1 Wellness Handbook & Website Details WEB PORTAL REGISTRATION PROCESS Go to All City of Burleson employees & spouses are New Users and must create an account for the site. Step 1: Click on Get Started, then Enter Access Code Employees & Spouse will enter the respective codes listed below (all CAPS required) BURLESONEMP BURLESONSPOUSE Step 2: Choose Your Preferred Language

2 Step 3: Create your Account Enter your name, employee ID (for employee ID, employees AND spouses will use your first and last name, no spaces, all caps (i.e. JOHNDOE), date of birth, gender, choice of username and password, , select Employee s Department, choice of security questions AND check the Participant Authorization box. Click I AGREE and proceed! Employee ID (for both employees and spouses) = FIRSTNAMELASTNAME (i.e. JOHNDOE) Step 4: Complete your account by clicking Continue. 2

3 Confirm your name, date of birth, , phone (optional), username and primary contact . Click Next to proceed. Step 5: OPTIONAL - You may create a personal mission statement for your goals and values & upload a photo of yourself. Click Next to Proceed. Step 6: Save your Account details & Your Registration is now complete! 3

4 Step 7: Go to Home Page to access full website! WEBSITE TIPS To update any account settings, click on the Head Icon in top right corner of page and View My Account To confirm completion of the Wellness Assessment or Online Coaching programs, click the Trophy Icon in top right corner of page and go to My Achievements 4

5 To access the website after initial registration, go to and go to Sign In in the top right corner. Enter your created User Name & Password If Forgot User Name or Password, click on link below Sign In button to enter your account details to obtain / reset. 5

6 How to Register for Online Challenges To access the website after initial registration, go to and go to Sign In in the top right corner. Step 1: Log In to your account. Step 2: Click on Challenges and you will be taken to an External Links re-direct page where you will click Continue 6

7 Step 3: Optional upload picture, add social media info, etc. Hit Save and Continue to skip. Step 4: Optional fill out basic info, diet & exercise preferences. Hit Finish to proceed. 7

8 Step 5: You are now taken into the Challenges website! Step 6: Scroll down and click on Join Challenge You are now enrolled in a Challenge! 8

9 How to Record Activity for a Challenge For Today: Step 1: Log into your account and click on Challenges Step 2: Click on Record Today s Activity Step 3: Click on Today s Date and then click on the To Do icon Step 4: Click on I did this! Step 5: You will then see this screen! 9

11 Website Programs and Details The Wellness Assessment FREE BRiCk membership if completed before 1/31/18 Click on The Wellness Assessment button & then Get Started. There are many aspects to health. That s why the Wellness Assessment is made up of four categories: Bio, Body, Mind, and Lifestyle. Each category has two levels, or brief groups of questions. Level 1 allows us to understand the basics. Then Level 2 gets into the details. Tap on the answers that best fit you. Then review your answers and results. You ll get personalized tips for a healthier lifestyle, including coaching program and items to add to your To Do List. TIP Simply take your Wellness Assessment certificate with you when you register at the BRiCk in order to get your FREE membership! Online Wellness Coaching & Online Tobacco Cessation Program Click on the Go to Coaching button & then choose your focus area. Click on Details for more information about the program & then click Get Started to begin. TIP Chose any program to begin. The program is self-paced, so you can start, pause, resume as needed! 11

Virtually visit continents to learn interesting destination facts and challenge for your fitness and nutrition goals along the way! 13 Additional Wellness Activities & Details Wellness Coaching provided by Baylor Scott & White Health Wellness Team Weekly sessions offered to all interested in making their health a priority. If you are ready to make a difference in your health, this is where to begin! Four week sessions will be offered in May and August of corporatewellness@bswhealth.org for more information! Onsite Wellness Events & Webinars provided by Baylor Scott & White Health Wellness Team Weekly onsite events during the challenges & webinars offered to all interested in having fun and focusing on their health to Be Well! Sessions will be held at various locations throughout the year. Attend a session and be entered into a raffle for a prize! 13

14 Wellness Form Employees who maintain his/her annual physical along with blood work, will NOT pay an additional $37.08 per month towards the cost of their employee health insurance premium from January 1, 2018 to December 31, If you have completed your annual physical along with blood work, please have your doctor complete the Wellness Form and send it to Baylor Scott & White Health. Your discount will begin the 1st pay period of the following month. In order to receive a confirmation from BSWH, you must send your Wellness Form via , and include your address on your form. Naturally Slim Are you ready to kick start the best weigh of your life? Be Well, your employee health and wellness program, is extending a special one-time offer to enroll in the NEW & IMPROVED Naturally Slim weight loss program! ONLY 30 employees will be selected to receive the program at $ (to be payroll deducted at $38.50 over 5 pay periods). The full price of the program is valued at $500. What Will It Take? 15 How It Works Foundations Course You will receive access to the Naturally Slim website to view a weekly video teaching you the program concepts. There are 10 self-paced, online video sessions, where you will view one video each week over a 10-week period with additional videos & support for an entire year! You will receive a program kit mailed directly to your home that will provide items to be used during the program. You will receive correspondence and guidance from a team of Naturally Slim counselors throughout the program. NS4YOU & NS4LIFE Once you ve completed Foundations, you have access to additional portions of Naturally Slim to help reinforce those skills and ensure you know how to maintain your weight loss long-term. NS4You : Every two weeks you will have a new class that provides a refresher about how to apply the Naturally Slim principles to your everyday life. You will receive correspondence and guidance from a team of Naturally Slim counselors throughout the program. NS4Life : You will receive new video lessons every month, along with continued access to your dashboard with tools to track your progress, the Minute with Marcia videos, NSTown and more. City of Burleson will have no access to the personal information that you include in the online application.
