2 How to Renew with the C.H.E.K Institute Follow These 5 Easy Steps: Step 1: Earn the required amount of Professional Development Credits every 2 years, on even numbered years. Refer to the chart on page 6 or the flow chart on page 7 for information on the number of PDCs needed and page 5 for the different groups of PDCs. Include the course, date completed, provider/organization, and number of PDCs. You do not need to submit proof of completion unless requested by the C.H.E.K Institute. PDCs for the 2016 renewal must be completed between December 16, 2014 and December 15, Step 2: Complete the C.H.E.K Institute Renewal Application Form online or send in a signed hard copy to the C.H.E.K Institute Education Department. The hardcopy application is available as a pdf download on the renewal page of the website : Step 3: Pay the Renewal Fee as outlined in the chart below. Renewal applications will not be accepted without payment. Step 4: Obtain Current hands-on CPR Certification and Current Liability Insurance.* You do not need to submit proof unless requested by the C.H.E.K Institute. Step 5: Submit the C.H.E.K Institute Renewal Application Form by December 15, Renewal Applications must be received or postmarked by December 15 th to be considered for renewal. Anything received after December 15 th will be considered a late renewal and will need to go through the Late Renewal Process. Number of Adv. 3 Frequently Asked Questions When have I completed an Advanced Training Program? You have completed an Advanced Training Program when you have passed the take-home final exam. The date you completed the first ATP in that Qualification is the date you will use for renewal. What can I use for my PDCs? Your 1st ATP taken with the C.H.E.K Institute cannot be used for PDCs towards the renewal. All ATPs after the first can be used towards the renewal. PDCs for the 2016 renewal must be completed between December 16, 2014 and December 15, There are 2 PDC Groups. Please refer to page 5 for detailed information. Please Note: C.H.E.K Institute webinars and Q & A calls cannot be used for PDCs unless there is an exam included. Do I have to send a separate Renewal Application Form for each Advanced Training Program I have completed? No, you may submit one Renewal Application Form for all Advanced Training Programs completed. The renewal fee will be determined by the number of ATP Qualifications you are renewing. Please refer to page 2. What if I miss the December 15, 2016 deadline? You will lose your status as a C.H.E.K Institute Trained Professional. You will be unable to enroll in any higher level C.H.E.K Institute Advanced Training Programs in that category. You will lose all benefits as a C.H.E.K Institute Trained Professional (i.e., Marketing, listing on the CHEK Connect Directory, Video Assessment Review on ptenhance, Access to Media Library, monthly webinars and coaching calls). To renew your status after December 15, 2016, you will be required to pass a renewal exam or submit a case history for each Qualification in question, at the posted exam fee, or you may retake each of the highest level Advanced Training Programs that you have completed, at 40% off of the posted tuition. How can I save money on my renewal? You can submit your Renewal Application Form online and save $25.00 USD! The Renewal Application Form will be available for download on June 1, Do I have to have Liability Insurance? All practicing C.H.E.K Institute Trained Professionals must carry current Liability Insurance, unless it is not available in the state or country they work in. Please see page 2 of this document for more information.
