1 PREINSTALL CHECKLIST Thank you for your investment in the i-cat FLX V-Series Cone Beam Imaging Solution. To ensure a smooth integration of this technology into your office, please complete the following checklist as confirmation of the necessary requirements that need to be met prior to the onsite installation. If there is anything we can assist you with, please call the Installation Coordinator at , please select option 3. Please note, your IT Specialist / Administrator is REQUIRED to be on-site for the software installation and your practices network must be fully functional the day prior to and day of the scheduled installation date. the checklist on the following pages and notate below with the status and any concerns on a particular section. CONE BEAM SHIELDING PLAN It is recommended that a Cone Beam Shielding Plan be completed to ensure that the equipment meets all state requirements. Your Equipment Specialist will assist in the submission of your shielding plan. If you have any questions regarding this process please speak with your local i-cat Representative. ELECTRICAL SPECIFICATIONS 115 VAC, 50/60 Hz, 10 Amps ( VMC) This scanner uses facility power and requires a dedicated line. All electrical devices should be protected from external power disturbances, surges, and spikes that can damage your equipment. i-cat has specified an optional power conditioner for purchase to provide that protection, off-the-shelf surge protectors purchased in an office supply or hardware store are not recommended.

3 4. NETWORK REQUIREMENTS All network components are required to be working at 1Gb/Sec. Wireless networks are not supported. Switch: 1 Gbit switch (10/100/1000Mbit) Network Cables: Cat 5e, 6 or higher Network Cards: 1Gbit Internet Access: High Speed Cable/DSL or better for Remote Support Do you have a network or firewall setup that would prevent tech support s ability to remotely diagnose i-cat software? Yes No i-cat will supply a touchscreen controller device that will manage the operations of the i-cat imaging modality. This device will require a network drop to connect to your network. The Controller Device is designed to be accessed as a network appliance and should not be added to a domain or altered in any way, alteration will render the system unusable and a replacement may be required. Use of a wireless mouse, keyboard, or monitor other than what is provided is not recommended. 5. SERVER REQUIREMENTS Mac servers are not supported. DATA STORAGE REQUIREMENTS The Data Storage of the 3D image requires a certain amount of hard drive space that will depend on: The number of the scans taken Type of data stored (other X-ray devices and applications) How long you want/can use your current storage solution We recommend a minimum dedicated data storage of 1 to 2 TB.

Backup System To protect your data, a reliable backup solution is mandatory. This should be in accordance to your expected data volume. Automatic backup should only be scheduled outside of normal business hours. Note: RAID is NOT a replacement for a backup system as it will not protect your data from all hardware failures, user errors, fire, water, or electrical storm damage. i-cat cannot be held responsible for any loss of data, regardless of reason.
