns Submenu Navigation Bar The Navigation bar at the bottom of the screen allows you to view page by page the results of the records. In other areas you will find similar navigation bars with the same ability. In summary each is described: 1/9/

24 Figure 27 Record Navigation First Takes you to the first page. Previous Takes you to the previous page. Page view manually enter the page you would like to view and total number of product pages. Next Takes you to the next page. Last Takes you to the last page of records. Refresh Refreshes the chemical index. Records per Page Choose the down arrow to change the number of records displayed on the page. Exports records to a spreadsheet program (see separate explanation) Exports records to a spreadsheet program for EHS purposes. (see separate explanation) Prints a PDF version of the Chemical List Displays the number of materials on the page and total number of materials in the filter or search. EXPORT The Export option when selected, brings up the export selection box. Choose what selections you want to export and select the Export button. Choose Download File and the Export and the file will show in your spreadsheet compatible program. Choose to save the file to your computer for viewing later or separately from. 1/9/

To avoid this message, change the security settings for Internet Explorer to enable File Downloads. See Microsoft help or more information from the message box for more details. Also you can press and hold the CTRL key on your keyboard while clicking on Export (on Chemical List). This action bypasses the need to have file download come into play. Continue to hold the key down for the entire process. EHS SNAPSHOT The EHS Snapshot allows the user to view all chemicals that may contain ingredients that are associated with a number of SARA or other Right To Know regulations. This feature works best when all information has been added to your Chemical Inventory including Ingredient Names, CAS (Chemical Registry System) numbers and percentage of ingredient. Please note that some reports also provide a disclaimer and please note those disclaimers in your review of the report. Please also note this report may take some time to generate dependent on the amount of records you have in your system. 1/9/

26 To use: 1. Click on the EHS Snapshot icon. 2. When the pop up appears, choose to open the file or save to your local computer s hard drive. Figure 29 EHS Snapshot File Download 3. If you click the Open button, the program will open Microsoft Excel or similar spreadsheet program and display all chemicals that meet each of the tabbed criteria. 1/9/

Internet Explorer Note: If you receive this notification from Microsoft, click on the message and choose Download File. This action bypasses the need to have file download come into play PRINT LIST The Print List function allows you to print a PDF version of the chemical list. To use: 1/9/

28 1. Select a location from the Select Location (you cannot perform a print list at the Company (Highest) level in the location tree. 2. Click the Print List icon. 3. A message box will appear letting you know the report is being printed. Click OK on the message box. When completed, a box will appear stating the report is complete. 4. The report will appear as a PDF document. You will be asked to open or save the document depending on the web browser you are using. Figure 31 Download window Print Index 5. 30 Viewing the SDS Details To view any information about a product, select and click on the hyperlink. The following pop up screen will appear: Figure 33 Product Details In the SDS Details, you have access in the minimum of the following functions: 1. Summary view of the product summary. 2. Safe Use Guide view of the Safe Use Guide associated with the product. 3. Safety Data Sheet view of the actual Safety Data Sheet. 4. Labels view and ability to print labels. 5. Storage Compatibility view the storage compatibility with other products. 6. PPE view the Personal Protective Equipment. 7. Ingredients view the composition information. 8. Optional tab (security restricted) Edit view additional items that require administrative access. Full explanation of each of the tabs is listed below. Summary Tab Details Window The summary tab contains the Hazard Statements and Warning labels for the product as well as summarized information provided in various sections of the SDS. If any documents, groups or locations are assigned to this product, you can find it in the summary tab by scrolling down the list of information. If a document is assigned, click the blue hyperlink on the document name to view the document. 1/9/

31 Figure 34 Product Details Summary Safe Use Guide Tab Details Window The Safe Use Guide Tab lists the Safe Use Guide associated with this product. Figure 35 Product Details Safe Use Guide Safety Data Sheet Tab Details Window: The Safety Data Sheet Tab will display the SDS of the product as well as any older revisions of that product. This option will display the MSDS in Adobe PDF format. To print the SDS, use the printer icon from within the tab. To save to your local computer, use the floppy drive icon from within the tab. 1/9/

32 Figure 36 Product Details Safety Data Sheet If you want to view other revisions of this SDS, click the Revision Date Drop down and choose a different revision date. The SDS tab will refresh with that SDS on the tab for Safety Data Sheet. This change will not alter any other tab s information. All other tabs will always show the most up to date Revision Date s information. Labels Tab Details Window Secondary container labels are required to be placed on any container that contains a hazardous chemical. OSHA required information includes the chemical name and appropriate hazard warnings such as the Physical and Health Hazards, Target Organ Affects, Routes of Entry and PPE supports 5 different label sizes of secondary container labels and the labels per page for each label size. There is a standard Secondary Container label and also the same label with NFPA Ratings. To print a label: 1/9/

33 Figure 37 Product Details Labels 1. Choose the label tab desired (Secondary Container Labels or Secondary Container Labels with NFPA) 2. Choose the button for the label size desired. 3. The label will appear in a pop up window using Adobe Reader. 4. Use a color printer to print the label in the label size desired. 5. Click the Printer icon from within the window to print. 1/9/

35 Storage Compatibility Details Window Use the Storage Compatibility to find what products can be stored along with this product. Figure 40 Product Details Storage Compatibility To Use: 1. When you click on the Storage Compatibility, all products that meet the three criteria on the sub tabs will display. 2. Click on the Tab headings to view the products in each tab. 3. Use the Search field to find a product name if there are numerous products in the list. Type in the name you want to find and then select the Search button. Click the blue hyperlink to view the regulatory information for that chemical. 1/9/

Exposure (exposure limits and carcinogen information) c. Hazard (OSHA Hazard Categories) d. Waste (RCRA Information) e. Synonyms (Listed Synonyms for that chemical) f. CA (California regulations for the chemical) g. MA (Maine regulations for that chemical) h. MI (Michigan regulations for that chemical) i. NJ (New Jersey regulations for that chemical) j. PA (PA regulations for that chemical) EMERGENCY CONTACTS The Emergency Contacts area allows you to view all emergency contacts for your organization where you can quickly find a contact when in an emergency situation. Each safe use guide provides valuable information such as the potential hazards and safety precautions and emergency response procedures for fires, spills and injury s for each category of chemical located in your workplace. 1/9/

41 DEFINITIONS The Definitions link provides simple definitions for such items as Target Organs, Physical and Health Hazards, Routes of Entry, and other technical terms associated with most SDSs. Figure 50 Definitions To Use: 1. Click on the Definitions link on the left navigation. 2. The Definitions page will display. 3. Click on the treeview navigation to view additional information to view more information. TRAINING The training link provides training on the System. Depending on the type of business you have (Healthcare or non Healthcare), the behavior of the Training screen will change. The access of what GHS Training Modules you have will depend on the contract signing for your access to MAXCOM. MAXCOM has contracted to work with two LMS providers. For Healthcare, the provider is Healthstream. For non Healthcare the provider is Learn Something. This training is based on Facility level. The training link is not available if you are looking at the Company Level in Select Locations. Both companies provide their own technical support. 1/9/

43 Figure 52 -GHS Training (LearnSomething) ENVIRONMENT OF CARE The Environment of Care is defined by The Joint Commission (TJC) (formally Joint Commission on Accreditation of Healthcare organizations (JCAHO)). A health care organization s environment of care encompasses the physical environment in which patient care, treatment and services take place. This physical environment includes the building and its arrangement and features; the equipment used to operate the building and support patient care; and the activities involved in maintaining a safe and functional environment for patients, visitors and staff. The Environment of Care section of allows you to keep track of these various areas to be easily accessed by employees. This feature is optional and needs to be added by MAXCOM. If you have a large list of plans, you can sort and search the window to find a message by clicking on the input box and typing any part of the data you are looking for. Click the search button. In this location, all other functions can be accessed using the left hand navigation as well as items below the list and ability to view system messages. Each of these abilities will be discussed separately. Summary Button The Summary button brings up the Summary tab from the chemical details and defaults to the Summary of the Material. MSDS Button The MSDS Button brings up the SDS tab from the chemical details and defaults to the MSDS of the material. Safe Use Guide Button The Safe Use Guide button brings up the Safe Use Guide tab from the chemical details and defaults to the Safe Use Guide tab. Safe Use Guide Icon The Safe Use Guide icon tells you the level by color of the material. The Safe Use Guide Number indicates what Safe Use Guide is assigned to that level. The number does not signify how dangerous or safe the chemical is. To describe the levels: 1/9/

46 4. Red Corresponds with the GHS Signal Word Danger 5. Yellow Corresponds with the GHS Signal Word Warning 6. Green Does not correspond with a GHS Signal Word but material can be considered relatively safe. GHS Pictogram If a material has a GHS Pictogram attached to it on the chemical list, hover over the pictogram to obtain more information of what that pictogram represents. 47 4. Manufacturer Part Number Once you enter an item in the search box, the Search and Reset buttons will enable. Click the Search button to start the search or press <Enter> on your keyboard. 50 Sorting the Chemical List: Product Name Lists the Product Name or the Synonym if listed on the SDS. Clicking on drop down arrow to the right the product name heading in the column will provide a menu to choose if you want to sort by ascending or descending order. The arrow directly to the right of the Product Name Heading will show which direction the list is currently sorted. In other areas you will find similar navigation bars with the same ability. In summary each is described: Figure 67 Record Navigation First Takes you to the first page. Previous Takes you to the previous page. Page view manually enter the page you would like to view and total number of product pages. Next Takes you to the next page. Last Takes you to the last page of records. Refresh Refreshes the chemical index. Records per Page Choose the down arrow to change the number of records displayed on the page. 1/9/

(see separate explanation) Prints a PDF version of the Chemical List Displays the number of materials on the page and total number of materials in the filter or search. 53 EHS SNAPSHOT The EHS Snapshot allows the user to view all chemicals that may contain ingredients that are associated with a number of SARA or other Right To Know regulations. This feature works best when all information has been added to your Chemical Inventory including Ingredient Names, CAS (Chemical Registry System) numbers and percentage of ingredient. Please note that some reports also provide a disclaimer and please note those disclaimers in your review of the report. Please also note this report may take some time to generate dependent on the amount of records you have in your system. To use: 5. Click on the EHS Snapshot icon. 6. 55 PRINT LIST The Print Index function allows you to print a PDF version of the chemical list. To use: 1. Select a location from the Select Location (you cannot perform a print list at the Company (Highest) level in the location tree. 2. Click the Print List icon. 3. A message box will appear letting you know the report is being printed. Click OK on the message box. When completed, a box will appear stating the report is complete. 4. The report will appear as a PDF document. You will be asked to open or save the document depending on the web browser you are using. Figure 71 Download window Print Index 5. 57 Viewing the SDS Details To view any information about a product, select and click on the hyperlink. Storage Compatibility view the storage compatibility with other products. 6. PPE view the Personal Protective Equipment. 7. Ingredients view the composition information. 8. Edit view additional items that require administrative access. Full explanation of each of the tabs is listed below. Summary Tab Details Window The summary tab contains the Hazard Statements and Warning labels for the product as well as summarized information provided in various sections of the SDS. If any documents, groups or locations are assigned to this product, you can find it in the summary tab by scrolling down the list of information. 65 You have the ability to add location specific information, label notes, documents, part numbers and groups as well as assign locations to products. Each sub tab is explained in detail below. Figure 86 Edit Product Details LOCATIONS Use the locations tab to view and assign additional locations for the product. Click on the check mark beside the location you want to add. This will automatically assign the product to the location. To un assign, unclick the checkmark beside the location you want to remove. Once you remove a checkmark, the product becomes archived for that location. This will archive the product for that location. Figure 87 Edit Locations Tab LOCATION SPECIFIC INFORMATION Use the Location Specific Information tab to add additional information about the product that will be based on the Location Filter you have chosen at the top of the Chemical List window. DOCUMENTS The Documents Tab allows you to upload documents that pertain to additional information for the MSDS Including and not limited to, Technical Product Data, notes, etc. that can be accessible by General and Superusers. General and Superusers have only view access to these documents. Since these documents are viewing at the MSDS Level, please use descriptive naming conventions for them for your users. The Facility Specific Documents are viewable under the same location so you want to make sure that users can identify which document type that you have uploaded. 1/9/

75 TRAINING The training link provides training on the System. Depending on the type of business you have (Healthcare or non Healthcare), the behavior of the Training screen will change. The access of what GHS Training Modules you have will depend on the contract signing for your access to MAXCOM. MAXCOM has contracted to work with two LMS providers. For Healthcare, the provider is Healthstream. For non Healthcare the provider is Learn Something. This training is based on Facility level. The training link is not available if you are looking at the Company Level in Select Locations. Both companies provide their own technical support. Our MAXCOM Administrator was given the information to access the training and they provide their documentation to support it. If you need additional information, please try the MAXCOM Administrator at your facility. Super users can also view this information however, the Users Tab is removed. You cannot edit the Users unless you are an admin user and change select location to the top most level of the location tree. There are 4 user groups. There is one more that is optional. 1/9/

77 Figure Role Administration Company Admin a company administrator is a user that has overall add/edit/archive/delete access to every facilities with MAXCOM GHS. This user is assigned to all facilities. To view specific facilities information, it is suggested to use select location and choose a specific facility. Facility Admin a facility administrator is a user that has overall add/edit/archive/delete access to the facility/facilities they are assigned to. During login, this person is directed automatically to his specific locations information or if assigned to more than one, the first one alphabetically. Superuser a Superuser is a user that has some administrative ability and can assign MSDSs to the facility or facilities they are assigned to. This user is facility/location specific. This user cannot view the User s tab or be able to add/change/delete users. General User A general user that has read only access to the facility or facilities they are assigned to. This user is facility/location specific. Departmental User If your MAXCOM GHS is set up for departmental users, this is a user who can only view a specific department within a location. Please see the chart in Appendix A to identify what each security role can perform 1/9/

79 user is denied access at this time. Click Submit to save the user. If the username exists in our MAXCOM system, you cannot use that username. You will need to select another username. Hint: If you want the user to view other facilities but only edit in one facility, choose General user in the Role area. 4. (Note: if you have an extended list of locations/departments you need to edit the user level for, hover over any of the headings (no access, general user, super user, admin user) and click on the heading this action will assign the user permission to all locations/departments at one time.) 5. To edit an existing user, select a user from the list of created users, and then click on the Edit button. 6. Make the changes you need to make to the user and then click the Submit button. 7. To delete an existing user, select a user from the list of created users, and then click on the Delete button. Before you delete, we confirm that this is indeed what you want to do before allowing the delete, click Yes to continue the deletion process. Otherwise, choose the No. 1/9/

80 8. To reset a password for a user, click the Reset Password button. Select Yes to reset or no to cancel the request. The user will receive an e mail with a new password. Click the search button to find the user. You can also sort the columns and select what columns you would like to have displayed. Additionally you have the ability to Export a list of users. To Export Users: 1. Click the Export button on the bottom navigation. 2. Click the Open button to open the file in a spreadsheet compatible program. 3. You can save this file to your computer by clicking on the Save button. 4. Click the Cancel button to remove the popup for the file. 1/9/

81 Un Assigned Users: The un assigned users lists all users marked in the No access rights category. Assign the user to a facility and a security role and then click the Submit button. Locations The locations tab is where you can add/change and delete locations. You can also assign chemicals (products) to a specific location or many locations. You can also export a list of the locations as well as Export Locations with materials assigned. Regulatory List The Chemical Regulatory List is a list of over 4000 chemicals and over synonyms from our chemical regulatory database. These chemicals are not associated with the actual MSDS ingredients. Use the Chemical Regulatory List to find out information about chemicals either by chemical name or by CAS (Chemical Registry System) Number. To use: 1. Click on Regulatory List tab within the Administration navigation. 2. A list of chemicals will appear. 1/9/

You can also find a chemical by sorting by CAS Number or by Chemical Name in the CAS Number and Name columns by clicking on the column name. 4. Double Click on the CAS Number of the desired chemical to view more details of that chemical. If there is no information for a tab, the tab will be disabled. The views of that chemical are as follows: a. Federal Regulations (EPA) b. Exposure (exposure limits and carcinogen information) c. Hazard (OSHA Hazard Categories) d. Waste (RCRA Information) e. Synonyms (Listed Synonyms for that chemical) f. CA (California regulations for the chemical) g. MA (Maine regulations for that chemical) 1/9/

To close, click the Close button to be returned back to the Regulatory List. Hazardous Materials Audit The Hazardous Materials Audit allows you to keep a periodical list of inventory counts of your chemicals onsite. You can also sort the columns and select what columns you would like to have displayed Chemical Approvals Chemical Approvals is used for materials approved prior to having them added to your chemical inventory. This function is fully configurable and how you would like the system to work with your current chemical approval process. You can setup Chemical approval options in the Administration area. The prerequisite to setting up is that you select each specific facility from within Select Locations to utilize. The Chemical Approvals setup does not initiate until you choose names for your users who will use the chemical approval process. The security level for use of this process are superusers and administrators for reviewing and approving. 1/9/

Approval Request Decision Setup: Choose the number declaring the number of approvals needed before the material is approved for onsite use or denied. 2. Approval Request Forms Setup: 3. Check if the MSDS Document must be uploaded with each approval request. This is required if you want MAXCOM to process your product after final approval. 4. Check if you want the chemical approval request form uploaded with each approval request. This is required if you want MAXCOM to process your product after final approval. 5. If you check the A chemical request form must be uploaded with each approval request., a pop up appears asking you to upload your chemical approval document. Select the Browse button and find the chemical approval form and when selected, click the Submit button to save it to MAXCOM. 1/9/

100 6. To cancel, click the Cancel button. 7. To view the Chemical Approval Request that was uploaded, click the View Document button. 8. To replace the Chemical Approval Request Form, click the Replace Document button and the Upload a Document window will re appear for you t reload your document. 9. Request Response Setup: Setup your messages with the appropriate words for an approved chemical or denied chemical and whom the is from. 11. When the requestor fills in a request to the requestor. b. When a pending request item was reviewed by one of the chemical approvers to all the approvers. c. When a pending request item was fully reviewed and approved to all the approvers. d. When a pending request item was fully reviewed and denied to all the approvers. e. When the product was added to the MAXCOM Database to all approvers. Once you add your values and upload your chemical approval document, the Pending Requests and Completed Requests will enable for administrators and superusers who were selected to use the approval system: 1/9/

This chemical may need to go through a number of approvals prior to being allowed onsite. To use: 1. Click on Request New Chemical. 2. A window appears requiring you to enter in information about your chemical. Based on the setup rules you may be required to add a company chemical request form and/or the SDS for the product. If required, click on You must complete this Approval Request Form and load this document to your local computer. 6. Use the Browse buttons to find the documents required to upload with your request. 7. To add the MSDS, click the brows button to find the MSDS for uploading. 8. Completed Requests: Completed requests is your historical view of all requests made and finalized. You can Re open any of these requests to add them to the pending requests lists. New Materials: New materials lists all newly added MSDSs by MAXCOM. It also lists the date the MSDS was added to MAXCOM, the product name, the manufacturer name, maxcomid and revision date. The MaxcomID is hotlinked to the MSDS. To view the MSDS, click on the hotlink Revised MSDS: The Revised MSDS tab displays all MSDSs that were revised in a specific time period. This will list product name, manufacturer name, maxcomid and revision date. The MaxcomID is hotlinked to the MSDS. To view the MSDS, click on the hotlink. Uploaded MSDS The Uploaded MSDS tab shows all MSDSs that were uploaded and what status they are in the MAXCOM system. This lists the upload date, name of the file submitted, Status date, and status (Posted, Pending Post, In Process, No Joy). If it states No Joy, the reason for the status is shown. ENVIRONMENT OF CARE The Environment of Care is defined by The Joint Commission (TJC) (formally Joint Commission on Accreditation of Healthcare organizations (JCAHO)). A health care 1/9/

109 organization s environment of care encompasses the physical environment in which patient care, treatment and services take place. This physical environment includes the building and its arrangement and features; the equipment used to operate the building and support patient care; and the activities involved in maintaining a safe and functional environment for patients, visitors and staff. The Environment of Care section of allows you to keep track of these various areas to be easily accessed by employees. This feature is optional and needs to be added by MAXCOM.
