1 How to Complete New Hire Benefit Enrollment You have 30 days from your date of hire to complete your Benefit Enrollment through SAP Employee Self-Service (ESS). You have 30 days from your date of hire to elect benefits through SAP Employee Self Service application or you w ill need to wait until the next open enrollment. Once hired your User Name and password for SAP ESS will be sent to you via . Make sure you check your Spa m or Junk Mail box for this information. 2 Go to: Step by Step Enrollment Instructions How to Enroll in Medical, Dental, Vision and Life Insurance Through SAP ESS STEP 1 (If you do not remember your login information, Click Get Support or the HelpDesk) Enter your User Name and Password (Sent via , please check your Spam or Junk Mail folders) STEP 2 If you need to add dependents to your plan, you MUST add them NOW. Click - Personal Information Click Family Member/Dependents STEP 3 Click on the type of Family Member / Dependent you want to add. (i.e. New Spouse / New Child) Enter Family Member/Dependent Information. Click Review. Then click Save. *NOTE: if your spouse is eligible for healthcare thought his/her employer, he/she is unable to go on the Bell medical plan** *You MUST enter your Family Members/Dependents before selecting benefits, if applicable. 2 P a g e

3 You have added your dependent Once you SAVE you will get the Green Check, if everything is correct. If not, an error message will tell you what is missing. Repeat Step 2 thru 3 for each Dependent and/or Beneficiary STEP 4 When all dependents have been added then Click on the Benefits and Payment tab to elect benefits. STEP 5 New Hire Click on New Hire. Step 6 The first screen will bring you to the affidavits page (the answers will default), please be sure to double check and answer both questions HONESTLY. **NOTE: if you do not have a spouse listed with your dependents, you will only see the Tobacco Affidavit. Affidavit 1: Affidavit of Tobacco Use/Tobacco Non-Use Affidavit 2: Spouse Exclusion (if applicable) 3 P a g e

4 STEP 7 ***IMPORTANT*** Scroll to the bottom of the Affidavits, click Edit. Then scroll back up and ANSWER BOTH questions (you may only have the Tobacco one depending on if you have a spouse listed) Once questions have been answered, scroll to the bottom and click Save. STEP 8 Verify that your Personal Profile is correct. If anything is incorrect, please correct in ESS under Personal Information. When everything is verified, click Next STEP 9 Verify that your Dependents and Beneficiaries are listed. If they are all not listed, you need to refer to Step 2 and 3. When everything is verified, click Next STEP 10 Health Plans this is where you will select which health plan you want. You click the Paper to the left to add your Medical, Dental, and Vision Plans. 4 P a g e

5 Select your plan and enroll dependents (check the box in front of the dependents), if applicable. Once you have made a selection, click Add Make sure to make an election for all plans, even if you are only waiving coverage. When all coverage is elected and/or waived, click Next Insurance Plans STEP 11 You can elect Voluntary Life insurance for yourself, spouse and/or child(ren). You must have Vol Life (EE) to elect coverage for your spouse and/or child(ren). For Vol Life (EE), you must select the number of units. Each Unit is 50,000 1=$50,000; 2=$100,000; 3=$150,000; 4=$200,000 Your election can not exceed 5 times your salary. When all coverage is elected and/or waived, click Next Review and Save STEP 14 Please make sure your elections are correct BEFORE you save. If anything is incorrect, you can either click Previous or the line bar options. If everything looks okay, click Save 5 P a g e

6 Go to: STEP 1 (If you do not remember your login information, Click Get Support or the HelpDesk) How to Enroll in 401(k) through ESS Enter your User Name and Password (Sent via , please check your Spam or Junk Mail folders) STEP 2 To add or change your contribution, click on Benefits and Payments link. Click on 401k Plan STEP 3 Click the button for 401k Plan or 401k Catch-up and then click Add Plan. 401k Plan = everyone Eligible will have this button. 401K Catchup = employees over the age of 50 are able to contribute an additional amount each year. Note: if you have a % in both 401k and 401k catchup both will be deducted from your pay until you reach the IRS contribution limit for the year. STEP 4 Select 401k Saving Plans or Waived Coverage-401K Plan. Then Enter the % or $ Amount you want deduct from your pay. Once the % or $ has been entered, click Select Beneficiaries. 6 P a g e

7 STEP 6 Enter the % in Beneficiary Percentage (Primary) for the main person(s) that would be responsible for the money. Contingent Percentage would be the person(s) that would be responsible for the money if the Primary is not living at that time of the employee s death. Click Add Plan to Selection If you need to add a Beneficiary you will need to Finish your current action and then add the missing beneficiary. To add the missing beneficiary: Click on Personal Information tab. Click on Family Member/Dependents Refer to step 3 on the first page. STEP 7 IMPORTANT! Once your enrollment and beneficiaries have been completed, You MUST click Review Enrollment Click on Save. STEP 8 Once you have SAVED the record, you get a message letting you know your selections have been saved. Congratulations! 7 P a g e

Then click Participation Overview Step 3 It will bring you the Benefits Participation screen. Make sure you change the date to the date your insurance is effective. Then Click Go Step 4 Scroll to the bottom and click Print Confirmation Form: All Plans Step 5 Your personalized statement will appear on the screen. Step 6 To print your confirmation, right click on your mouse and select Print Or hit Ctrl + P 8 P a g e
