The self-guided Wellness Workshops help you make healthy behavioral changes in several different areas. Additional online health tools help you track and analyze your fitness, nutrition and wellness data to better manage your health. 2 Registration Process: Go to HealthAdvocate.com/members 2 Type in Albuquerque Public Schools and click Submit 3 2

9 My Points Wellness Incentive Earn 50 points each by completing the Biometric Screening and Tobacco Affidavit. Earn an additional 250 points throughout the program to be entered into prize drawings! Track your progress on the My Points section. 16 Complete the Biometric Screening Click on Schedule to schedule your screening. Complete the scheduler prompts to make an appointment. 17 9

Select the option that is most appropriate for your situation, check the confirmation box, and then click Submit. 18 Taking your PHP: Optional Not Required Select Start the Personal Health Profile from your My To-Dos or click Health Profile at the top of the page 19 10

11 Taking your PHP: Optional Not Required Choose your Gender and enter your Date of Birth, then select Start a New PHP 20 Viewing your PHP Report: At the end of the survey, after you have completed all sections, click on Finish and Generate Report. In a few moments your personal report will generate 21 11

12 Viewing your PHP Report: You can choose to view more detailed information by clicking on Learn More 22 Saving or Printing Your Report: You can choose to save or print your report by clicking PDF to the right our your Personal Health Profile score

13 Signing up for Coaching: Go to the wellness home page and select Join Coaching at the bottom of the home page. A wellness coach will be assigned and you will be contacted by a coach. To begin coaching immediately contact Health Advocate. 24 Starting a Wellness Workshop: After you have completed the Personal Health Profile, a workshop will be added to your My To Dos. You can begin the workshop by selecting Start or to choose a different workshop, select Workshops from the top menu bar

14 Using the Health Trackers To add a tracker or track your wellness activity you can click on the Health Trackers from your wellness homepage or click on the Health Tracker tab. 26 Additional Wellness Resources Check out the Wellness Resources from your wellness homepage. Access the monthly newsletters, discounted gym members, and healthy tip sheets
