1 Driver License Requirement for Tax Year 2017 ** Please bring your NYS driver license with you for you and your spouse (if married) to your tax appointment or provide a copy (front and back) with the rest of your tax information. New York State is REQUIRING taxpayers to provide a driver license or state issued ID (except for a deceased person) for returns prepared using software. Here is the information as provided by NYS: Overview Beginning in Tax Year 2015 following the lead of the IRS New York began to capture taxpayers driver license or state-issued ID (non-driver ID) information for their electronically-filed personal income tax returns as an additional verification device to deter fraud. Providing this information was optional for Tax Year Beginning with Tax Year 2016, this information is REQUIRED for New York State personal income tax returns that are prepared using software. The requirement applies to driver licenses or stateissued IDs from any state. These fields appear in the Authentication Header section of the software's XML, however they're not visible on the return. If any taxpayer on any e-filed return (primary or spouse, not dependents) has either a driver license or state-issued ID, the following information is required: - ID number; - issuing state; - issue date; - expiration date; AND - document number ((The document number is specific to NYS IDs and is typically found in the bottom right corner on the front, or on the back of the ID.) If the taxpayer does not have a driver license or state-issued ID (or is deceased), they ll be presented with an option to indicate that they don t have either one. This will fulfill the requirement. It also helps us prepare the most accurate return possible and helps us legally minimize your tax liability or maximize your potential refund. Please note: For any return that is prepared while you wait, payment is expected at the time of completion. (We cannot legally deduct fees from your refund.) If you answer "yes" to any question, please provide the requested information so we can prepare a complete and accurate return in a timely manner. If you are unsure of the question or how to answer, you can leave it blank. YES NO 1) Did your marital or tax filing status change during 2017? If yes, provide new status. 2) If you are married and filing a joint return, does your spouse have any garnishments, unpaid student loans that are not current, or tax liens that the other spouse is not responsible for? If yes, provide details. 3) Did your address or phone numbers change during 2017? If yes, provide new address, new phone numbers and/or date of move. 4) Are you (or your spouse if married), legally blind for 2017? 5) Do you have any NEW dependents (including a relative for whom you provide more than 50% of their support) that you can claim for 2017? If yes, provide their name(s), social security number(s), date of birth(s), relationship to you, and how long they lived with you during ) Do you have any dependents you can no longer claim for 2017? If yes, provide their name(s). 7) Were you an employee at any time during 2017? If yes, provide W-2's. 8) Did you receive income from Social Security, a pension, an annuity, an IRA, a 401k, a 403b, deferred compensation plan, or a myra during 2017? If yes, provide statements (1099-R, SSA-1099, or RRB-1099). 9) Did you rollover any retirement funds from one account to another? If yes, provide statements (1099-R, SSA-1099, or RRB-1099). 10) Are you (or your spouse if married), a retired public safety officer (police, fire, rescue, ambulance or chaplain) AND had your accident, health or long term care insurance premiums paid directly from your pension? If yes, provide details of premiums paid. 11) Did you receive any interest from bank accounts, tax-exempt interest, or dividends from investments? If yes, provide statements (1099-INT and 1099-DIV). If yes, provide details of amounts and countries. 13) Did you sell any stocks, bonds, mutual funds or own a stock that became worthless? If yes, provide sales price, sales date, purchase date, and purchase price. 14) Did you LOAN someone money that is no longer collectable? If yes, you MUST provide WRITTEN details of amounts loaned, when, why and WRITTEN PROOF why it is no longer collectable. 15) Did you sell any real estate (including land, personal residences, or rental property)? If yes, provide closing or HUD settlement statement. If not a personal residence, also provide purchase date, price and cost of improvements. 16) Did you receive unemployment benefits in 2017? If yes, you MUST go online and print a copy of your 1099-G. 17) Did you receive any other income in 2017? (Such as: alimony, gambling, lottery, prizes & awards, partnerships, S-corporations, education savings account, distributions from an HSA or ABLE account, distribution due to death or disability of a public safety officer in the line of duty, distribution for wrongfully incarcerated individuals, etc.) If yes, please provide 1099 s, K-1 s and/or amounts received. 18) Did you receive any rental income in 2017? If yes, provide WRITTEN records of income and expenses paid. (This can be printouts from accounting software, spreadsheets, handwritten bookkeeping or the actual receipts.) If yes: Did you make any payments that would require you to file Form(s) 1099? If yes, did you or will you file required Forms 1099? Did you use your vehicle to collect rent and make repairs? If yes, provide details of mileage driven. 19) Did you receive any income from self-employment in 2017? If yes, provide WRITTEN records of income and expenses paid. (This can be printouts from accounting software, spreadsheets, handwritten bookkeeping or the actual receipts.) If yes: Did you make any payments that would require you to file Form(s) 1099? If yes, did you or will you file required Forms 1099? Did you use any part of your home for business purposes? If yes, provide amounts paid for insurance, repairs and utilities. Did you use your vehicle for business purposes? If yes, provide total miles driven and miles drive for business. Did you pay health insurance premiums for a child under age 26? If yes, provide amounts paid. 20) Were you (or your spouse) an educator (principal, teacher, etc.) in 2017 AND did you spend more than 900 hours in the classroom? If yes, provide amounts paid for related expenses (supplies, books, equipment, etc.) 21) Did you incur travel expenses as a member of the military (as part of your service obligations) for travel that was more than 100 miles (one way) from your home? If yes, provide details and amounts paid. 23) Did you make a contribution to a traditional IRA, a Roth IRA, or a myra? If yes, provide form 5498 or details and amounts paid. 24) Did you pay any interest on a student loan for you, your spouse (if married), or a dependent (if the loan is in your name)? If yes, provide Form 1098-E. 25) Did you pay any college tuition for yourself, spouse (if married), or dependent? If yes, provide amounts of expenses paid, in addition to Form 1098-T. 26) Did you make a contribution to a health savings account? If yes, provide amounts paid. 27) Did you pay any job related moving expenses for a move that was greater than 50 miles? If yes, provide details and amount of expenses paid. 28) Did you pay alimony or spousal maintenance (NOT child support) during 2017? If yes, provide amount paid, ex-spouse's name and social security number. 29) Did you make any gifts to any one person in 2017 in excess of $14,000? If yes, provide dates of gifts and to whom. 30) Did you claim the first-time home buyer credit in 2008? If yes, provide amount of credit claimed. 31) Did you purchase a fuel cell vehicle, purchase an electric vehicle, or install alternative fuel vehicle refueling property during 2017? If yes, provide date purchased and cost of vehicle or property installed. 32) Did you pay any child care expenses? (Daycare, after school, pre-k, etc.)? If yes, include the provider's name and tax ID number, amount paid, and for which child the expenses were paid for. If yes, did you receive dependent care benefits from your employer? 33) Did you pay any adoption expenses for an adoption that was finalized in 2017? If yes, provide details of expenses paid. 34) Did you make any energy saving improvements to your home in 2017? (This would include solar, insulation, exterior windows and doors, certain furnaces or A/C.) If yes, provide details of improvements and amounts paid. 35) Were you, your spouse (if married), or any of your dependents enrolled in a qualified health plan during the year? If yes, provide form 1095-A, 1095-B, or 1095-C. 36) Were you, your spouse (if married), or ANY of your dependents WITHOUT health insurance during ANY part of the year during 2017? If yes, provide names and dates when there was no health insurance coverage. Page 4 of 7

5 If you are able to itemize (file long form), questions #37 - #49 apply to you. If you are unsure, please answer questions #37 #49 to the best of your ability. YES NO 37) Did you make any major purchases or start a new lease (such as car, boat, mobile home, RV, etc.) during 2017 for which you paid sales tax? If yes, provide details of purchase/lease and sales tax paid. 38) Did you pay any medical expenses that were NOT reimbursed (including POST-tax health insurance premiums, co-pays, prescriptions, doctors, dentists, hospitals, glasses, therapy, nursing home care, etc.)? (Visit Tax Center, for a detailed list to download) 39) Did you purchase a new home in 2017? If yes, provide a copy of the HUD settlement or closing statement. 40) Did you pay any property taxes, mortgage interest, or home equity interest during 2017? If yes, provide statements. (Form 1098 and/or property tax receipts) 41) Did you receive a New York State property tax freeze rebate/credit? If yes, provide the check stub or amount. 42) Did you make any charitable contributions by check and/or cash in 2017? 43) Did you donate property (such as clothing or furniture) to a charitable organization? If yes, provide description and value of items donated. 44) Did you donate a car, truck, vehicle, boat or aircraft to a charitable organization? If yes, provide Form 1098-C 45) Did you pay any miscellaneous expenses (including union dues, tools, supplies, tax preparation fees, safe deposit box, continuing education, uniforms, working out of town, IRA custodial fees, professional license, military expenses, etc.)? If yes, provide details of amounts paid and for which expense. (Visit Tax Center, for a detailed list to download) 46) Did you use any part of your home OR your personal vehicle (other than commuting) on behalf of your employer? If home office, provide amounts paid for insurance, repairs and utilities. If you had mileage, provide business miles driven as well as total miles driven. 47) Did you re-finance your mortgage during 2017? If yes, bring a copy of the closing or HUD settlement statement. 48) Are you an individual with a disability and have impairment related work expenses? 49) Did you incur a substantial loss (more than 10% of your income) due to damaged or stolen property during 2017 that was NOT reimbursed by insurance? If yes, provide details of loss and dollar amount of loss. (End of questions for long form or itemization. Please continue on the next page.) Page 5 of 7

If yes, provide details of amount paid. 53) Were you a volunteer firefighter or ambulance worker for the entire year? If yes, provide name and address of company or department. 54) Did you receive a volunteer firefighter or ambulance worker length of service award? If yes, please provide the amount that was paid to you. 55) Did you rehabilitate a historic property or make improvements to a certified "Green Building" during 2017 that was located in New York State? If yes, provide details of expenses paid. 56) Did you purchase bioheat (clean heating fuel) to be used for space heating or for hot water production for residential purposes? 57) Did you directly pay the NY nursing home assessment tax to a residential health care facility located in NY state? If yes, please provide receipts with the assessment clearly marked. 58) Do you purchase or lease a solar energy system and install it at your principal residence? If yes, provide date and cost of the solar equipment. 59) Did you buy anything via the Internet, through a catalog, from a television shopping channel or on an Indian reservation for which you DID NOT PAY NY State sales tax? If yes, provide details of amounts purchased or we can use a flat rate chart. 60) Is your TOTAL income less than $18,000 for the year 2017? If yes, provide amount of rent paid or property taxes paid. 61) Did you make a contribution to the NYS 529 College Savings Program? If yes, provide details of amount paid. 62) Do you need an extra copy of your tax return for the Senior Enhanced STAR Program or the Elderly Pharmaceutical Insurance Coverage (EPIC) program? 63) Were you ever convicted of an offense for bribery of a public servant, corrupting the government, or defrauding the government? If yes, provide the account number of the savings account. 67) If you have a balance due, do you want to pay by direct debit from a checking or savings account on a date designated by you or by credit card?
