6 YOU MUST CONFIRM YOUR ADDRESS FOR EACH LICENSE. Click NEXT at the bottom to go on. NOTE: Once you begin the process, you can always CLOSE AND SAVE at the bottom of any screen if you need to leave it and come back later.

7 Declare Active or Inactive Status. Click NEXT to go on.

8 Enter your DEA number AND upload a PDF copy of your DEA certificate. Click NEXT to go on.

9 Declare which Drug Schedules your DEA certificate includes. Click NEXT to go on.

10 Enter information on any action taken against your DEA license. Click NEXT to go on.

11 Affirm your information and confirm your electronic signature. Click NEXT to go on.

12 REVIEW your information. It s always a good idea to Print Review so you will know what you ve entered. Click ADD TO INVOICE at the bottom.

13 This is the first Invoice for the first license you are renewing. DO NOT CLICK PAY YET! Instead, click Create/Resume a Renewal to renew your next license. You can pay for all of them at once. You will come back to this page to select your next license to START the next renewal process.

14 Declare your Military Status. Click NEXT to go on.

15 Enter your personal information to verify your identity. Click NEXT to go on.

16 Declare any Specialty Options. Click NEXT to go on.

17 List all the hygienists you employ. If you have an ADHP student with a permit, they are considered Non-licensed Staff. If you are an Associate or employed by a corporation, and/or are not responsible for the employ of hygienists, skip this step. If you are unsure, please contact the AL Board of Dental Examiners. Click NEXT to go on.

18 List all the non-licensed employees in your office, such as dental assistants, receptionists, office managers, ADHP candidates with permit, etc. If you are an Associate or employed by a corporation, and/or are not responsible for the employ of non-licensed auxiliary or office personnel, skip this step. If you are unsure, please contact the AL Board of Dental Examiners. Click NEXT to go on.

19 Respond to questions about yourself and your dental license. Click NEXT to go on.

21 Certify your CE requirements, your CPR and Infectious Disease courses, and VOTE! Click NEXT to go on.

22 You will see another REVIEW page and a prompt as before to add the Dental License payment to your Invoice. Again, click on Create/Resume a Renewal on the left side of the page to go to your next license. It should be your Oral Conscious Sedation license.
