2 Select Role from drop-down list Select Role Signatory Individual who will be signing/certifying submissions to EPA or managing facility user permissions. Preparer Individual who will be viewing and editing data Click Next to continue

3 Personal Information Fill out required information All boxes with asterisks * For Signatories, ensure First & Last Names are your legal names Click Next To continue

4 User ID and Password Create User ID and Password Choose 3 Security questions Enter an answer Are case Sensitive Agree to the Terms and Conditions

5 For Signatories (only): Signature Question Verification Select 5 security questions from the drop-down list and provide answers Answers are case sensitive These questions will appear when electronically signing Click Next

6 Search for your organization. Organization name and State are required, other search fields are optional Organizational Information This is the organization you work for (your employer) Click Find to continue

7 A list of similar named organizations may appear. Make sure you select the correct organization by looking at the address and city. Click Select on the lefthand side to add your organization to continue NOTE: If you cannot find your organization, skip to slide 12. Organizational Information

9 Contact Information Fill out required information Click SEND VERIFICATION CODE. A green pop-up note will appear on screen informing an was sent to this . You will receive an from CDX helpdesk with a Verification Code within a few minutes.

18 Signatory Signatories (formerly Certifiers/DARs/Permit Administrators)will need to continue as additional information is required Identity Proofing Electronic Signature Agreement (ESA)

19 Identity Proofing Fill out required information All boxes with asterisks * Agree to Electronic Signature Agreement (ESA) Click Verify and Sign

Electronic Signature Agreement To complete the process, you will have to enter the password that you newly created in CDX to authenticate your account Answer

20 Electronic Signature Agreement To complete the process, you will have to enter the password that you newly created in CDX to authenticate your account Answer one of your (5)CDX security questions to verify your identity Click Sign to sign the Electronic Signature Agreement This completes the electronic signature process.

21 After successfully signing your ESA, you will be redirected to CDX MyCDX This is the landing page when logging into CDX Clicking on your ROLE will take you to the NeT-MSGP application
