1 L.A. Care Connect Account Creation User Guide for L.A. Care Covered Step 1: Create an L.A. Care Connect account by opening any web browser and typing in members.lacare.org in the URL address bar. Step 2: You will be directed to the following webpage. Date compiled 13 December

2 Step 3: Click on Create an Account Step 4: You will be sent to the License Agreement webpage to either agree and Continue the account creation or cancel the account creation of you disagree with the License Agreement. Date compiled 13 December

3 Step 5: If you agree with the License Agreement and press on Continue, you will be sent to the webpage asking for your membership information. Once entered, please press on Continue. Date compiled 13 December

4 Step 6: You will be prompted to enter your log in credentials and set Security Questions. Please enter all required information. Date compiled 13 December

5 Step 6A: For Security Questions, the questions will be the same for all three questions. Please select the questions that apply to you. Step 7: Once all security log in credentials have been set and all security questions have been selected and answered and you have pressed NEXT, you will be directed to the Registration confrmation page. 6 Step 8: You will be redirected to the HOME screen of your new L.A. Care Connect account. Here you will see a welcome message and options to make your frst payment using either a credit or debit card or a check. Date compiled 13 December

7 Step 9: You will be redirected to the electronic payment screen where you can make a one-time payment using either a credit card, a debit card, or a check or set up recurring payments using only a check from your checking account. Date compiled 13 December

9 Set up Recurring Payments Option: CONGRATULATIONS! You are now ready to select your Primary Care Physician or Medical Group of choice and make your frst payment to enable all of your benefts. The following steps will guide you in searching and selecting your preferred provider. If you have any questions or concerns, or any issues accessing any part of your account, including creating an account, problems making a payment, or any needing your password reset, please do not hesitate to call us 24 hours a day, 7 days a week, including holidays, at (TTY/TDD 711). 10 Choosing you preferred Primary Care Provider (PCP) or Medical Group Step 1: To search for your preferred PCP or Medical Group, go to My Doctor, Provider Search. Press on Provider Search. Date compiled 13 December

11 Step 2: The Provider Directory will open on a separate tab. Press on Physician to search for your Primary Care Provider. Date compiled 13 December

12 Step 3: At this screen you may search by PCP Name, License or NPI; Location including Zip Code or Street Address; Gender; New Patients Acceptance; Specialty, Medical Group, Language spoken by Provider and Staff, and Hospital Affliation. Click Go! to see a list of Providers. Date compiled 13 December

13 Step 4: You will now see the Provider Results. Click on any Provider link to see more information about that particular Provider. Date compiled 13 December

14 Step 5: When clicking on the name of a Provider, you will see additional details, including their contact information, address, License Number, Specialty, hours of operation and if they are accepting new patients. Date compiled 13 December

15 Step 6: Once you have found the Provider you would like, navigate back to your L.A. Care Connect account and press on My Doctor, Change My Doctor. Date compiled 13 December

16 Step 7: Please enter the name of your chosen Provider in the New Doctor Name feld, select the Reason for Change on the drop down menu and enter the PCP Site Number from the previous screen in the Additional Information/ Comments text box. Finally, you can have this Provider be the Provider for all of your family or for only some of your family members. Press SUBMIT and your request will be processed the next business day. An confrmation and a new ID card will be sent to you. Congratulations on selecting your preferred Primary Care Physician.
