1 CHECKLIST: REQUIRED INFORMATION FOR AUTHORIZED REQUESTER APPLICATION To make the Authorized Requester application process easier, use this worksheet to collect the organization information you will require for your application before contacting MTO Business Information Services at (416) or REQUESTING AN APPLICATION The following is a list of information you will be required to provide over the telephone to MTO Business Information Services. Reviewing this information will determine your access to the online Authorized Requester application. Registered Legal Name or Operating As: (e.g., Ontario or ABC Holdings) Previous Application #: (If applicable) (e.g., 1234) Business Type: (One of the following) Sole Proprietorship Corporation - Incorporated Government - Municipal Partnership - Limited Corporation - Federal Charter Government - Provincial Partnership - General Corporation - Letters Patent Government - Federal First Nation Government - Crown Corporation Client Type The purpose for which your organization is using MTO information products. Your client type will be determined based on the explanation you provide. North American Industry Classification System (NAICS) Code The code displayed on Canada Customs and Revenue Agency tax returns to specify the type of business. Available at (e.g., Office of Lawyers)

COMPLETING YOUR APPLICATION When you gain access to the online Authorized Requester application, you will be required to enter the following additional information. Driver Products: Vehicle Products: Carrier Products: Order Channels The communication channels over which you would like to receive your orders. Internet Telephone Mail Use of Information For what purpose will the information be used? Provide examples. Security of Information How will the information be kept secure? Provide examples (physical barriers and security features within the office, building security features, computer security features and protection of stored hard copies)

CREATING YOUR ACCOUNTS When your organization is authorized to become an Authorized Requester, you will be required to provide the following additional information to Business Information Services to create accounts. You want to create a new account for every different combination of account details. If you plan to have more than one account, print a copy of this page for every account you want to create or use the electronic template found on the Getting Started web page. Account Create a name to distinguish one account from another (e.g., name by location, intended use, class of service - immediate on-line service, overnight service etc.) Account Name Product Format(s) Abstract Pre-formatted products available for viewing and printing on your desktop Raw Data Data strings delivered for client reformatting. (See Batch File Input/Output Formats.) Account Contact(s) The main person to contact for questions related to the account, including credit card deposits. Position Description: Account Location The mailing address where certified products can be mailed for this account. Mailing Address: Physical Address (if different)

6 4. ADDING USERS When your application is approved to allow your organization to become an Authorized Requester, you may add other users who can order products. If someone other than the application contact will be placing orders, print a copy of this checklist for every user. If you plan to have more than three additional users, print additional copies of this page or complete the electronic template available on the Getting Started web page.
