1 Provider Guide Welcome to PROVIDERfiles 2.0! This portal allows you to track all of your EAP referrals online, in a secure, encrypted environment. Only you and CONCERN have access to your client file information. Follow these steps to populate your practice information, enter case data, and submit visit claims. 5/31/2018 1

2 Enter your preferred for portal access. Re-enter for confirmation. Select a password. Re-enter for confirmation. Select Register. 5/31/2018 2

3 After initial registration, you will log on to PROVIDERfiles at: 5/31/2018 3

4 Menu Items Enter your provider details including specializations, office locations, and licensing information. Allows you to message us directly re: billing, referrals, and updating your details. Includes all of your referred, accepted, and open cases. FOR PROVIDERS WHO PROVIDE CIS AND TRAINING ON CUSTOMER SITES. FOR COUNSELORS WHO PROVIDE SERVICES ON CUSTOMER SITES ONLY. 6 Please enter your CAQH (if you have it), NPI, preferred correspondence method, mailing and billing addresses. 5/31/2018 6

7 Please update your weekend and evening availability, your associations, ages and groups (the distribution has changed), and languages, and specializations (new specializations have been added). 5/31/2018 7

8 Update your office locations. For Office Name, please use the following naming convention: City Last Name. 5/31/2018 8

9 Set up your office details here, including hours and contact information. 5/31/2018 9

10 Please add any special information regarding your office location, including ADA accessibility, parking, and public transportation access. 5/31/

11 Please add or update your professional license information here. You can overwrite the older license as we only need the current version. 5/31/

12 Select your professional license type from the drop-down list. You will upload a copy of the license under My Documents (pages 15-16). 5/31/

13 Please enter your professional liability information. You will upload a copy of the policy under My Documents (pages 15-16). Only the current version is required. 5/31/

14 Enter your policy information, including the policy carrier from the provided drop-down list. You will be able to upload a copy under My Documents. 5/31/

15 Please use this section to upload your renewed license, professional liability coverage, and W-9 (for new office locations). 5/31/

16 Select Browse to find the document to upload. 5/31/

17 My Messages 5/31/

18 We are pleased to re-introduce My Messages with our Provider Portal 2.0. You can both receive and send messages to various departments within CONCERN. 5/31/

19 You may message the Call Center regarding a referral, Provider Payments regarding a billing question, and Provider Relations regarding updating your credentials or if you plan to be unavailable. 5/31/

21 Here you can review Referral Opportunities Pending, Referred Cases (which you accepted), Open Cases, and a history of Closed Cases. 5/31/

22 By hovering over one of the blue Details buttons, you will see a pop-up with details of the case. 5/31/

23 Select View in the Referral Opportunities Pending section to see referral details. Please wait until first appointment is scheduled prior to accepting the case. 5/31/

24 Here you can review the Pending Opportunity and Accept or Decline the case. Then press Submit. Please wait until first appointment is scheduled prior to accepting the case. 6/1/

25 A case moves from Referred Case status to Open Case status once you submit the Case Opening form and an initial visit and that initial visit is accepted. 5/31/

26 Case Opening Form: There are several fields that are now mandatory. You will see them with green asterisks. They will not flag you until you are entering visits or trying to close the case. The intention is to give you enough time to fully assess before your Opening Form is completed. 5/31/

27 Click Modify to enter your visits. Be sure to click Submit to ensure CONCERN receives your submission for payment. This can either be done with each visit, or you can push Submit All Sessions for multiple visit entries. PLEASE NOTE: enter all visits prior to completing the Closing Form. 5/31/

28 For each session, enter Session Date, Session Type, and any notes if necessary (not usually needed). 5/31/

29 Case Closing Form: Our customers are asking for more aggregate outcome data. There are several fields that are now mandatory. You will see them with green asterisks. They will not flag you until you are trying to close the case. Please do not close the case until you have entered all visits. 6/1/

30 My Services My Services is COMING SOON. This section is for providers who provide services on site at an organization, like critical incident response and training. 5/31/

31 My Activities My Activities is for counselors who provide services at a customer s site only. 5/31/

32 My Activities is where you bill for ALL activities that are not directly connected to a client (i.e. training, orientation, no client). 5/31/

33 My Billing 5/31/

34 My Billing allows you to track your Pending Claims, and Processed Claims history. 5/31/
