3 GENERAL INFORMATION The instructions in this guide will provide you with the necessary information to complete your 2012/2013 French Language Services (FLS) report. This user guide is intended for identified agencies only. Identified agencies have the obligations in their M-SAA / H-SAA / L-SAA to report on FLS to the LHIN. The completion of this report, designed as a survey, will allow identified agencies to be in compliance with their accountability agreement with the LHIN, regarding reporting on French Language Services. This FLS report is in an Excel format and includes Macros. Make sure to enable macros in order to be able to complete this file. Refer to the most recent CATSRI User guide (Appendix) for detailed instructions. Alternatively, if, when you open the report you see the illustrated below, click on it to proceed with the report. button, as For additional information, please contact your LHIN FLS Coordinator. 2

4 MAIN MENU The following steps will have to be completed in step order to finalize your FLS annual report: Set up Reporting: Step 1: Provider Details. Step 4: Access. Step 2: FLS Data. Step 5: FLS Progress Report Step 3: Select Services provided. Click on the name (in Blue) of each step to be directed to the page to complete. Start with step 1 up to step 5. When you open the FLS report, you see the following page; this is your main menu: 3

5 STEP 1: PROVIDER DETAILS In this section: Complete the Details column by typing the requested information in the yellow cells, and by choosing the answers from the drop down lists offered in blue. It is possible to return to the main menu by clicking on the hyperlink Return to Main Menu at the top left of the page. When the page is completed, click on the hyperlink Continue to Step 2 of initial Setup>>> at the bottom left of the page to complete step 2. 4

6 STEP 2: FLS DATA In this section: Complete the Data column to the best of your knowledge by typing the requested information in the yellow cells. It is possible to return to the main menu by clicking on the hyperlink Return to Main Menu at the top left of the page. When the page is completed, click on the hyperlink Continue to Step 3 of initial Setup>>> at the bottom left of the page to complete step 3. 5

7 STEP 3: SERVICES SELECTION SCREEN In this section: Enter an x under the Funded Services column for all the services funded by the LHIN in your agency. Upon selection, the services will automatically be transferred into the next two steps (Access and HR). The lists of services are extracted from Ontario Healthcare Reporting System (OHRS). It is possible to switch the listing from Hospital services to Community and LTC services by clicking on the hyperlinks Link to Community and LTCH Services >>> (illustrated below) and Link to Hospital Services>>>. It is possible to return to the main menu by clicking on the hyperlink Return to Main Menu. When the page is completed, click on the hyperlink Continue to Step 4 of the Report>>> at the top of the page to complete step 4. 6

8 STEP 4: ACCESS The purpose of this section is to provide an overview of the accessibility of French language services within the HSP. Accessibility is defined as the extent to which the Francophone population in need can use the services. Only the services selected in step 3 Services Selection Screen are listed in step 4 Access under Service / Program Name. If some services are missing, go back to step 3 to refine the selection. The catchment area appears automatically as selected in step 1 Provider Details. If the service is offered in a different catchment area, overwrite it and indicate the proper catchment area. The status of the service appears by default as identified. Should the service status be different, select the proper status from the drop down list. For each service, indicate under FLS Coverage the % of time that FLS are available during the hours of operation. It is possible to return to the main menu by clicking on the hyperlink Return to Main Menu. When the page is completed, click on the hyperlink Continue to Step 5 of the Report>>> at top of the page to complete step 5. Your list will look different according to the services selected in step 3 Services Selection Screen. 7

9 STEP 5: PROGRESS REPORT This report is built as a survey, aligned with FLS implementation and designation plans. Please, answer the series of questions. For each question: - In the column Answers select an answer from the drop down list. You will have the opportunity to explain your answer in the Additional comments column. - The next question will appear when you have completed the answer to the current question or series of questions. - In the column Planned in the FLS Plan select from the drop down list in bleu Yes if the action is already included in your FLS plan or No if the action is not included yet in your FLS plan. Comments will appear in cells identified with a red triangle in the upper right corner as illustrated bellow. These comments will bring you some clarification regarding the meaning of the section or the question. It is possible to return to the main menu by clicking on the hyperlink Return to Main Menu. Remember to save your report. 8

10 ACRONYM TABLE & DEFINITIONS Acronym Table FLS French Language Services FTE Full Time Employee H-SAA Hospital Service Accountability Agreement HR Human Resources HSP Health Service Provider L-SAA Long-Term Care Home Service Accountability Agreement M-SAA Multi-sector Service Accountability Agreement PT Part Time Definitions Active Offer To ensure that members of the public know they have the right to use the official language of their choice, the institutions are required to actively offer services in both languages. The requirement consists of informing the public that services are available in both official languages. FLS committee The FLS committee is a permanent committee who makes recommendations to the Board, and reviews FLS policies annually. FLS working group The FLS working group will support the implementation of the FLS plan. Identified agencies An identified HSP, program or service has been identified by the former district health council, the LHIN or the Hospital Services Restructuring Commission to provide some or all of its services in French. A list of identified HSPs is available from your LHIN. Linguistic variable Information related to the identification of the language of the patient. 9
