To Start the Online Process Sign into Family Access located on our website (www.deforestschools.org) under Families then Skyward Family Access.

1 Directions for Online Registration Online registration must be done for each student. Fees can be paid as one transaction. There is NO transaction fee for paying online when registering between July 15th and August 6th. Those paying fees online after August 6 th will be charged a $2.00 transaction fee. To Start the Online Process Sign into Family Access located on our website ( under Families then Skyward Family Access. If you forgot your password follow the forgotten password link. If you don t know your login call Click on Online Registration, all children will be registered separately. Click on the child s name you wish to enroll. Please read the message, and then click on the child s name you are enrolling. Step 1 Verify Student Information Step 1a Student Information **NEW** There are three Military questions that parents need to answer. If any of the questions are YES, click the checkbox. If the answers are NO, just leave it as is, don t click the checkbox. Also, you can update your child s home , Birth County, State and Country. If there are any changes in your child s name, birthday, Language, home phone or race you will have to contact your child s school office. Once complete, or if there aren t any changes, click either the link Complete Step 1a only or Complete Step 1a and move to Step 1b 1 P age

Step 1b Family Address Make your address changes. If there are no changes, click the link Complete Step 1b only or if you want to move to Step 1c click the link Complete Step 1b and move to Step 1c.

You do not have to do Steps 1b, 1c, 1d, and 1e for each child. Once you update this information for one child it will update for the rest. Step 1c Family Information Update your employer and home . You can update your phone numbers here too, but you must keep them in the order of 1 st Primary phone (can be your cell if that is your primary phone), 2 nd work phone then the 3 rd cell phone (3 rd phone must be your cell phone even if you have entered it under the primary phone). Click the link Complete Step 1c only or if you want to move to Step 1d click the link Complete Step 1c and move to Step 1d. 2 P age

3 Step 1d Emergency Information Here you update any health alerts; for example allergic to peanuts. If the alert is critical, check the box Critical Alert Information Update any doctor, dentist, hospital, etc. information Click the link Complete Step 1d Only or if you want to move to Step 1e click the link Complete Step 1d and move to Step 1e. Step 1e Emergency Contacts Parents are automatically designated as emergency contacts, but you can have up to four contacts. Here you can add more. Please do not change the order of the phone, 1 st Primary phone, 2 nd Work phone, and 3 rd Cell phone. Please click yes/no if that emergency contact has your permission to pick up your children. Click the link Complete Step 1e Only or if you want to move to Step 1f click the link Complete Step 1e and move to Step 1f. 3 P age

Step 1f Health Information Here you can add any health information for your child.

Once the form is complete, click Save, which will bring you back to this screen.

If you don t have any information just leave it blank (do not add n/a) and click the link Complete Step 1f Only or if you want to move to Step 1g click the link Complete Step 1f and move to Step 1g. Step 2 Permission Form Click on the Permissions Form link and complete the form (see form below). Once the form is complete, click Save, which will bring you back to this screen. Click the link Complete Step 2 Only or if you want to move to Step 3 click the link Complete Step 2 and move to Step 3. Use the drop down menu to answer the yes/no statements. Defaults to Yes Make sure you enter your name and the date you filled out the form. Then click the Save button in the upper right corner. This form is slightly different for elementary/middle and high school. 4 P age

Step 3 Early Release FOR ELEMENTARY STUDENTS ONLY Click on the Early Release Form link and complete the form (see form below).

Click the link Complete Step 3 Only or if you want to move to Step 4 click the link Complete Step 3 and move to Step 4.

Once the form is completed, click Save in the upper right corner. That will take you back to the online registration.

5 Step 3 Early Release FOR ELEMENTARY STUDENTS ONLY Click on the Early Release Form link and complete the form (see form below). Once the form is complete, click Save, which will bring you back to this screen. Click the link Complete Step 3 Only or if you want to move to Step 4 click the link Complete Step 3 and move to Step 4. The Early Release Instructions form is only for ELEMENTARY STUDENTS. Middle and high school students will not have this form. Once the form is completed, click Save in the upper right corner. That will take you back to the online registration. Click the box I have completed this step and move on to Step 4. Step 4 Free and Reduced Meals Application If you want to complete the Free and Reduced application click on the link Free and Reduced Meal Application If you do not wish to apply for free and reduced meals, or do not qualify click the box I do not qualify for benefits or do not wish to complete an application Click the link Complete Step 4 Only or if you want to move to Step 5 click the link Complete Step 4 and move 5 P age

8 Step 11 - Special Transportation request form Print and complete this form if you want to request special transportation for your child. Step 12 Bridging Brighter Smiles Bridging Brighter Smilies (formerly Smiles 4 Life Dental), a non-profit organization formed in January of Their vision is to create a community where every child is given the opportunity to receive preventative oral care. Bridging Brighter Smiles increases access to preventative oral hygiene care by bringing portable dental equipment right into schools. We offer families peace of mind as we accept the Forward Card (BadgerCare) and no child is ever turned away due to a lack of financial resources. Complete this form if you would like them to contact you. If you do not wish to enroll your student click the checkbox at the top of the form (next to the Print button) I do not wish to fill out this optional form This will complete this step and you can move to the next step. 8 P age

Step 13 Make a Fee Payment Click Make a Fee Payment to make a payment. Your Program and Textbook fees will automatically show for all your children.

9 Step 13 Make a Fee Payment Click Make a Fee Payment to make a payment. Your Program and Textbook fees will automatically show for all your children. You may add optional fees for any child, like sports, athletic passes, yearbooks, etc. High school and middle will have a few more optional fees than elementary. Click the link Complete Step 11 Only or if you want to move to Step 12 click the link Complete Step 11 and move to Step 12 Click on Update Payment Amount you can add additional fees to pay, like sports, athletic fees, yearbooks, etc. You can also put money in your child s lunch account. To add the Program Fees and in middle and high schools, Textbook fees. If you have a previous balance from the last school year it will also show up here. Just click on the boxes under Pay Charge To add the additional fees click on Add. You can pay for all of your children at the same time. There is a $2.00 transaction fee to pay online by charge card after August 6 th. 9 P age

10 NEW LOOK FOR REVTRAK Online Payments Once you have what fees you are paying click Make a Payment This screen will pop up, to pay fees choose Purchase Fees Click on one of your student s name 10 P age

11 Click on which fees you want to pay. It will add the fees to the right. Now click Add selected to Cart You can either click Continuing Shopping to add your other student fees or to put money in your Food Service account or click Checkout to pay online. If you haven t created a RevTrak account in past years, you will have to before you pay. You can pay online by echeck, credit or debit card. 11 P age

12 You don t have to pay your fees on line you can come to registration and pay in person by check or cash. However, There is NO transaction fee for paying online when registering between July 15th and August 6th. Those paying fees online after August 6th will be charged a $2.00 transaction fee. Program and Textbook fees are due by the first day of school (but can be paid ahead of time). All other fees are due right away. Step 14 Complete Online Registration Make sure all steps are completed. Then click the link Submit Online Registration. You will receive an automatic stating that you have completed the online registration. The examples shown above are for elementary school students.
