Welcome to the Ellwood City Hospital s Patient Portal, hosted by InteliChart Our patient portal allows you to access your medical records from the comfort of your own home, whenever you need them. It will allow you to view and send your records to your next provider. Ready to get started? Creating a Patient Portal Account with a PIN If you received a PIN from a recent inpatient stay in the hospital, you can create your account by clicking on the following link: Ellwood City Hospital Portal - PIN Registration On the following screen, type in your PIN (without the dash) and last name, and click Continue 3
Once you ve hit continue, the next page will ask you for some login information. All fields are required. Type your email here Retype your email here Your email address will be your login. It must be unique to you NOTE: Your password must be between 8 and 30 characters with at least one number and one letter. Type password here Retype password here Choose two security questions in case you forget your password Type the answer to your 1 st security question here Type the answer to your 2 nd security question here Be sure to check that you have read the terms and conditions 4
Once you have completed your registration, all that remains is to check your email for a verification message. Click on the link in that message to verify your account. You can then log in using your email address and password. 5
Creating a Patient Portal Account Without a PIN If you wish to create an account but do not have a PIN, you can do so by clicking on the following link: Ellwood City Hospital Portal - Self-Registration On the following screen, choose whether you are registering for yourself or your child: If you are registering for your child, the child must be 13 or younger. Also, you must have an account in order to link it with your child s. If you do not have an account already, you can create one when you register your child. 6
On the next screen, fill in your information, and click Continue. Type first name here Type last name here Use Calendar icon to choose your birth date Type street address here Type city here Type state here Type 5 digit zip code here 7

Once you have completed your registration, there are two more steps to be completed: 1. Check your email for a verification message. Click on the link in that message to verify your account. 2. Come to the hospital to have your identity verified before private health information can be sent to your portal. 1. Verification can be done at either the admissions or medical records departments. 2. Please bring a photo ID for verification You can then log in using your email address and password. 9
Creating a Patient Portal for Your Child Remember: to have a portal account, your child must be 13 years old or younger. Your child was given a PIN during his/her last hospital visit. Click here and enter the PIN and your child s last name. Click Continue 10
You will see this screen: Click Here if you DO NOT have your own account Click Here if you have your own account If you choose, I Have an Account, your next screen will be a log in screen: Log in to your portal account with your own email and password 11
If you choose, I ll Need to Create an Account, your next screen will be a registration screen. Complete this screen with information as it applies to you, NOT your child. Continue with the registration as previously explained. Once you ve completed your registration and clicked on your email for verification, you will be able to see your child s account by clicking on the Accounts tab on the top banner. Your child s account can be found by scrolling to the bottom of the page. Simply click on your child s name to view his/her information. 12
Your portal can be personalized by adding a profile photo. To do so, first be sure you have a photo of you on your computer. Be sure you know where this photo is located. Once you have a photo and know its location on your computer, log into your patient portal here and then use your mouse to hover over the empty gray picture until you see the option Change pop up: Click Here to Change Your Photo On the next pop-up, click Browse to find your photo. Once you ve found it, click Save to set your photo. Your profile picture is now set: 13
Finding Your Health Information: When you first log on to your patient portal, you are faced with a snapshot of some of your latest health information including medications and lab results. However, to access all your health information, you must view your chart in one of two ways: 1. Along the top border, there is a tab labeled My Health Record. When floating over that tab, you will see options to view your lab results, histories, allergies, visits, problems, and documents: 2. You can also click View My Chart from the left side bar: The View My Chart option displays tabs for all your health information on one page. 14
To view or print a summary of your most recent visit, access your clinical documents either by clicking on documents from the drop down menu OR by accessing the documents tab from the View My Chart option: 15
If you have been a patient in the hospital, you should see a section labeled Clinical Documents with at least one clickable document listed: Click Here to View Your Document When you click on the document, a new window will appear as a pop-up: 16
From that pop-up, you can make the document easier to read by clicking Open in New Window - which will expand the document into a new browser window. To expand the document for easier reading, click here 17
Messages Within the patient portal, there is the ability to send and receive secure messages. These messages can be accessed by clicking the Messages tab along the top of the portal. If you have a new message, you will see a red alert next to Messages When you click on Messages, you will be taken to your inbox. You can read any messages you may have by clicking on them. You can also send a message by clicking on Compose New Message : Click Here to send a new message Click Here to read your message 18
A new box will pop up. Be sure to choose Ellwood City Hospital in the location drop-down. You will then have the option to choose which department you are sending the message to. Once you ve chosen your recipient, simply type a relevant subject and your request/question in the body of the message. Complete the message by clicking Send. Choose Ellwood City Hospital Choose which department to send to Type your message here When complete, click Send You will be able to see any sent messages in your Sent Items tab on your main Messages screen 19
If you request information from medical records through your messages, you may receive that information as an attachment to a returned message. You have the option of saving that document to your computer for future reference and then - if you choose - uploading it into your portal. To save the attached document, click on the attachment s link in your message. You will need to have Adobe Acrobat Reader on your computer. The document will then open in a new window. In that window, simply Save a Copy or Download (depending on your version of the Adobe reader). Save it to a location you will remember in order to find it for uploading. Uploading a Document to Your Portal If you have a document you would like to add to your portal (your own or one generated by Medical Records), return to your documents via either the drop-down menu or the View My Chart option (instructions can be found here) 20
Under Documents, click the Upload a Document link. Click Browse and find the document you would like to upload. Add a description to help remind you what the document is, and click Save. The document will now be available for you to view in the future. Type your Document s Description Here Find the document where you saved it. When finished, click Save 21
Changing Your Login Email and/or Password If you wish to change your login email address or password, click on the Accounts tab. Under My Patient Portal Account, there are options to change your email address and/or your password. Keep in mind that if you change your email address, you must have access to the new address as a verification email will be sent before you can log in with that email. NOTE: Your password must be between 8 and 30 characters with at least one number and one letter. 22
Outpatients: Receiving Your Results Once you have an authenticated account, you can receive your results electronically within your portal. Simply send a message to Medical Records. Click the Messages tab along the top of the portal. If you have a new message, you will see a red alert next to Messages When you click on Messages, you will be taken to your inbox. Send a message by clicking on Compose New Message : 23

Choose Medical Records Request, and simply type a relevant subject and your request in the body of the message. Complete the message by clicking Send. When you request information from medical records through your messages, you may receive that information as an attachment to a returned message. To view those results, simply click on the attachment to the message. This attachment will most likely have a.pdf at the end of it. You will need to have Adobe Acrobat Reader on your computer. The document will then open in a new window. If you are expecting lab results, your labs should populate in the lab tab once medical records has sent them. Please allow adequate time for your labs to be resulted and the records to be sent. 25
