1 Entering AC Results in the New PT Lab Center Please note: Instructions specific to AC results begin on Page 4. Browser Information: Please access the PT Lab Center using the preferred browsers - Google Chrome or Firefox. Using Internet Explorer or other outdated browsers may cause technical difficulties. 1. Login through the ASHI website. Go to and click Sign in securely. Enter your personal username and password. PT results are not accessible through Lab accounts. If you are an ASHI member, use your membership username and password to login. If you are unsure of your username and password, please contact Mandi Cruz at mcruz@ashihla.org. Expired, lapsed, or unpaid ASHI membership accounts will have to be renewed or switched to a Non-member account prior to gaining access. To renew and make a payment, please visit the ASHI website. To have your account switched to a Non-member account, contact the PT Coordinator at chartman@ashi-hla.org. Otherwise, you will not be able to access the new PT Lab Center to enter, view, or submit PT results. If you need to establish an account (member or non-member), please see the Login and Account Procedures Manual on the ASHI PT page before continuing.

2 When using a new Non-Member account, you may be directed to the following page during login. If you see this page, click Continue with Current Membership to proceed. Select Lab Programs tab and select Proficiency Testing Program. On the Proficiency Testing page, click the New PT Lab Center link in blue to access the new PT Lab Center. Uncheck the Remember Me box and enter your personal username and password then Sign In.

3 2. Log into the Lab Center. On the Lab Center Homepage, click Log In to proceed to the PT Lab Center Dashboard. If you receive the following message: We could not determine your associated lab, there are a few instances when this error appears: If your ASHI member account is inactive, you must renew the account or have it switched to a non-member account. To renew your account or pay your balance, sign into the ASHI site and proceed with the steps to renew. mcruz@ashi-hla.org if you have any issues renewing. If you do not wish to renew, please contact the PT Coordinator at chartman@ashi-hla.org to have your account switched to a Non-member account. If the computer remembers you from your last session in the lab account, you must logout of your last session completely before proceeding. Please access the Login and Account Procedures Manual (Full Logout section) on the ASHI PT page for further instructions. If your personal account is not properly linked to your lab account. See the Login and Account Procedures Manual (Linking a Personal Account section) on the ASHI PT page for further instruction. The Dashboard includes the following tabs for the new PT Lab Center. Please be sure you are in the correct Lab record. Check the PT Lab name - upper right corner.

4 3. Enter AC Results. Select Results Submission, then Enter/Submit Results. Please be sure you are in the correct Lab record (see Lab column), correct Shipment (2 for 2nd shipment of the year), and correct Survey Type (AC) before continuing. To enter results for the second AC shipment of the year, click "Enter Results" next to AC under Shipment 2.

5 Select Specimen Verification and Quality Assessment tab to enter the correct sample numbers and quality of each sample. If your lab ordered the AC module, enter the quality of the whole blood specimens in the top two Whole Blood Specimen sections. The remaining sections are for the serum samples. Please note: If your lab did not order whole blood samples - and ordered the AO module instead - you can move directly to entering the quality of each of the serum samples. The Whole Blood Specimen sections should only appear for labs who ordered the AC module, not the AO. If the Whole Blood Specimen sections appear on your page and your lab ordered the AO module, please notify the PT Coordinator, Cheryl Hartman at chartman@ashi-pt.org at your earliest convenience. Enter the Quality for each specimen using the drop-down menu. When Unacceptable is selected, results cannot be entered for that specimen.

6 If you choose Unacceptable, but do not include comments for that specimen, you will receive a warning: Comments are required. After entering specimen qualities, please be sure to SAVE your entries before selecting another tab or exiting the page. Proceed to Class I & II Antibody Methods & Results tab. Only specimens listed as Acceptable or Good will be listed on Sample Number tabs across the top. Any specimens marked Unacceptable will not be listed. Select the first sample number. Please be sure to match the results you are entering to the correct sample number for each specimen.

7 [From AC instructions]: Entry for antibodies specific to class I and class II are under separate headings. Start with entering results for Class I Antibody Testing. Select the 1 st method for that specimen using the Method/Kit drop-down selection. Please note: If you select a screening method (ex. One Lambda FlowPRA Screening ), you will be unable to enter Antibody Identification results for that method. If you don t find the necessary method/kit on the list, you can choose Other and specify the method/kit type in the Comment section. Next, select the result obtained using that method/kit by selecting one of the By this method.. drop-down selections.

8 When identifying the antibodies, there are several options to choose from: None Detected (Positive) (method used as a screening test, not for specificity identification). Detected (Positive) (method used for antibody specificity identification). Not Detected (Negative) (method used as a screening test, not for specificity identification). Not Detected (Negative) (method used for antibody specificity identification). Not Tested with this method (default) Please note: If you choose Detected (Positive) (method used for antibody specificity identification), a new button - Enter Antibody Identification - will appear on the right side below the Comment section. When selected, it will allow you to enter antibody specificity results. If you enter a method as Negative or Not Detected, no result entry button will appear to enter antibody identification information for that method. If you enter one method as Positive and one as Negative, you will only be able to enter specificity results for 1, but not the other. Each box on the Antibody Identification screen contains the drop down options below. The default for each box is blank/negative. Drop-down options: P - Positive blank - Negative X - Not tested A - Allele antibodies Note: If you did not detect a specificity/the specificity was negative, leave the corresponding box under that specificity BLANK.

9 Once all the antibody identification results are entered for that method/kit, you can add any comments about allele specific details in the Comments box. Clicking Save will save your entries and return you to the Class I Antibody Testing screen. When needed, additional methods/kits can also be added by selecting Add Method/Kit at the bottom of the Class I Antibody Testing page. For each additional method used, you must select Add Method/Kit to add an additional method. If you need to remove one of the kits, the Remove Method/Kit button will allow you to do so. (The remove button only appears if you choose to Add a Method/Kit.)

10 For each method added, a new row appears under each section on the Antibody Identification results page. Row 1 = 1 st Method/Kit, Row 2 = 2 nd Method/Kit, and so on. You can add as many methods as you tested each sample for. If you enter a method or kit indicated as a screen (ex. One Lambda FLOWPRA Screen ), and you click the Enter Antibody Identification button, the row which corresponds to that kit will be grayed out and will not allow you to enter antibody identification results for that method. This will also happen if you say that an additional method/kit resulted as negative/not detected. After entering Class I results for a particular sample, proceed to enter Class II results for that sample. When all methods and results for Class I and II for the 1st sample have been entered, click Save. A message at the top of the screen will indicate which specimen saved.

11 Clicking on the next specimen tab will highlight it in red and activate entry fields for that specimen. After Class I & II Antibody Methods & Results are completely entered and saved, you can proceed to Crossmatch Results if you subscribed to the full AC survey. Click on Crossmatch Results tab. Definitions of abbreviations are provided on the top of the Crossmatch Results page: Enter Crossmatch Results for Each Method, Target and Serum Specimen Tested POS = Positive NEG = Negative [blank] = Not Tested, method not used in laboratory (DEFAULT) NT2 = Not Tested due to sample quality problem NT3 = Not Tested due to reagent problem/technical failure (e.g. unavailable from vendor, QC failure, dropped tray) NT6 = Other, specify why in comments Note: if you reported a cell or serum specimen to be "Unacceptable" for testing, it is not possible for you to enter any crossmatch test results for that sample. The data fields on this page contain drop-down menus that can be seen once your cursor is on the field. As noted in the instructions included on the page, If you reported a cell or serum specimen to be Unacceptable for testing, it is not possible to enter any crossmatch test results for that sample.

12 For each sample, you can enter CDC, AHG, Flow Cytometry, & Solid Phase. The default is blank, so if you did perform testing, you must enter results. If it was not tested, leave blank. If it was not tested due to sample issues or technical errors, utilize the appropriate NT code listed above. If you reported a whole blood specimen as Unacceptable, results will come up as NT2 - Not Tested - automatically. Specimens marked as Unacceptable can be commented upon additionally at the bottom of the page. Once results have been entered for all specimens, results can be reviewed on the Review/Submit button. To print the review page, use Control + P. If only a portion of the page is included in your printout, you may have to switch to a different browser to print all of your results.

13 Review/Submit Page When done entering results for each whole blood/serum, save results and review entries on the Review/Submit button.

14 If your lab performs AT1R, click on the AT1R tab. Specify the kit used. Enter the Concentration detected for each serum sample. Select the Interpretation of that Concentration from the drop-down menu. The data fields for Interpretation are Positive, Negative, and At Risk. Once AT1R results are entered, please enter reagent lot # information in the comments box, click Save and then go to Review/Submit tab to view AT1R results.

15 Submitting Results Once all results for Class I, II, Crossmatch, AT1R results have been entered as appropriate and reviewed thoroughly, you can Submit Results. Please note: If anyone in your laboratory edits entries prior to the closing of submission for the survey, make sure that edited results are both Saved and Submitted. Saved results will NOT automatically submit once the survey submission closes. Icons You can use the icons on the top right to perform a variety of tasks. The first icon (flag) is the Report Issues icon. Selecting this icon will take you to a form which will allow you to select the type of issue or question from the drop-down menu.

16 Please include your question or request in the Comments section then Save. The i icon is for Instructions. Select the Instructions icon to view a copy of the instructions specific to your survey. Instructions will appear in a pop-up box. The Print Icon displays the recommended command for printing. Control + P is recommended for printing results since using Control + P allows the user to print at any point during the process - when entering results or when viewing a complete list of entries on the Review/Submit screen. Please note: If only a portion of the page displays on your printout, you may have to switch to a different browser to be able to view the entire page of results on your printout. The Revisions icon will allow the user to look at a history of revisions for your lab.

17 Clicking Compare allows the user to compare two different timeframes of results entered. On the Compare screen, you can see the previous entries on the left and the new entries on the right. To return to entering results, click Return to Enter Results. After returning to the Results Entry screen, if you wish to revisit the Revisions screen for further Revisions information, you can return by clicking the Revisions icon. On the Revision page, you can get a summary of the data entered on a certain date or time by selecting one of the date/time options under the Revision column. On the Revision page, you also have the option to change the entries back to the entries of a previous date/time. When you choose Revert - under Operations - the system will change the results back to the information which was entered on that date and time.

18 If you choose Revert, you will receive the following warning message before proceeding: Are you sure you want to revert to the revision from (date/time)? If you click the Revert button, the system will make the changes, and you will see a message and updates on the Revision page indicating the changes have been made.
