1 Initiating a New Case Note How to Create a Case Note- Case Manager Steps that will be used in this How-To process. 1. Initiating a New Case Note 2. Entering Data on a Case Note 3. Submitting a Case Note Initiating a New Case Note 1) Log in to Therap 2) From the dashboard, check to see if your profile is set to Internal. Note: If the profile is set to Internal proceed to step 5. If not, proceed to step 3. 3) Select the Switch Profile link under the Choose Program option of the Dashboard. 4) In the Switch Profile page, click on the Internal Radio button then click Switch. 4.1)-Depending on provider preference, the profile name could be different. In this case, contact your Therap Security Administrator if you are unsure which profile is the correct one. How to Create a Case Note- Case Manager: Page 1 of 6

2 Initiating a New Case Note 5) Select the Individual tab on the left. 6) In the Care section, select New case note. See screenshot above 7) Select an individual from the Individual List by clicking on their last name in the far left column. Note: By default, the Individual List displays all individuals for whom you can complete a case note. You may narrow this list by choosing a letter in the bar at the top of the page. This will limit the results to individuals whose last name begins with the letter you choose. Alternatively, you may used the filter under the letter bar to narrow your results by last name and/or first name. Do not add a comma between the last and first name. For example, John Doe and Doe John will retrieve the same listing. Doe, John will yield no result. This field is not case-sensitive. 8) Enter the Service Date and Select a Template from the Select Date and Template section, then click the Submit button. Note: If this individual has an existing note (s), it will appear under the Existing Case Note(s) section. How to Create a Case Note- Case Manager: Page 2 of 6

3 Entering Data on a Case Note Entering Data on a Case Note 1) Input the details as necessary in the fields available on the Case Note Form. 1.1)-Service & Unit Rate ($) not to be used 1.2)- A language changed in Location: From: CDSS Setting To: Primary Residential Setting 1.3)- Non Reportable was removed from the Activity Type Note: As a reminder, do NOT change the Service Provider dropdown. 2) From the Questionnaire section, click Open. 3) Select a value from the Type of Contact drop down list then click Save. 4 4) Click the Close Popup button once a selection has been made. Entering Data on a Case Note 4.1)-Then click Ok from the Popup Message Window to navigate back to the case note form. 5) If you are adding a document, in the Attachment(s) section, click Add File to add a document. In the Add File box, click Browse and navigate to the document you would like to add. 6) After adding a file, enter a file description and click the Upload button. Once you click the Upload button, the description cannot be edited without removing the file and re-uploading. 7) To upload a scanned document, click Scan File if you are adding a file by scanning. 5 8) In the Scan File popup window, select the appropriate Scanner Options and click the Scan button. Entering Data on a Case Note 9) After previewing your scan in the Preview/Edit Scanned File(s) section, enter an appropriate description and click the Upload button. 10) In the Note section, enter your case note by clicking in the First Line under the B header or click on the P at the bottom left corner. 10.1)- As a reminder, special formatting of text is available while creating a note: Bold, Italics, and Bullets How to Create a Case Note- Case Manager: Page 5 of 6
