The Important Forms & Documents page provides PDF copies of many of the forms available in the Main Office as well as information on how to submit certain requests online. Additionally, you can review important documents regarding policies at the University Village, including the License Agreement and Village Handbook. For residents who use social networking sites, you can follow us on Twitter or become a fan on Facebook. On these sites we post major announcements as well as upcoming programs and events. Sign up using the links above and you can receive Village announcements anywhere you can view these sites. For your convenience, the University Village uses the StarRez Portal system to provide 24 hour online services to University Village residents. There, you can enter your Bronco ID Number and the PIN Number that was issued to you when you registered online. Using your Portal account you can do the following: Make payments via credit card to your Resident Account. Submit maintenance workorder requests for your apartment Submit your application to relicense for upcoming terms Update your addresses and emergency contact information If you have forgotten your PIN number, please enter your Bronco ID Number and click Forgot Pin Number. Your PIN will be ed to the address on file. Please note that some actions, such as submitting maintenance workorders and requesting late fee waivers, can only be done online. Please browse our website as well as login to your Portal Account and familiarize yourself with these features.

4 University Village Address Information In order to ensure prompt delivery of your mail and packages please address all mail as follows: Include your full name as it appears on your Village License Agreement. Include your mailbox number. (e.g. Box #1337, not P.O. Box) If you have trouble entering the proper format, particularly online, try one of the alternate formats below. Please note that VIS is the postal abbreviation for Vista. Please do NOT: Have any mail delivered to your box under any other name. It will not be delivered. Address your mail using your building and apartment number. Address your mail to 3400 Poly Vista Office or Building 300 Use as your Zip Code. This is for P.O. Boxes only and will delay your mail. Here are some examples of valid address formats: John Doe John Doe John Doe 3400 Poly Vista #1337 Box # POLY VIS #1337 Pomona, CA Poly Vista POMONA, CA Pomona, CA Please be sure to check your mailbox regularly. If your mailbox becomes too full to deliver additional mail, the contents of your mailbox will be brought to your apartment by our office staff and you may be subject to a $25 delivery fee. Additionally, packages not picked up within two weeks may be returned to sender. Finally, if you decide to move away from the Village, be sure to submit a change of address with the United States Postal Service as well as the companies and organizations that send you mail such as the DMV, banks, schools, etc. This will ensure you continue to receive your mail even after you have moved away.

5 University Village Computer Policies All bedrooms at the University Village are equipped with one wired network jack per resident. Additionally, Phase III apartments are equipped with a network jack in the living room. These jacks provide access to the Cal Poly Pomona campus-wide network. For your security, all network traffic is monitored and the following policies apply: I. Only one device may be connected to a network port at any one time. The use of hubs, switches, or routers to provide access to multiple devices is prohibited. II. Wireless routers or any other device used to provide wireless access to the campus network are prohibited. III. Using the network to send or receive music, movies, TV, or any other copyrighted material without express permission of the copyright holder is prohibited. This includes, but is not limited to, the use of programs that use protocols such as Bittorrent, Direct Connect, Ares, and Gnutella. IV. Running commercial operations, servers, or experimental network software is prohibited. Unusual activity may cause your network access to be disabled to protect the network. Violations of any of the above policies will result in the deactivation of your network access. Depending on the severity of the offense, your access may be restored after correcting the violation. Multiple or severe violations may result in permanent deactivation of network access as well as further disciplinary action. The entire I&IT Acceptable Use Policy can be found online here. Additionally, your network access may be disabled if your computer becomes infected with malicious software such as a virus or Trojan horse. Please make sure you have a virus scanner installed and keep it up-to-date at all times. Please see the Safe Computing page on the University Village website for more information. If you have trouble setting up your computer to work with the campus network, please call the I&IT Helpdesk at If there are any problems with your network connection, please submit an IT Workorder on the University Village Website. Make sure to include the MAC address of your computer as well as the jack number on the port you are attempting to use.

6 University Village Communication Policy The University Village may send communications to prospective and current residents either by mail or by . is the primary way in which the University Village communicates with students on virtually every topic, including general information, licensing, fees, deadlines, and appeals. All students receive an account and address from the University at the point of admission. Student addresses at Cal Poly Pomona include an identifier for the student and end All Village communications will be sent to the resident s University account. It is the student's responsibility to check his/her University account daily in order to stay current with University Village communications. Students may chose to have their University forwarded to another account by entering the forwarding address in the Identity Services Control Panel (information on changing forwarding can be found on the e-help website). However, if the student chooses to forward their University address, we will not be responsible for the handling of by outside servers. Given the wide range of computers and programs used by students, every attempt will be made to send messages in a format that can be easily read by all students. Students are responsible for maintaining an accurate mailing address with the University Village through the University Village Online Portal system and for accessing/forwarding their Cal Poly Pomona account. The University Village intends to replace many of the letters sent to students via the U.S. Postal Service with communications. As a result, the University Village recommends that students check their accounts daily. The consequences of not checking are the same as those for not checking your U.S. Postal mailbox. Some of these consequences include missing payment and licensing deadlines. Students are responsible for the consequences of not reading, in a timely fashion, Village-related communications sent to their account. Students have the responsibility to recognize that certain communications may be time-critical. Excuses such as not checking on time, errors in forwarding , returned due to undeliverable address, undeliverable because the mailbox is full, etc. are not acceptable reasons for missing University Village correspondence.
