The CMS website is the ultimate source of information and knowledge regarding physician requirements to attest for Meaningful Use. 2 Patient Demographics Registering users for SeeYourChart Once User Registration is selected, there are three choices available to complete the first part of the measure (providing access): 1. Print the page can be printed for the patient to take the information home and access when convenient BUT there is no guarantee the patient will follow through AND accurate typing of the long link will be a difficult task. 2. this option is only accessible if an is entered on the demographics page. NOTE: a personal is required. This link will not be active if a business is on file in OncoEMR. This option is preferred above Option 1 but there is still no guarantee the patient will follow through with login/password creation and then actual login to SYC. 3. Assist the patient to register by accessing the link on a portable device in the office. This is the PREFERRED OPTION because both parts of this measure will be met immediately after the patient accesses the link for creation of password and then subsequent assist can be given to ensure the patient logs into SYC Altos Solutions Page 2 of 9

3 SeeYourChart Registration registration process Once any one of the three actions is taken, C 7.1 measure will have been met for this patient. 4 Patients need to access SeeYourChart (SYC). The first step is to create a login name and password. Assist the patient to select the registration access link (or the patient can do this at home). Complete date of birth and gender then select Verify Information. SeeYourChart - verify DOB and gender Complete user Name, password, and confirm the password > Register SeeYourChart - create UserName and Password Acknowledge the confirmation screen that the patient is registered Altos Solutions Page 4 of 9

6 Continue to close all pop-up screens; select Save to close the Demographics screen. 7 Once the patient s login and password are created, the monitor icon next to the patient s name will change from RED to BLUE. (This will not occur immediately but will be in place after a refresh of the computer). By looking at this at the top of ANY page in OncoEMR, you will have an easy way to know if the patient has access to SYC yet. If a patient forgets their password, this is where you would go to obtain the information for the patient. Demographic page SYC username and password are visible To meet measure 7.2, the patient must log into SeeYourChart. 9 At this point, measure 7.2 has been met for this patient. SeeYourChart Patient fully signed in Note: Direct Messaging will become functional in OncoEMR. When it does, a patient will be able to direct message their information to another Provider from if the Practice has set up this Provider with the direct messaging address. Currently, the patient can print a Summary of Care or download and save electronically from SeeYourChart Altos Solutions Page 9 of 9
