1 Step 1 IC Online Registration Parent Guide Access your Online Registration via the URL. Step 2 Note: The recommended browsers to use are Google Chrome and Mozilla Firefox (most recent and previous versions) or Safari 6 or 7. Before beginning the application, turn off your Popup Blockers on your browser. Select which language you would like your application to be in by clicking on the blue link. Step 3 Click on the Start New Registration button to begin your registration. NOTE: If you have already started an application, click on the Return to Saved Registration. Step 4 Fill out the required (required fields are indicated by a red star) information for the Parent/Guardian completing this application. NOTE: DO NOT put student information here unless the student is his/her own guardian. Step 5 Step 6 Please type the upper case and lower case letters and/or numbers in the box. Click on the icon next to the box to get a new image. Click the blue Begin Registration Forms button Please take note of the confirmation number. Then click OK button. NOTE: You will need this number if you have to return to your application for any reason. If returning to an application, use the same URL you used to access the registration form and enter the application # where indicated. 1

2 Step 7 After reading the statement, type your whole name to confirm that you have read and agree to the terms of this application. Click the blue Submit button. Step 8 Please read the Centralized Admissions Welcome letter. Please take note of the supporting documents that you will need to bring to the Centralized Admissions office when you complete your application. Once you have read the letter, click on the Begin button at the bottom of the letter. Step 9 1. Enter your Home/Primary Phone Number. 2. Check the second box if your phone number is unlisted. 3. From the dropdown menu, select your primary contact language. Once you have entered your information, click the grey Next button. NOTE: If you do not have a phone, click the No Home/Primary Phone check box. 2

3 Step Enter your address in the required fields (required fields are indicated by a red star). 2. As you enter your address, if it is already in the system you will see it populate in the highlighted area. Click on the correct highlighted address once it displays. If it is an apartment, make sure you select the correct apartment number. Click on the grey Next button. NOTE: If you do not have a physical address, check the Please check this box button at the top of the screen. Step 11 If your address does not populate in the green area, proceed to the Next button. If your Mailing Address is different from your Physical Address, uncheck the box and repeat Step 10. If your Mailing Address is the same as your Physical Address, click on the blue Save button. Step 12 Please read the statement and then click on the grey Ok button. Step 13 Fill out the required (required fields are indicated by red stars) Parent/Legal Guardian information. Enter birth date --/--/----. Click on the grey Next button. NOTE: Parent/Guardian information should match the Parent s/guardian s ID. 3

4 Step 14 If the Parent/Guardian lives at the home physical address, click the grey Next button. If the Parent/Guardian lives at a different address, uncheck the checked box and follow the information in Step 14b. Step 14b NON-HOUSEHOLD PARENT If the Parent/Guardian does not live at the home physical address and you do not know their address, check the I will not provide check box. Then click the grey Next button. Enter the non-household parent s address in the required fields (required fields are indicated by a red star). As you enter the address, if it is already in the system you will see it populate in the green area. Click on the green address once it displays. Enter the home/primary phone number for the non-household parent. Click on the grey Next button. NOTE: If you do not have a physical address, check the Please check this box button at the top of the screen. Step 15 If your address does not populate in the green area, proceed to the Next button. 1. Enter the Cell and Work Phone numbers for the Parent/Guardian you entered, if they have them. 2. Enter the Parent/Guardian primary Address. You may also enter a secondary address if they have one. 3. After entering the Parent/Guardian Address, uncheck the boxes that you DO NOT want to receive s about. Click on the grey Next button. NOTE: If the Parent/Guardian does not have an address, check the Has no box. 4

5 Step 16 Answer each of the three questions. Yes answers may require additional information by using drop down boxes. NOTE: For active duty date, use today s date. Click save when done. Step 17 If you need to add another Parent/Legal Guardian click on the blue Add Parent(s) button and repeat Steps NOTE: This section is ONLY for legal Parents/Guardians. If you have entered all Parents/Guardians, click on the blue Save/Continue button. NOTE: If you need to add or change any info, click on the blue Edit button next to Parent/Guardian name you wish to edit. Name(s) highlighted in yellow indicate that a person is missing required information. Click on the blue edit button to correct issue. Step 18 Add any Emergency Contact by clicking on the Add New Emergency Contact button. NOTE: An Emergency Contact is anyone who is NOT a Parent/Legal Guardian or high school student. If you do not have any Emergency Contacts, check the By checking this box button. Click on the Save/Continue button. NOTE: If you choose this option, APS will not be able to contact anyone other than the Parent/Guardian in the case of an emergency. 5

6 Step 19 Please read the statement and then click on the grey Ok button. Step 20 Fill out the required (required fields are indicated by red stars) Emergency Contact s information. Please use their legal name no nicknames. Click on the grey Next button. Step 21 Enter at least one phone number for the Emergency Contact. You can also enter an address if you know the contact s address. NOTE: If the Emergency Contact does not have a phone number, READ THE STATEMENT and check the I have read and agree check box. Click on the grey Next button. Step 22 DO NOT CHECK THIS BOX even if the contact lives at the address shown. If you have the address for the Emergency Contact you are adding, enter the address in the spaces provided. If you do not know the address of the Emergency Contact, click the blue Save button. 6

7 Step 23 If you need to add another Emergency Contact click on the blue Add New Emergency Contact button and repeat Steps NOTE: Name(s) highlighted in yellow indicate that a person is missing required information. Click on the blue edit button to correct issue. If you have entered all Emergency Contacts, check the I authorize officials of box. Click on the blue Save/Continue button. NOTE: If you need to add or change any information, click on the blue Edit button next to Emergency Contact name you wish to edit. Step 24 Skip this section and click on the blue Save/Continue button. NOTE: DO NOT click on the Add New Household Member button. Step 25 Click on the blue Add New Student button. Step 26 Enter the student s information. Enter student s complete name as it appears on the student s birth certificate. NOTE: You can also enter your student s Nickname, cell phone number and address if you have it. 7

8 Step 27 Answer the questions by selecting a choice. Step 28 In Part A, from the dropdown, select whether the student is Hispanic/Latino. In Part B, check the box(s) that describes the student s Race/Ethnicity. NOTE: You may choose more than one option. If you select American Indian or Alaska Native you will be prompted to complete the Title VII Indian Ed form. This form is only for those that are US born Native American/Alaskan Native families and belong to a federal or state recognized Native American tribe. In Part C, from the dropdown, select how the student s Race/Ethnicity was identified. NOTE: if you are the parent/legal guardian, select Parent Identified. 8

9 Step 29 Read the questions in each of the sections, and answer each question. NOTE: If you check the Yes boxes in the first two sections or any of the boxes in the third section, a dialog box will appear. Enter more information about the program the student was/is in. If you student has an IEP or 504 plan, please bring it with you to Centralized Admissions. Step 30 Read each of the questions in each of the sections, and select your answer. NOTE: If you check the No box in the first section, a dialog box will appear. Enter more information about why the student is not in Good Standing at their previous school. If you check the Yes box in the second section, a new box will appear. Check which option best applies and enter the expulsion start and end dates. See the EXAMPLE 1. EXAMPLE 1 Click on the blue Safe Environment Notice link. Once you have read the document, close the window, then check the I have read box. NOTE: You must read the Safe Environment Notice before checking the I have read box. You will not be able to check the box till you have read the document. 9

Print out the document, fill it out, and bring it with you to the Centralized Admissions Office when you come to verify your application. In the first section, enter the information for the last three schools that the student has attended. NOTE: If your student has never attended a school before, skip this section. See EXAMPLE 1. In the second section, read the description and enter the requested dates. Date entered US will only show if the child was born outside the United States. NOTE: EXAMPLE 2. In the third section, read the question and select the appropriate answer. NOTE: If you select Yes, two dropdowns will appear. Select the previous school that the student attended and their grade level. EXAMPLE 3. In the fourth section, read the question and select the appropriate answer. NOTE: If you select Yes, three dialog boxes will appear. Enter the School/Program information that the student is currently enrolled in. EXAMPLE 4. Read the Compulsory School Attendance statement. Check the I have read box. Click the grey next button. 10

11 Step 34 EXAMPLE 1 EXAMPLE 2 EXAMPLE 3 From the dropdown, identify the relationship that each Parent/Guardian has to the student. NOTE: The options are listed with the Parent/Guardian relation first, then the student relation (example: Father/Son, Mother/Daughter). EXAMPLE 1. If the child lives with both parents (50/50 custody), check the secondary household box next to one of the parents. If the child lives with one parent the majority of the time, DO NOT check the secondary household box. NOTE: EXAMPLE 2 To the right of the relationship dropdown, there is the Contact Sequence. From the dropdown, select the order in which a Parent/Guardian should be contacted by the student s school. NOTE: EXAMPLE 3. Step 35 EXAMPLE 1 EXAMPLE 2 From the dropdown, identify the relationship that each Emergency Contact has to the student. NOTE: The options are listed with the Emergency Contact relation first, then the student relation (example: Aunt/Nephew, Aunt/Niece). EXAMPLE 1. To the right of the relationship dropdown, there is the Contact Sequence. From the dropdown, select the order in which an Emergency Contact should be contacted by the student s school. NOTE: The sequencing numbers will not have the numbers that were used in Parent/Guardian section. EXAMPLE 2. 11

12 Step 36 Other Household Relationships From the dropdown, identify the relationship that each sibling has to the student. Step 37 In the first section, select the Health Insurance option for the student. In the second section, enter the student s doctor and phone number. NOTE: If you do not have the Doctor information, skip this section. In the third section, select which over-the-counter medication that the student can be given at school. Step 38 On this page, enter any Health Conditions that the student has. NOTE: If your student has no Health Conditions, check the No medical check box. 12

13 Step 39 Read the statement and select appropriate option for Media Release permission. Step 40 Read the statement and select appropriate option for Field Trip permission. Step 41 Read the statement and select appropriate option for Grade Consent permission. Step 42 Step 43 Click on the blue Internet Responsible Use Policy link. Once you have read the document, print the document. Fill out the document and bring it with you to the Centralized Admissions Office when you come to verify your application. Close the window, then check the I have read box. NOTE: You must read the Internet Responsible Use Policy before checking the I have read box. You will not be able to check the box till you have read the document. Read the full notice. Check the If you DO NOT box if you do not want your information to be given the Armed Forces. NOTE: This section is for 11th & 12th grade students. If the student is in Kindergarten 10th grade, skip this section. 13

14 Step 44 Step 45 Read the statement and select the appropriate option for the Parent/Guardian. NOTE: If YES is selected, a blue link will appear. Click on the link and print the document. Fill out the document and bring it with you to the Centralized Admissions Office when you come to verify your information. Click the blue Save button. If you need to add another Student click on the blue Add New Student button and repeat Steps NOTE: Name(s) highlighted in yellow indicate that a person is missing required information. Click on the blue edit button to correct issue. NOTE: If you need to add or change any info, click on the blue Edit button next to Student s name you wish to edit. If there are no more students to add, click the blue Save/Continue button. Step 46 Step 47 If any of the section tabs are red, key information is missing. Click on the red tab to go back and correct any issues. Click on the Application Summary PDF to review the information you entered. If you need to make changes, close the PDF window and click the back button. Then click on the section tab where you need to make changes. If you have verified all of your data, click the red Submit button. After clicking on the red Submit button, a window will appear. If you are ready to submit the application, click the OK button. WARNING: You will not be able to modify any information after submitting. 14

15 Step 48 Click on the blue Application Summary PDF link. Step 49 Print the Online Registration Summary document and bring it, and any other supporting documents, with you to the Centralized Admissions Office for the application approval. 15
