1 Welcome to Modio Health Quick Start Guide Created for medical professionals by a physician-led team, our online based platform provides a central and secure location to store and manage your credentials and licensure. Sign up takes minutes and you will immediately be able to access to view your credentials, renew your DEA licenses and more. Rest assured - your profile details will never be made public. 2 The Unified Provider Record Ambulatory Surgery Center Information exchange in healthcare is confusing, repetitive, and painful. OneView offers a centralized source of provider healthcare information including NPI, licensure, DEA registration, important documents and tasks, with the ability to automatically collect and keep this data up to date The record can be accessed across organizations to limit the amount of information transferred between parties. Our goal is to minimize, or even eliminate, the need to send redundant information that can be kept in one place and accessed by all parties. 4 Licensure Alerts and 1-Click License Renewals Our 1-click license renewals and automatic alerts mean you will never have to deal with an expiring State or DEA license again. Our Licensure page, under the Manage Credentials tab allows you to manage multiple licenses, all in one central location. Color coded Indicators: Yellow: There are 60 or fewer days until the license expires. Red: The license has expired.

5 OneView Credential management for your entire team. All in one place. Documents Upload, Download, & Share Modio Health OneView also offers the ability to store any type of document. From the Documents tab, you can quickly upload one or more files. To upload a single document, simply drag and drop to the platform. For Batch document uploading, click the link in the box with dashed lines. Note, OneView autodetects the type of documents you upload. Click the? titled File Name Guide for quick reference. To Upload: 1. Name the file(s). 2. Select the file Type(s) from the drop-down menu. 3. Select the file Permission(s), which determines who has access to the document. 4. Click the Expiration date(s). Expiring or expired documents will show up in the Issues list. 5. Click Upload. To download any document, click the button for the desired file. Click to get a zip file of all documents. Public Private Permission Key Visible only to your team. Visible only to you. Click to share your documents. Admin Coordinator Visible to Modio Administrator. Visible only to Coordinator.

6 Hospital Affiliations Adding Primary & Secondary Affiliations Easily add your current practice affiliation(s) and other hospital affiliations under the Manage Credentials tab and in the Hospital Affiliations section. To get started: 1. Click. 2. Enter Hospital Name. 3. Select Primary affiliated and/or Currently affiliated for this hospital. 4. Enter Start and End Dates (if applicable). 5. Select Credentialing Date, Last Credentialing Date and Reappointment Date. See color coded alerts if those dates are expiring or have expired. 6. Select your Staff Category from the list of options. 7. *For Primary affiliation, enter the Contact Name, Title, and Phone Number. Color coded Indicators: Yellow: there are 30 or fewer days remaining for your reappointment date. Red: The affiliation date has passed and needs attention.

7 Tasks Your Personal To-Do list Your Practice Coordinator will send you tasks when your credentials approach expiration, if your profile requires attention, or if there is an action that needs to be completed. Example Tasks: 1. V i e w i n g a d o c u m e n t t h a t requires a signature. 2. Opening a link to an article or video. Be on the lookout for new Tasks on the top menu bar. Track your tasks here and quickly see items that are in need of attention. Also, you can use Tasks to send reminders to yourself or assign them to other members of your team.

8 CME Tracking Creating Goals and Adding Credits Modio Health helps you track and organize your CME goals and credits more efficiently. Through our partnership with Challenger, all Modio Providers will receive a discounted rate for their CME Courses. To receive your discount, click on the CME link on your dashboard. From the CME tab, your goals and requirements will be preset from your active licenses. To Add Credits: 1. Select the button. 2. Choose which Goal this CME Credit corresponds to. 3. Enter the number of Credit Hours completed. 4. Select the calendar icon to enter the Date Completed. 5. Add optional Description. 6. Check the box Count Towards Goal(s) as necessary. 7. Select the Organization associated with your credits. 8. Lastly, select the type of CME Content you are tracking. 9. Click Save. 10. Upload certification of completion if available. To Edit/Delete CME Credit: 1. Click the icon under the Actions column. You can edit by changing the Dates or Status of the Credit. 2. To delete a specific Credit, click.
