Enter the requested information for all fields and then click the Save & Continue button. Make sure to select which time of day you prefer to be contacted. This is where you consent to transfer your results and/or fact of participation of the voluntary wellness screening to BCBSMA. 5 Step 8: The next screen will look similar to the one below. Click the Download button. Step 9: Once the form is downloaded on your computer, it will look similar to the one below. Print the form.

6 Step 10: Once the form is printed, take it to your provider (physician, physician s assistant, or nurse practitioner) and have him/her complete the form. Make sure to bring the form home with you after it is completed by your provider. It is YOUR responsibility to return the form. Step 11: Submit the completed form by mail, fax, or to the address shown on the bottom of form. Or, you can upload a scanned copy of the completed form in the on-line registration portal. To upload the completed form, follow the instructions on the next page. HOW TO UPLOAD YOUR COMPLETED FORM IN THE PORTAL Step 12: Scan your completed form and save it to your computer files.
