2 Background The ability to digitize histopathology slides automatically, rapidly and with high resolution has been pursued by numerous investigators around the world. One of the goals is the implementation for the clinical practice in Pathology. One of the pending issues is the speed of the entire process. The scanning time is faster and the performance of a scanner is more stable, however, the viewing speed is still not satisfactory for most users.

8 Materials and Methods PlayStation 3 and controllers were adapted to our study because it can manipulate huge graphic data without any stress locally and also over the network. We have evaluated the system in three situations at the Department of Pathology of MGH. 1. Simulation of signing out a case, 2. Discussion tool at a consensus conference, 3. Usage at a teaching course, 4. Speed comparison: local vs public network. Prior to the trials, we analyzed the required functions of the Graphic-User Interface (GUI) and Human Interface for each purpose and a slightly different GUI was made to fit each purpose ideally.

11 Requirements (not all) Type Observation Focused point Primary diagnosis Consensus Conference CME Course Slides Conference Look at all slides Changing magnification Quick interaction with LIS Comparison IHC/Special Stain Look at selected slides from many slides Often use pointers with all/some attendees Often revisit the spots Combination of microscope + camera and WSI Unpredictable No time for training of audiences Every speaker has to be able to operate elegantly without training Begin to look at the slides in order then select the slide from the list (because of time) Quickly moving to next slide and back Marking the slides have been reviewed Synchronize multiple images Quickly selecting a slide from many slides Easy to use pointers with all/some attendees Marking (bookmark) spots with all/some attendees and revisit the spots Easy to switch an operator Compete microscope + video Simple to use Secret supporting function Quickly moving between slides Quickly selecting a slide from many slides

16 Results The system had remarkably good performance. The points of evaluation were 1. user-friendliness 2. less frustration 3. fit to purpose.
17 Results Pathologists were able to use the system comfortably after 0-15 minutes of training. Pathologists who played games regularly at home required 0-3 minutes of training. Pathologists who never played games required a minute training session. There were no complaints regarding the speed. Most pathologists were satisfied with the functionality, usability and speed of the system. The most difficult situation was to simulate the signing-out of cases

21 Conclusions The preliminary results of our Ultra High Speed WSI Viewing System were promising. The speed of viewing the images demonstrated the possibility of using WSI in daily practice in the near future. We continue to evaluate the system for further improvements.
