1 STEP DESCRIPTION 1 Login to Infinite Campus Portal Visit /portal/elmbrook.jsp If you have a portal account, enter your username ( address) and password and click Sign In If you are logging in for the first time, click activate your account If you have a portal account but forgot your password click reset your password 2 Launch Online Registration Tool Under Family, click on Messages Inbox Online Registration for Currently Enrolled Students link. 3 Begin Registration The registration process will ask you to update/confirm information for the students listed here Click Begin Registration 4 Confirm your identity by typing your first and last name. Click Submit to continue

2 5 HOUSEHOLD Household-Primary Household Phone and Home Address: Verify your household information. Home phone, house/apt number, street, city, state, zip code are all required. Click Next to advance to the next Household-Mailing Address: Families can enter an alternative mailing address, different than the primary household street address. Once the mailing address is confirmed or entered, click Save/Continue to advance to the next section. 6 PARENT/GUARDIAN Verify/enter all Parents/Guardian and indicate whether they are attached to the primary household. Demographics: First and last name, as well as gender, are required. Click Next to advance to the next Print screen of parent/guardian listing

3 Contact Info: Verify/enter as many contact phone numbers as apply. address is strongly encouraged to facilitate regular, non-emergency communication between the schools and families. Click Save/Continue to advance. Completed Parents/Guardians will be displayed. All other parents/guardians should be added by clicking on the Add New Parent/Guardian button. Once all have been entered, click Save/Continue. 7 EMERGENCY CONTACT In an emergency, the parent/guardian contacts will be called first. If unavailable, all families must provide at least one (two preferred) emergency contacts. Click Add New Emergency Contact. Enter the contact s first and last name and gender. Click Next to advance to the next Enter the emergency contact s phone information. Click Next to advance to the next

4 Enter the emergency contact s address information or check the box to indicate the contact lives at the primary household address. Click Save/Continue to confirm contacts or add another emergency contact. 8 OTHER HOUSEHOLD MEMBERS Demographics: Add New Other Household Members who are ages 0-4 OR in grades K3-12 but NOT attending or planning to attend the Elmbrook Schools. When complete, or if none, click Save/Continue to advance to the next 9 STUDENT A list of currently enrolled students is presented. Information is presented one student at a time. Follow the prompts to confirm/add the student information. Demographic and Student Information: Verify/modify the student information noting the required fields (*) on the screen.

5 Elementary families are asked to detail emergency school closing plans should students be sent home early. Please check the box most appropriate so our school and transportation staff can keep our children safe. Elementary families can also declare different End of Day Instructions. These are shared with the classroom teacher and office staff. For example, families may take the bus home M, W, F and get picked up after school on T, Th. High School students wishing to obtain a parking pass can enter information for up to two vehicles. Relationships to the student are then entered for Parents/Guardians, Emergency Contact(s), and Other Household Members. Emergency contact sequence numbers are also assigned to parents/guardians and emergency contact(s).

6 Health Services-Emergency Information: Enter primary care physician and dentist information. Consent to contact either in the case of an emergency is also given on this Health Services-Medical or Mental Health Conditions: Verify/enter all medical conditions that currently exist for the student by click Add Condition, Current Condition, or Remove Condition as appropriate. The link to the Student Immunization Form is also on this If your child is a new student or entering KG or 6 th grade, this form is required. Please follow the link, print the form, complete it, and return it to your child s school by the first day of class. Health Services-Medications: Enter all at-home only medications on this page or check the box for No athome only medications. All medications to be dispensed at school require a completed and signed form. Follow the appropriate link to print the form, complete it as the form indicates and return to your child s school by the first day of class.

7 Release Agreements Complete the release agreements section, following the provided links to review school/district policies. The School Directories are created by each school s Parent Teacher Organization (PTO) and can include parent/student name(s), primary address, household phone, and parent . The student information is now complete. To edit or correct it, click the Edit button next to the student. If applicable, verify another student by clicking Edit/Verify and returning to the beginning of step #9 STUDENT. Once all students have been verified, click Save/Continue one last time. 10 SUBMIT Families have the opportunity to print an Application Summary PDF or click Back to edit. Once all information has been verified, click SUBMIT.

8 11 SUBMISSION CONFIRMATION A final summary of the registration process will be presented. Families have another opportunity to print the Application Summary PDF. Families can also follow the link for the free/reduced lunch applications. Lastly, they can proceed to MyPayments Plus ( for fee payment and to complete the online registration/fee payment process.
