Video Tutorials can be found on our website: So here are some basic instructions to get you started: The file that you downloaded is a Template that was created and intended to be used with in MS Word or Word for Mac. SAVE THE ORIGINAL TEMPLATE IN A SAFE PLACE!. You can make copies of the original for personal use only. First of all, I would suggest that you get familiar with using Word Templates and creating forms in Word. Here are two resources from Microsoft I would suggest you read and get familiar with as you start: c5cc1-e b-94ac-542f732c8c8b Be aware that there are some differences in how you open the document for the template to remain intact. Basically, if you double-click on the template file it will open it as a new document and preserve the template. (It is recommended that you use MS Word or Word for Mac for editing. However, if you are a Mac user and are using Pages, you should still be able to edit the paperwork and use the templates but those steps will be different) 1. First thing to do is customize the template for your pra ti e. Ope the Session Note Helper- asterte plate by Double-Clicking on the document file. This will open the form as a new document. 1

2 2. Enter your practice information at the top of the form. 3. Next under File click on Save As. Be sure to now save this as a Word Template using the file type tab in your folder usi g a differe t a e. e. Pra ti e Name essio Note. This ew template will now become your working session note template. 4. To begin using the new working template, simply Double-Click on the on the file. This will open it as a new Word Document which has all of your practice information pre-loaded and ready to use. 5. To write a session note, simple use the content boxes and dropdown menus to complete the form. You will notice that as you fill in the note, the content boxes are removed. So if you need to edit that note in any way, you just simply type in what needs editing after you have made a selection. (If none of the phrases fit for your note, simply select another one, and then type in a corrected phrase.) 2

When you complete your note, you can then Save As a new document or copy and paste the contents of the note to your practice management software or any other electronic record keeping you might have. You can also create a PDF file of you note using the Export function in Word. To do this go to File then Export. Tips For Customizing Further!- Keep your working template open in another window as you work to add new phrases as you begin completing your session notes.. To do this you will need to first open the template in Word. Go to File then Open. Find the working template and open it. By doing this you can edit the working template. Or simply Right-Click on working template in the file folder and click Open. Here are the steps: Make sure the Developer tab is enabled. To do this: o Click the File tab. o Click Options. o Click Customize Ribbon. o Under Customize the Ribbon, click Main Tabs. o In the list, select the Developer check box, and then click OK. After you have enabled the Developer tab click on the Developer tab, under Controls and click on Design Mode (this needs to be on anytime your change content boxes) Next to add to new phrases to a dropdown content box, click on the content box to highlight it. (where it says, Choose a ite. ; do t use the drop dow arrow. Click on the Developer Tab again under Controls and click on Properties. This will open up a dialog box. Click on Add then type in the phrase you want to add. You can also click on any existing phrases and click on Modify to change the wording.
