A biometric screening can help detect abnormal health values that may indicate significant risk of or presence of chronic conditions such as hypertension, heart disease and diabetes. The screening will include a full blood lipid profile, which measures your cholesterol values and blood glucose. No eating or drinking anything other than water for 8-12 hours prior to the blood test is required. What are my options for completing a biometric screening? There are four approved methods for completing your biometric screening. Only biometric results obtained through one of the approved screening methods will be accepted for completion of the requirements to earn myhealth myrewards participation rewards. Health Provider Screening (personalized form download/ completion/submission required) UnitedHealthcare on-site screening (if available) by appointment LabCorp visit (personalized form download/completion/ submission required) At-home kit (allow adequate time for ordering/completion/ submission/testing) How do I sign up? To obtain needed forms, register for an event, order a kit or obtain more information: Register/log into myuhc.com. Click on Your step-by-step instructions The following outlines the sign-up process for each approved method: Health Provider Screening, UnitedHealthcare on-site screening, LabCorp visit, and At-home kit. Health Provider Screening Register/log into Click on Biometrics Screening. Confirm your personal information and click Continue to be taken to the Biometric Screening Registration Site. To learn more about completing a biometric screenings via a Health Provider Screening Form, click on the Learn More link. Click on the Register button under Health Provider Screening Form to begin the registration process. Select the appropriate member status for you and click the Continue button. Click on the Download Selected Form button. Follow the instructions on the screen to download the form. Take the Health Provider Screening Form with you to your health provider visit. Then either ask the them to complete it AND fax it to the number provided on the form, OR you can upload the completed form electronically to the Biometric Screening Registration Site. To access the site, log into and click on

2 UnitedHealthcare on-site screening To learn more about on-site biometric screenings, click on the Learn More link. Click on the Register button under On-site Program to begin the registration process. Select a location from the list of available locations that have on-site events. Please note: most sites only have one date for onsite screenings. Select the date and time frame that works best for you. Select from the available time slots. Confirm the date and time. You will then receive a registration summary confirming your registration.

3 LabCorp visit To learn more about completing a biometric screenings via a Lab Program, click on the Learn More link. Click on the Register button under Lab Program to begin the registration process. Enter the city/state or ZIP code where you would like to find a participating lab and click on the Find Lab button. Select a lab that is convenient for you by clicking on the radio button next to the most convenient location, and click the Continue button. If you do not find a location that is convenient, you can either change the city/state or ZIP code and look for another location or click the Back button to select another form of biometric screening. Select the appropriate member status for you and click the Continue button. Read the Consent to Release data and the Consent to Perform the test and agree by clicking the appropriate radio button and entering your full name in the appropriate boxes, then click the Confirm Registration button. You will then receive a registration summary confirming your registration. Print the registration confirmation for your records. Be sure to download and print the Lab Requisition and take with you to your selected lab. To download it, Click the Download the Lab Requisition button and follow the instructions on your screen. The process will vary based on the browser you are using. Please be sure you go to the selected lab. Not all Labcorp labs are included as part of this program.

4 At-home kit The cost of the kit is charged to the P&G Health Plan at the time it is ordered. Not at the time you submit it. Kits are personalized to the individual signed up for the screening and can only be used by the individual for whom the kit is ordered. to be taken to the Biometric Screening Registration Site. Select the appropriate member status for you and click the Continue button. Enter self-reported information (height, weight and blood pressure) and click the Continue button. Please note: If you don t know your blood pressure information, many retail pharmacy locations and large retain chains have pharmacy services with blood pressure machines that you can use at no charge. Confirm the information you just entered and press the Continue or Back button as appropriate. If shipping address is the same as your home address, click on the Yes radio button, otherwise enter a shipping address and press the Continue button. Read the Consent to Release data and the Consent to Perform the test and agree by clicking the appropriate radio button, entering your full name in the appropriate boxes, and clicking on the Confirm Registration button. At-home test kit must be ordered, received and completed with your blood sample submitted before the end of the calendar year. Allow sufficient time by ordering and completing early in the year.

5 What if I need to change the mode of my biometric screening? To change the mode of your biometric screening, you ll first need to cancel your current mode. Please note that you cannot cancel at-home kit registrations. You will need to call Please note that on-site screening registrations cannot be cancelled after 12:00 p.m. CST, the day before the event. You will need to call to do that.
