3 Objectives 1. Introduce the ImmunizeCA app that helps families manage immunizations. 2. Describe ongoing collaboration between ImmunizeCA and BORN to facilitate the transfer of the information 3. Discuss opportunities to leverage registries and mhealth to deliver personalized care 3

20 Benefit of sharing/exchanging Immunization Data? Parents are a valid source of immunization data Public Health Units in Ontario are mandated to monitor adherence to immunization schedules for children in schools and daycare centres (Immunization of School Pupils Act & the Day Nurseries Act) 20 Need to monitor and trend coverage rates and understand who in the population has been immunized, where they live and go to school Prior to school-age (<5) the opportunities to collect records are few.

21 Registry Role Immunization status is an important public health issue that is gaining momentum. As a child health registry we can play a role in: Storing and sharing immunization data among providers Capturing adverse events Centralizing population data related to immunizations Identifying at-risk populations or individuals 21

23 What a parent will have to provide Parents must consent to sharing data with their local public health unit 23

24 Benefits to the Public Health Unit Additional, novel, and persistent source of vaccination data Reduce the independent burden of parents submitting records by phone, fax, or in-person In future: direct connectivity between the public health central repositories would be a valuable to reduce manual entry burden. 24

26 Future Opportunities for Immunize CA and BORN Provide this feature to other Public Health jurisdictions in Ontario to improve record capture Leveraging our BORN KideConnect work with primary care EMRs to provide bi-directional sharing of immunization between families and providers via the app 26

27 How mhealth could be used obtain better health outcomes? Individuals and families mobilize to improve their own health status when empowered with clear information Imagine opportunities where women could manage their postpartum risk factors of diabetes or hypertension through mhealth applications that direct next steps or best practice 27
