2 1. Open the Fitware app on the tablet and click Start
3 2. Ask the guest to select the I m interested in joining button
4 3. The guest will be prompted to enter their phone number and check the box if they would like to agree to the terms to receive text messages. Click the arrow on the top right of the screen to continue
5 4. The guest will be prompted to enter their address and check the box if they would like to agree to the terms to receive s. Click the arrow on the top right of the screen.
6 5. The guest will then enter their personal information (name, date of birth, gender) and click the arrow on the top right of the screen to continue.
7 6. The guest will have the opportunity to take a photo of themselves. By clicking Add Photo a front-facing camera will open, the user can take a picture to continue. If they do not wish to take a photo simply click Skip. *This photo will automatically be saved in Fitware ** The photo will not be saved to the ipad camera roll and should not be used for any other purpose than reference in Fitware
8 7. Next, the guest will start entering their home address/billing address. As member begins to enter address it will auto populate because your location services are turned on for the ipad * This should be their billing address

The guest will be prompted to select a membership type (single, Joint, Family) * They will be able to view the pricing for the program fee, access card fee and monthly dues ** You will be able to set these prices in Fitware (See Training Doc) Please select the type of membership you would like to sign up for: One adult 18 years or older. Both members must be an adult 18 years or older living at the same address and billed to one account. Family Adults (up to two) and minors ( minors must be between years of age) living at the same address and billed to one account.

The guest will be prompted to select a payment type Monthly 14-Day 7-Day 3 Month Prepaid 6 Month Prepaid 12 Month Prepaid * This screen will only appear if you enable plans for both prepaid and reoccurring, if you do not it will simply skip this screen and display the options you have enabled.
11 10. The user will be prompted to select the membership plan they d like to sign up with. Choose your membership plan * This data is pulled directly from Fitware on the plans you have enabled (If you do not enable 7 day and 14 day billing options it will only show monthly. 7 Days 14 Days $XX.XX Every 7 Days $XX.XX One-time Program Fee $XX.XX One-time Access Card Fee $XX.XX Club Enhancement Fee $XX.XX Every 14 Days $XX.XX One-time Program Fee $XX.XX One-time Access Card Fee $XX.XX Club Enhancement Fee Monthly $XX.XX Every Month $XX.XX One-time Program Fee $XX.XX One-time Access Card Fee $XX.XX Club Enhancement Fee
12 11. The user will be prompted to entering in their credit card number and expiration date or clicking the Scan Card button to scan their card. *This does not save the payment method information in the tablet, it will only save the payment method information in Fitware
13 12. Next, the user will review a breakdown of the all of the charges they will acquire at sign up. This will include: Prorated dues until their first monthly billing happens (if you do not offer anniversary billing, if you have anniversary as the enabled option for your tablet application it will bill the full amount at sign up) Program Fee Access Card Fee * Prepaid options will include the Club Enhancement Fee if you have enabled it in Fitware for these plans. Your Enrollment Information Membership Plan Single Monthly Payments $29.95 billed to your card ending in x Billed each month starting 1 Feb 2017 *Subject to a $XX Club Enhancement Fee. Today s Payment Dues at Sign Up $75.00 Program Fee $75.00 Access Card Fee $20.00 Tax $6.54 Total (USD) $ Join
14 13. The new member will then need to enter in a name and phone number for who they would like to have as their Emergency Contact
15 14. The new member will be prompted to sign the Membership Agreement Terms and Conditions on the tablet with their finger. * The user will need to scroll to the bottom of the agreement in order to sign.
16 15. The new member will be prompted to sign the Club Usage Terms and Conditions Agreement at the bottom of the screen, using their finger. * The user will need to scroll to the bottom of the agreement in order to sign.
17 16. The member will see a welcome screen and preview their name, phone number and address.
18 Fulfilling the membership in fitware 1. Login to Fitware à Customers à Order Fulfillment 2. Find the new member you are going to fulfill and click the Actions icon on the right hand side 3. Lastly, enter the barcode you are assigning the new member and click save
