Please note: For any return that is prepared while you wait, payment is expected at the time of completion. (We cannot legally deduct fees from your refund.) If you answer "yes" to any question, please provide the requested information so we can prepare a complete and accurate return in a timely manner. If you are unsure of the question or how to answer, you can leave it blank. 1) Did your marital or tax filing status change during 2018? If yes, provide new status. 2) Do you (and your spouse if married) have a driver license or a state issued ID? If yes, bring license or ID with you or provide a copy of the front and back. (Both IRS and NY State are REQUIRING this information to file your return.) 3) If you are married and filing a joint return, does your spouse have any garnishments, unpaid student loans that are not current, or tax liens that the other spouse is not responsible for? If yes, provide details. 4) Did your address or phone numbers change during 2018? If yes, provide new address, new phone numbers and/or date of move. 5) Are you (or your spouse if married), legally blind for 2018? 6) Do you have any children, relatives, or anyone else living with you as a family member in 2018 that were T living with you in 2017? If yes, provide their name(s), social security number(s), date of birth(s), relationship to you, and how long they lived with you during ) Did anyone that you claimed as a dependent in 2017 move out of your house or are no longer eligible to be your dependent in 2018? If yes, provide their name(s). 8) Were you an employee at any time during 2018? If yes, provide W-2's. 9) Did you receive income from Social Security, a pension, an annuity, an IRA, a 401k, a 403b, deferred compensation plan, or a myra during 2018? If yes, provide statements (1099-R, SSA-1099, or RRB-1099). 10) Did you donate any part of an IRA distribution directly to a qualified charity? If yes, provide statements (1099-INT and 1099-DIV). 14) Did you receive income, have financial assets, or have signature power in a bank or brokerage account in a foreign country? If yes, provide sales price, sales date, purchase date, and purchase price. 16) Did you LOAN someone money that is no longer collectable? If yes, you MUST provide WRITTEN details of amounts loaned, when, why and WRITTEN PROOF why it is no longer collectable. 17) Did you sell any real estate (including land, personal residences, or rental property)? If yes, provide closing or HUD settlement statement. If not a personal residence, also provide purchase date, price and cost of improvements. 18) Did you receive unemployment benefits in 2018? If yes, you MUST go online and print a copy of your 1099-G. 19) Did you receive any other income in 2018? (Such as: alimony, gambling, lottery, prizes & awards, partnerships, S-corporations, education savings account, distributions from an HSA or ABLE account, distribution due to death or disability of a public safety officer in the line of duty, distribution for wrongfully incarcerated individuals, etc.) If yes, please provide 1099 s, K-1 s and/or amounts received. 20) Did you receive any rental income in 2018? If yes, provide WRITTEN records of income and expenses paid. (This can be printouts from accounting software, spreadsheets, handwritten bookkeeping or the actual receipts.) If yes, provide details of expenses paid. 24) Did you incur any business expenses related to being a performing artist or a fee-based government official? If yes, provide details and amounts paid. 25) Did you make a contribution to a traditional IRA, a Roth IRA, or a myra? If yes, provide form 5498 or details and amounts paid. 26) Did you pay any interest on a student loan for you, your spouse (if married), or a dependent (if the loan is in your name)? If yes, provide Form 1098-E. 27) Did you pay any college tuition for yourself, spouse (if married), or dependent? If yes, provide amounts of expenses paid, in addition to Form 1098-T. 28) Did you make a contribution to a health savings account? If yes, provide amounts paid. 29) Did you pay any moving expenses due to a military order for a permanent change of station and are a member of the Armed Forces on active duty? If yes, provide details and amount of expenses paid (and reimbursed, if any). 30) Did you pay alimony or spousal maintenance (T child support) during 2018? If yes, provide amount paid, ex-spouse's name and social security number. 31) Did you make any gifts to any one person in 2018 in excess of $15,000? If yes, provide dates of gifts and to whom. 32) Did you claim the first-time home buyer credit in 2008? If yes, provide amount of credit claimed. 33) Did you purchase a fuel cell vehicle, purchase an electric vehicle, or install alternative fuel vehicle refueling property during 2018? If yes, provide date purchased and cost of vehicle or property installed. 34) Did you pay any child care expenses? (Daycare, after school, pre-k, etc.)? If yes, include the provider's name and tax ID number, amount paid, and for which child the expenses were paid for. If yes, did you receive dependent care benefits from your employer? 35) Did you pay any adoption expenses for an adoption that was finalized in 2018? If yes, provide details of expenses paid. 36) Did you make any energy saving improvements to your home in 2018? (This would include solar electric, solar water heating, small wind or geothermal. As of the printing of this checklist the credit for insulation, exterior windows and doors, certain furnaces or A/C had T been extended yet.) If yes, provide details of improvements and amounts paid. 4 37) Were you, your spouse (if married), or any of your dependents covered by a health insurance plan at any time during the year? If yes, provide form 1095-A (if enrolled through a health insurance marketplace), form 1095-B (provided by the health insurance company), and/or form 1095-C (provided by your employer). 38) Were you, your spouse (if married), or ANY of your dependents WITHOUT health insurance during ANY part of the year during 2018? If yes, provide names and dates when there was no health insurance coverage. If you are able to itemize (file long form), questions #39 - #53 apply to you. If you are unsure, please answer questions #39 #53 to the best of your ability. 39) Did you make any major purchases or start a new lease (such as car, boat, mobile home, RV, etc.) during 2018 for which you paid sales tax? If yes, provide details of purchase/lease and sales tax paid. 40) Did you pay any medical expenses that were T reimbursed (including POST-tax health insurance premiums, co-pays, prescriptions, doctors, dentists, hospitals, glasses, therapy, nursing home care, etc.)? (Visit Tax Center, for a detailed list to download) 41) Did you purchase a new home in 2018? If yes, provide a copy of the HUD settlement or closing statement. 42) Did you pay any property taxes on US held real estate only, mortgage interest, or home equity interest during 2018? If yes, provide statements. (Form 1098 and/or property tax receipts) 43) Did you receive a New York State property tax freeze rebate/credit? If yes, provide the check stub or amount. 44) Did you make any charitable contributions by check and/or cash in 2018, including the new NY Charitable Gifts Trust fund? 45) Did you donate property (such as clothing or furniture) to a charitable organization? If yes, provide description and value of items donated. 46) Did you donate a car, truck, vehicle, boat or aircraft to a charitable organization? If yes, provide Form 1098-C 47) Did you re-finance your mortgage during 2018? If yes, bring a copy of the closing or HUD settlement statement. 48) Are you an individual with a disability and have impairment related work expenses? 49) Did you incur a substantial loss (more than 10% of your income) due to damaged or stolen property during 2018 that was T reimbursed by insurance AND in a federally declared disaster area? 5 NY State has decided to apply the laws prior to the new tax law that was enacted in December Some of these questions were applicable to both Federal and NY State last year but apply only to NY State this year. 50) Did you pay any miscellaneous expenses (including union dues, tools, supplies, tax preparation fees, safe deposit box, continuing education, uniforms, working out of town, IRA custodial fees, professional license, military expenses, etc.)? If yes, please provide receipts with the assessment clearly marked. 61) Did you purchase or lease a solar energy system and install it at your principal residence? If yes, provide date and cost of the solar equipment. 62) Did you buy anything via the Internet, through a catalog, from a television shopping channel or on an Indian reservation for which you DID T PAY NY State sales tax? If yes, provide details of amounts purchased or we can use a flat rate chart. 63) Is your TOTAL income less than $18,000 for the year 2018? If yes, provide amount of rent paid or property taxes paid. 64) Did you make a contribution to the NYS 529 College Savings Program? If yes, provide details of amount paid. 65) Do you need an extra copy of your tax return for the Senior Enhanced STAR Program or the Elderly Pharmaceutical Insurance Coverage (EPIC) program? 66) Were you ever convicted of an offense for bribery of a public servant, corrupting the government, or defrauding the government? (End of questions for NYS residents. Please continue with question #66.) 67) Did you make any estimated tax payments for the tax year 2018? If yes, provide dates and amounts paid. 68) If you are entitled to a refund, do you want the refund direct deposited into a checking, savings, or retirement account? If yes, provide bank routing number and bank account number or a voided check (not a deposit slip), if different from 2017, for up to three accounts. 69) If you are entitled to a NYS refund, do you want all or a portion of your refund direct deposited into a NYS 529 college savings account? If yes, provide the account number of the savings account. 70) If you have a balance due, do you want to pay by direct debit from a checking or savings account on a date designated by you or by credit card? Electronic copies are easier to store and you can print as many USB (yours) copies as needed throughout the year. This year you have four electronic CD options: 1) Online storage through a secure portal set up by us. 2) a USB USB (ours-$6) flash/thumb drive provided by you (free) or provided by us ($6). 3) ed through a secure website with password protection or 4) on CD provided by us. (If you would like both a paper copy and electronic copy, there is an additional fee of $7) 72) This year we are continuing to offer the popular Stress Free protection plans which covers you in case of an IRS or NYS income tax audit. We already respond to any letter you might receive, so why not have the additional peace of mind knowing that we will represent you if your income tax return ever gets audited. There is a nominal fee for these plans, but it is minimal compared to the hundreds, or even thousands, it could cost you in fees and taxes without proper representation.
