2 EnviroChain Sample Generator Guide Getting Started Getting Started Login When you are ready to log in, click the Login link in the upper right corner after activating your account or navigate straight to: Log in using the address and password you chose during registration. On your first login, you will be presented with the screen shown below: Follow Step 1 and enter all required contact details. Be sure to click the Save User Details button when you are finished. Follow Step 2 and select your favorite lab(s). Once your account is created, you can update your user details, privacy settings, default signature, and password from My Account page. You can also view existing CoCs, choose favorite labs, and add or delete clients and projects from the My Data page

3 Create a New CoC Form Create a New CoC Form Create New Chain of Custody From the EnviroChain dashboard, click on the Create a New CoC button and follow the steps outlined in this guide. Copy Existing Chain of Custody From the EnviroChain dashboard, click on the radio button in the Pick column next to the chain of custody you wish to copy. After selecting the radio button, click on the Copy an Existing CoC button

4 Select a Laboratory Select a Laboratory By default, you will be taken to the Lab section of the chain of custody process first. In this section, you will need to pick a laboratory and a contact at the lab. You can click the blue Save button at the bottom of the screen at any time to save your changes. To save and move on to the next step, click Save & Next. Pick a Lab From the Lab tab, click on the blue Pick a Laboratory button. From the Pick a Lab popup, you can filter your results by Favorite Labs or Commercial Labs. You can also search the lab listing by keyword. When you have located the lab you wish to use, click on the radio button in the Select column, and then click on the blue Save button

5 Select a Laboratory Pi ck an Existing Lab Contact To select a lab contact, slick on the blue Pick a Contact (or Create a New One) button. In the Pick Contact Details of Lab popup, select an existing contact using the radio buttons. Click Save

6 Select a Laboratory Create a New Lab Contact In the Pick Contact Details of Lab popup, click the blue + Add a Contact button. In the fields provided, enter the contact s name, phone number, and address. When you are finished, click Save

7 Select Project/Client Select Project/Client In this section, you will select or create a project and client for your chain of custody. You can click the blue Save button at the bottom of the screen at any time to save your changes. To save and move on to the next step, click Save & Next. If you would like to revisit a previous section, click the Previous button, but make sure you have saved your changes first. Get started by clicking the Pick a Project (or Create a New One) button. Pick Existing Project If your desired Project/Client is already listed, select the applicable radio button. You can search for projects by client, project name, city, or state. Once you have selected a project, click the blue Save button. Create New Project (Existing Client) If you have a new project for an existing client, click on the blue + Add a Project button above the list of projects in the Pick a Project window

8 Select Project/Client Fill in all of the required fields, and then click Save. Create New Project (New Client) If neither your project nor your client is already in the database, begin by clicking on the + Add a Client button. Fill in all of the required client details, and then click Save

9 Select Project/Client You will be returned to the Pick a Project popup, where the new client will be at the bottom of your list. Select the radio button next to the new client and click + Add a Project. Follow the directions for adding a new project with an existing client above. When you have selected the proper client/project, click Save

10 Enter Sample Information Enter Sample Information In this section, you will enter the sample information for this chain of custody. You can click the blue Save button at the bottom of the screen at any time to save your changes. To save and move on to the next step, click Save & Next. If you would like to revisit a previous section, click the Previous button, but make sure you have saved your changes first. Basic vs. Advanced View Two views are offered in this section Basic and Advanced. Basic input allows for all required information, plus time, number of containers, and turnaround time. Advanced input has addition fields for depth, sample type, and additional container details. You will need to use the horizontal scrollbar to view all of the options. Add a Sample To add a new sample, click on +Add a Sample. Enter all required information (marked by a red asterisk), such as Sample ID, Date, Matrix, etc. To select a Method, simply begin typing the method in the text field, and a list will be populated. You can also click on the Search icon next to the text field:

11 Enter Sample Information As you are adding samples, you have the option to Copy Selected, Edit Selected, or Delete Selected. Simply select the sample(s) you wish to modify by using the checkboxes, and then click one of the buttons above the sample list. Once you have finished entering your samples, you can Print Labels for sample containers or proceed to the next step

12 Enter Shipping Details Enter Shipping Details This is where you enter cooler information and select a shipping carrier or lab courier. Coolers Fill out the number of coolers and total weight for the shipment. Shipment Method Select your shipment method (FedEx, UPS, Courier or Dropoff). If the lab has entered their shipping account number, you can select to use the lab s shipping account. If you want to use your own shipping account, select Use Other Account #, and then enter your shipping account number in the field to the right. If using FedEx or UPS, select the shipping method/priority. If you have a tracking number, you may enter it in the optional Package Tracking/Airbill #. If you would like to print shipping labels for your shipment, click on the Print Shipping Labels button. You can also Find Dropoff Locations. When you are finished, select Save or Save & Next

13 Add Signatures Add Signatures Add Sampler Signature Make sure the correct name is in the Sampler Name field (will default to the user s name). Next to the Sampler Signature field, click on the Click Here to Sign link. You can use one of the recommended signature fonts or select your own. When you are finished, click Save. Add Relinquished/Received Signatures Click on the blue + Add Signatures button. Enter the name and company of the person relinquishing the samples in the first column. Click the checkbox to agree to the terms and conditions, and then click on the Click Here to Sign link to add a digital signature. Follow the same steps in the second column for the person/company receiving the samples. When you are finished adding signatures, click Save or Save & Next

14 Submit Your CoC Submit Your CoC Under Billing Information, select whether the charges will be billed to you or your client. Under Summary, review to verify all of the information is correct. Under Submittal Preparation, you can select EDD format (if the lab has a preferred format, you will not be able to change this). You can also choose to send a copy of the completed CoC to an additional address. When you are finished with the CoC, you can print or preview with the Print/Preview Your CoC. Submit electronically to the lab with the Submit button

15 Pre-Logging Samples Pre-Logging Samples To pre-log samples, go to Tools Pre-log Samples & Print Labels. Click on the blue +Add (Pre-Log) Samples button. As with adding samples to a CoC, you can choose between Basic and Advanced views. Enter sample information, and click Save when done. To share pre-logged samples with other users, scroll all the way to the right and click the Share link in the row with the sample you would like to share. Click on the blue +Add User button. Begin entering the person s address, and if he/she is in the system, the rest of the address will pop up. Select the address, and the person s information will pre-fill. Click on Save, and the sample will be shared with that user
