Creating a Feedback activity This skill sheet will demonstrate how tutors can create a Feedback activity in a Moodle course. The Feedback module allows you to create and conduct surveys to collect feedback. Unlike the Survey tool it allows you to write your own questions, rather than having to choose from a list of pre-written questions and unlike the Quiz tool, you can create non-graded questions. Feedback responses may be anonymous if desired, and results may be shown to all participants or restricted to tutors only. Any feedback activities on the site front page may be completed by nonlogged-in users. Feedback activities may be used For course evaluations, helping improve the content for later participants To enable participants to sign up for course modules, events etc. For guest surveys of course choices, school policies etc. For anti-bullying surveys in which students can report incidents anonymously Follow these simple steps to create a Feedback activity: 1. Click the Turn editing on button (top right of the screen). 2. Select the section (topic) of the course you would like to create the feedback activity in. Now click the Add an activity or resource link. The Add an activity or resource window will open. 3. Under the Activities heading, select Feedback and then click Add. This will launch the Adding a new Feedback activity page.
4. Give the Feedback activity a Name this is the link the students will click on from the main course page to access the feedback activity. 5. Add a Description provide a summary of the feedback activity. You can make the description available on the course page by selecting Display description on course page. 6. Under the Availability tab you can set the date range for when users can complete the activity. 7. Under the Questions and submission settings heading you can allow multiple submissions, set submissions to anonymous, enable notification of submissions and enable automated numbers for each question.
8. The After submission settings allow you write a completion message, show an analysis page of results (to students) and provide a link to the next activity. 9. Once you are happy with the options you have chosen, scroll to the bottom of the page and click on Save and return to course. This will create your Feedback activity and take you back to the course page. The next step is to add questions to the activity. 10. On the course page click the link to enter the feedback activity. At the top of the page you will see the following menu: 11. Click on Edit questions to add questions to the activity. From the dropdown list select the type of question you would like to create. Here are the options: Longer text answer This option is for creating a text box (you specify how big it will appear in rows and columns) which people can write a long answer into. Multiple choice - This will give you a series of radio buttons, which starts on 'Not selected' and then has your options afterwards. Only one can be chosen and 'Not selected' is a valid answer if the question is not set to 'required'. Multiple choice (rated) - This is similar to the other multiple choice options, except that each option
has a numerical value associated with it. You have the choice of using radio buttons or a drop-down list to display the answers. Numeric answer - Here, you ask a question which must have a number as an answer and specify the acceptable range e.g. "How many arms would you like, if more than 2 were possible? (Please specify 0-10)" with a range of 0-10 set in the options. It helps if you specify the acceptable range in the question text. Short text answer - This option lets you specify a single line answer, with an input box which is a set number of characters long (you choose). You also specify the maximum number of characters you will accept, so that the answer is not too long and/or does not run over the length of the box on screen. You also have some additional format and layout options in this dropdown menu, these are: Captcha - A test to make sure a real person is filling in the form and not an automatic spamming program of some sort. Asks a person to write out some distorted text which is displayed on screen. You normally won't need this unless you find you are getting spammed a lot. Information - This allows you to choose to display one of three types of information: the time of responding; the course and/ or the category where the feedback is located. Label - Like a standard Moodle label, this allows you to add arbitrary text between questions allowing for extra explanation or to divide the Feedback into sections. 12. Select the question type you would like to create in this example, a short text answer questions will be created. You should now see the following page:
The options you see when creating a question will depend on the question type you have created. Click Save question to add it to the activity. Repeat the process above to add additional questions to the activity. 13. On the menu, you will see that there are three other options: Templates, Analysis and Show responses. Under the Templates tab you can save questions as a template for future use, or use a template that has already been created. You can also export questions from the activity should you wish to import them into another course. Under the Analysis tab you can see a graphical summary of the results of each question, along with the option to download the results to Excel. Unless the responses are set to anonymous, there will also be a "Show non-respondents" tab where the Teacher or Feedback activity administrator can select some or all users who have not yet completed the feedback and send them a reminder message. Under the Show responses tab you can view all the responses and delete individual responses.
