QUICK START USER S GUIDE The Mount Nittany Health team is excited to bring you My Mount Nittany Health, the online tool that allows you to play a bigger part in your own healthcare journey. This Quick Start User s Guide provides instructions for how to get started with using the online tool. It also contains a wealth of information about the major features and functionalities that are available for you to use through My Mount Nittany Health. If you have additional questions that are not answered completely in this document, please call your provider s office or send us an email MyMNH@mountnittany.org.

GETTING STARTED To access My Mount Nittany Health for the first time, you must first complete the one-time enrollment process from one of the options listed below and verify your information for security purposes. 1. Open Enrollment. If you haven t received an enrollment email or letter from Mount Nittany Health: 1. Start by visiting MyMountNittanyHealth.com and clicking the Enroll Now button at the bottom of the login page, which looks like this: How to contact us: If you have any questions or concerns about the portal, please feel free to contact us in any of the following ways. 1. Email us at MyMNH@mountnittany.org. 2. Contact our support line, available Monday Sunday from 8:00 am 8:00 pm, at 814.231.7103 3. Visit mountnittany.org/mnh for additional information about the portal. 4. Submit a technical support form online (available at mountnittany.org/patient-portal-support). 2. After clicking Enroll Now, you will be taken to a form to create your account. Scroll to the bottom and Agree to the Terms and Conditions by clicking on the button. 3. Then click the button that is labeled Create Account Step and complete all required fields Every time you want to access My Mount Nittany Health, visit MyMountNittanyHealth.com and enter the username and password you created during the enrollment process. 4. After creating your account, you may sign in any time to start using My Mount Nittany Health, but the full range of features will not be available until your account is linked to your records by our Medical Records Department in 2-3 business days. 2. Automatic Enrollment Provide your email address during your next visit to a Mount Nittany Health facility. Your email address will be your My Mount Nittany Health user ID. You will receive an email with a link to the My Mount Nittany Health online registration page. Once you verify your identity, you will be an active My Mount Nittany Health user. Note: Each portal account requires a unique email address. If you need an email address, you can get free email addresses from Gmail.com, Yahoo.com or Hotmail.com.

MY MOUNT NITTANY HEALTH FEATURES The portal home page and top navigation menu contains the primary features My Mount Nittany Health has to offer, and this section will explain how you can use each of these features: > Home Tab In this section you can find links to key features such as Quick Links, My Calendar, My Watch List and more. > Quick Links In this section, you can click on the corresponding icons to view lab results & health records, find a physician, manage your medications or send a message for appointments, documents, referrals or medication requests. > My Calendar Use this section to request appointments at any Mount Nittany Health location where scheduled appointments are offered or encouraged, including both Medical Center and Physician Group offices. You can also use this section to view pending future appointments requested as well. > My Watch List You can use this section to select certain lab results you would like to monitor. > Health Summary Tab This section can also be used to manage daily health tracking goals/information, medications and allergies. Here, you can also view any documents your care team has sent you, a list of your care team members, and provider reports and documents. > Secure Mail Tab Secure electronic messages that you send to your provider s office, like questions and appointment requests, are logged and stored here. You will be able to view all secure electronic messages that we send to you, too. Types of messages Mount Nittany Health may send you include answers to your questions, confirmations for your appointment requests and more general announcements. > My Profile Tab This section of My Mount Nittany Health allows you to review the contact information that we have stored for you, such as your address and phone number. You can also use this section to manage your account settings, including changes to your security questions and password. > My Health Dashboard You can use this section to connect and sync your fitness tracker in store your fitness data in one place and share your fitness data with your care team. > Download the app You can now access your My Mount Nittany Health patient portal account from your mobile device. Download the My Mount Nittany Health app for free in the App Store for iphone and ipad or Google Play for Android Devices.

COMMON QUESTIONS What do I do if I forget my username or password? Simply go to MyMountNittanyHealth.com and find the area on the bottom right corner of the page where you would normally enter a username and password. Under the Sign In button, find the following link: "Forgot your user name or password?". The login information area should look like this: After clicking the appropriate link, you ll be asked to answer the security challenge questions you set up when you first created your My Mount Nittany Health account. How do I access my child's or other family member's records? If you would like to become a proxy to another patient's account, visit the Account Settings section under your Profile to complete the required request form. The completed form will be submitted to our Medical Records department. Please allow 2-3 business days for processing. Who else can see my information? Your healthcare provider already sees your medical record information and contact information when you visit Mount Nittany Health facilities for appointments. If you are an adult, no one but you will be able to view your information unless you share your account information * or save, email or fax your own medical record information to someone else. If you are a parent or legal guardian and have been granted access to a dependent s information, the minor should be informed that you as a parent or guardian will be able to see his or her past and future medical record information through My Mount Nittany Health. * Mount Nittany Health does not recommend sharing your account information. Will I be able to contact my physician directly using My Mount Nittany Health? We cannot guarantee that a physician will answer every message sent through My Mount Nittany Health themselves, but we do guarantee that every message will be directed to, and ultimately answered by, your physician s team of professionals, and specifically, those most appropriate to respond to your request or question. Questions and re-quests are always treated as confidential by our staff, just like a phone conversation or in-person conversation at one of our facilities. QUICK START USER S GUIDE 5

What kind of computer, browser or Internet connection do I need to use My Mount Nittany Health? Any computer with a reliable Internet connection and one of the Internet browsers mentioned below will allow you to use My Mount Nittany Health. While any browser could allow you to access My Mount Nittany Health, we recommend using Microsoft Internet Explorer version 8 or higher, or an up-to-date version of Firefox. Safari and Google Chrome can also be used to access My Mount Nittany Health. Any DSL or broadband connection will enable you to access and comfortably use My Mount Nittany Health. If you are unable to access MyMountNittanyHealth.com from work or a public location, please check with your systems administrator to make sure the computer you are using allows you to access external websites.
