3 Case Creation Steps The following are the steps to create cases: Step Page 1. Add a new Employee Record 2 Employee profiles must be created first to add cases for program participants. 2. Add a Participant 4 The program participant can be the employee or another family member (see pages 5 & 6). 3. Add the Case Detail 8 The Case Detail is all information related to the participant case. 4. Add the Communication 10 Communication is any conversation or correspondence in regards to the participant s case. 5. Add the Referral 12 Referral(s) submitted by provider(s) 6. Close the Case 14 Once a case is completed close the case. 7. Print Reports a. Plant Report. 25 b. Activity Report 26 bos/opeiu459aflcio 1
4 Add a new Employee Record To add a case the employee must be created first. Click the Create New Case link. bos/opeiu459aflcio 2
5 1.) Enter Employee Name and Address. Required Fields are bolded. 12.) Click the Save Current Record button. 2.) Enter Employee Demographic Information. Required Fields are bolded. 3.) Enter Employee Contact Information (Optional). 4.) Select the Employee Status (Required). 5.) Enter the Employee Classification (Optional). 6.) Enter the Employee Department (Optional). 7.) Enter the Employee ID or GMIN (Required). 8.) Enter the Employee Plant Seniority Date (Optional). 9.) Enter the Shift (Required). 10.) Select the Insurance type (Optional). 11.) 6 Adding A Participant 1.) To add a participant (employee, spouse, etc.) click the Participant link. bos/opeiu459aflcio 4
7 Employee Participant 1.) The Share Level is Not Shared by default. The Participant is the employee by default. Share Levels include: a.) Not Shared Do not share this case with other representatives. b.) Shared Share this case with other representatives. c.) Never Share Never Share this case with other representatives. If this is used the case will never be shared. To change the Share Level for an employee select another level and click the Save Current Record button. bos/opeiu459aflcio 5
8 Family Member Participant 1.) If the participant is a family member click the No button. 2.) Click the OK button in the popup window. bos/opeiu459aflcio 6
9 1.) The Share Level is Not Shared by default. Share Levels include: a.) Not Shared Do not share this case with other representatives. b.) Shared Share this case with other representatives. c.) Never Share Never Share this case with other representatives. If this is used the case will never be shared. 2.) Add Family Member Name and Address. Required Fields are bolded. 3.) 10 Case Detail 1.) To add/update the case details click the Case Details link. bos/opeiu459aflcio 8
11 2.) The date the case was opened defaults to the current date and can be changed if desired. 3.) Enter the Projected Closing date of the case (Optional) 4.) The current Date Closed status is displayed. 5.) Select the Case Status. 7.) Select a Referral Source. 6.) If the CDR referred within 24 hr. click Yes. Otherwise, click No and enter explanation. 8.) Click the checkboxes of the Work Performance Problems, Presenting Problems, and Program Recommendations that apply. 9.) Select an Acceptance option. 10.) 12 Communication 1.) To add/update communication information click the Communication button. bos/opeiu459aflcio 10
13 2.) The Contact Date defaults to the current date and can be changed if desired. 3.) The duration of the communication conversation or correspondence defaults to 20 minutes and can be changed if desired. 4.) Enter notes regarding the communication. 5.) 14 Referrals 1.) To add/update referral information click the Referrals button. 2.) Select a Provider or Enter a New Provider by clicking on the link see page 21 on adding a provider. 3.) Enter notes regarding the Referral. 4.) Enter the Referral Date 5.) Click the Save Current Record Button. bos/opeiu459aflcio 12
15 To Delete a Referral click on the Delete Referral link or to Edit a Referral click on the Edit Referral link. bos/opeiu459aflcio 13
16 Closing 1.) To add/update closing information click the Closing button. 2.) Enter the Closing Date. 3.) Select a Closing Reason 4.) Enter a Closing Summary 5.) Click the Save Current Record Button. bos/opeiu459aflcio 14
17 Adding New Cases for Employees 1.) Click on the Case List link. Yes indicates Employee Participant 2.) Click the New Case button bos/opeiu459aflcio 15
18 3.) Add/Update the case related information in these areas. See pages bos/opeiu459aflcio 16
19 Edit or Remove Cases 1.) To edit a case click the View button. To remove a case click the Remove case button. bos/opeiu459aflcio 17
20 Edit a Case/Update an Employee 2.) Add/Update the case related information in these areas. See pages ) And/Or make your changes to the employee profile and click the Save Current Record Button. bos/opeiu459aflcio 18
21 Sorting Cases 1.) Click on the Case List link. 2.) Click on the Column Heading. The ascending/descending indicator appears and the Case List is sorted in this order. Sorting can be performed on the LastName, FirstName, Opened, and Closed columns. bos/opeiu459aflcio 19
22 3.) Clicking on the Column heading once more changes the direction of the ascending/descending indicator and Case List sort order. bos/opeiu459aflcio 20
23 Adding A Provider 1.) Click on the Provider List link. 2.) To add a new provider click the Create a New Provider link. bos/opeiu459aflcio 21
24 1.) Enter Provider Information. Required Fields are bolded. 2.) Enter Location Information (Optional). 3.) Enter Contact Information (Optional). 4.) Click the Save Current Record button. bos/opeiu459aflcio 22
25 Update/Remove A Provider 1.) Click on the Provider List link. 2.) Select a State and click the Go button or click the View/Edit link for the desired Provider. bos/opeiu459aflcio 23
26 Update the provider profile and click the Save Current Record button or click on the Remove Provider link to remove the provider. bos/opeiu459aflcio 24
27 Plant Reports 1.) Click on the Plant Reports link. 2.) Enter a Date Range 3.) Select a Report 4.) Click the Preview Report button to see the results on screen or click the Download as MS Excel button to load the report into a spreadsheet. bos/opeiu459aflcio 25
28 Activity Reports 1.) Click on the Activity Reports link. To remove an activity click the Delete link. IMPORTANT: Activity/Training is used to input your daily activity when there is non-case EAP activity. Utilizing this will prevent your plant location from showing delinquency for the Month. 2.) Selecting an option from the list automatically populates Activity/Training with a corresponding category. You may also enter your own description instead. 3.) The activity date defaults to the current date and can be changed if desired. 4.) Enter an activity note (optional). 5.) Click the Save Activity button. bos/opeiu459aflcio 26
29 Printing To print a webpage or report use the print icon on your internet browser: Other methods in Internet Explorer include clicking on the File and Print menu toolbars or Ctrl+P keys. More on printing from Internet Explorer: NOTE: If you are not using Internet Explorer as your default browser such as AOL, Netscape, etc. You will need to contact your Internet Service Provider or refer to their Help instructions on how to print from the browser. bos/opeiu459aflcio 27
