4 Management Reports Management Reports is a Q/ris 3000 Client feature that allow the user to obtain statistical information about medical activities: activity by Ordering Physician, Statistics Report by procedure, etc. Each report type uses only the designated template, which is stored in a folder on the server hard disk and specified in the Q/ris Server application. Management reports can be accessed by clicking Reports > Build Reports in Q/ris 3000 Client. Creating Reports 1. In the Q/ris 3000 Client main interface window, click Reports > Build Reports. 2. In the Reports dialog window, check the settings by clicking the DB Settings button: In the Connection section, confirm that the check box for the server you wish to connect to is selected. You can only connect to one server at a time, so if there are multiple servers in the list, be sure to select the appropriate check box. 2

5 Click the OK button to save the changes or the Cancel button to abort them. 3. Select the report you want to create from the Type of Report drop-down list. (see the Reports Type Description section for details.) The report types available in the list are those that have been checked in Q/ris Server > Configuration > General > General Configuration window > Management Reports. 4. Click the Query button and set the criteria for the desired results. If there are inconsistencies between the selected criteria and data base records or if the system has insufficient data, the query cannot be performed and an error message appears. If the selected report can display the results also as Graphs, then a dialog window appears, after the query parameters are set and the Ok button is pressed, requesting to choose how the results should be displayed. 3

6 Choose one of the provided options and then click the Ok button. 5. Click the Print button to print the report. If the Q/ris Server cannot access the folder with the report templates, the corresponding report template is missing, or the path to this folder is not correctly specified in Q/ris Server, then an error message is displayed. Exporting the reports The Reports dialog window contains a feature that allows the user to send the results of a report to a MS Excel worksheet. To export the results 1. Create the report as described; 2. Click the Export button in the Reports window. MS Excel opens showing a worksheet with the contents of the report. Note: To use this feature, MS Excel 2002 or a newer versions must be installed. Custom Management Reports The users can add new custom reports in addition to the predefined management reports. Custom Management Reports are defined by the users and allow them to obtain the information that meets their particular needs. There are two distinct actions regarding custom management reports: Add or edit a new custom management report a new report is created or edited. It will be available when the Custom Type of report is checked; 4

7 Run a custom management report a custom management report is selected from the list and is run to obtain the information from database. The users need distinct rights for each of the actions presented above: Admin Custom Management Reports only the users that have this right may create or edit a new custom management report. They also may run these reports; Custom Management Reports - the users that have this right may run custom management reports, but they may not create a new custom management report. When the user has at least one of these rights, only then are visible the Predefined and Custom radio buttons. When the user has the Admin Custom Management Reports right, the Add and Edit buttons became visible. Adding Custom Management Reports 1. In the Q/ris 3000 Client main interface window, click Reports > Build Reports; 2. In the Reports dialog window, check the Custom radio button. If there are no Custom reports defined, an Error message displays. Click the Close button to continue. 3. In the Reports dialog window, click the Add button; 4. In the Dialog window, define the custom management report using available controls: Custom Report Name give a name to the report. This name will be displayed in the Type of Report drop-down list from the Reports dialog window; Custom Report ID this is a non-editable control that displays the generated Report ID; SQL Script enter the SQL script in this text box to get the required information from the database. The script should use a valid SQL syntax to obtain the information. The validity of the script can be tested using the Test Script button; Column Names the user may specify the column names that will display in the result table. The column names must be separated by a comma; 5. Click the Test Script button to test the SQL syntax validity. If the script is correct, the results display in the Query results box. If the script is not correct, an error message displays. 5

8 6. Click the Save button to add the report to the Custom Type report drop-down list. Editing a custom report The Custom Management reports can be edited and saved for later use. To modify a defined custom report, follow the steps below: 1. In the Q/ris 3000 Client main interface window, click Reports > Build Reports; 2. In the Reports dialog window, check the Custom radio button. The drop-down list displays the available reports. 3. Select the report from drop-down list and click the Edit button. 4. In Dialog window, make the changes in the fields described in the Adding Custom Management Reports section. 5. Click the Save button to save the changes. 6

11 % of Business by Ordering Physician (% of Business by Ordering Physician) Provides the top 10 Ordering Physicians based on the number of ordered exams, that are not Cancelled, in the selected month. Criteria settings: Facility (on which the user has rights), Month The columns indicate: # of Procedures Month the number of the procedures ordered by the physician in the selected month. % of Day the percent of procedures ordered by a physician on the selected day based on the total of counted procedures/day % of Month the percent of procedures ordered by a physician in the selected month based on the total of counted procedures/month % of Year the percent of procedures ordered by a physician in the selected year based on the total of counted procedures/year. 9

12 Cancelled Procedures by Date Range Provides a detailed list of cancelled procedures that were scheduled by the user in the specified time interval. Also specifies the cancellation reasons Facility (on which the user has rights), Time Interval (Beginning Date, Ending Date). 10

13 Completed Procedure Activity % (Custom) Provides statistical information about completed procedures by procedure, or procedure group, related to the selected date. The selected date is the date the procedure was completed. The displayed percentages are calculated based on the total number of completed procedures per week to date, month to date or year to date. Subtotals per procedure group are also displayed. Date 11

18 Detailed Patient History (Custom) Provides a list by facility of the procedures of selected patient(s) where the following criteria is met: The procedure status is one of: cancelled, completed, or any status after completed The schedule date falls within the selected time interval Facility (on which the user has rights), Time Interval (Beginning Date, Ending Date), Patients. Exam Date/Time date of the scheduled procedure or forced appointment Status actual status 16

19 Detailed Signed Report Provides the end user a list of the all final signed and addendum reports for the selected radiologists, within the date range selected. The results are sorted by the Radiologists names, also displays a total for each of the radiologists. Facility (on which the user has rights), Time Interval (Beginning Date, Ending Date), Radiologist(s) Option Select all toggle option that allows you to select/unselect all radiologists with a single click. 17

20 Detailed Unsigned Reports (Unsigned Reports) Provides a detailed list, by radiologist, of what assigned results that need a signature. The system collects all procedures with the D, DT and/or P (requires radiologist sign-off) status. The report also displays the unsigned AD procedures. Facility (on which the user has rights), Radiologist. 18

21 Detailed Transcription Report Provides a list of all the procedures transcribed within the selected date range by the selected transcriptionists. Note: Unlike the Summary Transcription Report, this report displays the patient's details as well as the procedure's details Facility (on which the user has rights), Time Interval (Beginning Date, Ending Date), Transcriptionist(s) Option Select all toggle option that allows you to select/unselect all transcriptionists with a single click. 19

22 Details Patient Schedules (Patient Schedules / Orders) Provides statistics of all registered exams for the selected date, by: - week to date from the beginning of the week to the selected date, - month to date from the beginning of the month to the selected date, - year to date from the beginning of the year to the selected date. The results also contain details by the procedure group. Facility (on which the user has rights), Month. The rows and columns indicate: First row procedures by selected facility The next N rows procedures by modality, where N is the total number of available modalities. Last row totals for all facilities. Columns the number of the procedures per Day, Week (to date), Month (to date), and Year (to date) in the indicated modality. 20

23 Dictated exams by radiologist Provides a list of the exams dictated by each of the selected radiologist in the specified time interval. There are counted the procedures whose status is beyond C (including CN procedures) and dictation time is inside the time interval selected for report. Note: The list is ordered by radiologist s last name then by dictated time. Time Interval (Beginning Date, Ending Date), Radiologist(s) Option Select all toggle option that allows you to select/unselect all radiologists with a single click. 21

24 Duplicate Patient Report Provides a list of the patients that have the same First Name, Last Name, Date of Birth (DOB) or SSN and are registered on the same facility. Facility (on which the user has rights), First Name, Last Name, DOB, SSN! Must be checked at least two criteria to be able to perform the report's query. Options Sort Order the order specified by the numbers from the Sort order column of text boxes specifies how will be displayed the results in list. The criterion that has the smallest number is the first criterion used to sort the list. 22

25 Eligibility Verification Report Scheduling Appointment Report (Custom) Provides a list of scheduled appointments in the specified time interval including checked procedure groups. All procedures with their schedule date in the specified time interval are counted, excludes cancelled procedures. Facility (on which the user has rights), Time Interval (Beginning Date, Ending Date), Procedure Group. Options Select all toggle option that allows you to select/unselect all procedure groups with a single click. Note: The procedures of the patients without at least one Insurance Carrier defined are not displayed. 23

26 Fax Status Report (Custom) Provides a list of faxes sent by facility in the specified time interval. Facility (on which the user has rights), Time Interval (Beginning Date, Ending Date) Options All faxes displays all faxes sent by a facility. Failed faxes only displays the failed faxes. 24

27 General Professional Charge Posting (General Professional Charge Posting) Provides the end user with a list of all professional charges captured for a facility in the specified time interval. Facility (on which the user has rights), Time Interval (Beginning Date, Ending Date) 25

28 General Technical Charges Posting (General Technical Charges Posting Report) Provides the end user with a list of all technical charges successfully captured and sent for a patient in a specified time interval. Facility (on which the user has rights), Time Interval (Beginning Date, Ending Date), Patient. Selecting the patients The patient selection is made in the Select records for query dialog window, available when the Add Patients button is pressed in the Select criteria dialog window. The Select records for query dialog window provides three search criteria, ID, First Name and Last Name, that you can use to find a specific patient. Search results are displayed in the table. To add the patients, to be used to query the database, select them in the search result table and click the Select button. 26

29 Marketing Report (Custom) Provides information about all registered orders regardless of their current status, that have: - The Referring physician registered in the specified date interval or - The Insurance Carrier registered in the specified date interval or - The Attorney registered in the specified date interval.!! The procedures of the patients with an expired primary insurance are not counted. Facility (on which the user has rights), Time Interval (Beginning Date, Ending Date), Filter (Referring physician, Insurance Carrier, Attorney) Date of exam date of schedule or forced appointment. 27

30 Modified Procedures (Modified & Cancel Orders) Provides the end user with a list of procedures that have been modified where the modifications changed their status. Time Interval (Beginning Date, Ending Date), Facility (on which the user has rights), Procedure, Accounts. 28

31 No show / Cancellation Report The report displays a list of cancelled procedures whose Date is within specified time interval, belongs to the selected facility, and has one of the checked Cancellation Reasons. The results are sorted in the following order: a) By Cancellation Reason b) By Appointment/Cancellation Date c) By Patient Name or Ordering Physician Name (depending on the selected option) Time Interval (Beginning Date, Ending Date), Facility (on which the user has rights) Cancellation Reason check the cancellation reasons to be displayed when the cancelled procedures are counted. Options Select all toggle option that allows you to select/unselect all the cancellation reasons with a single click. Date section allows the specification of what date will be used when the report is created: Appointment Date will display the cancelled procedures whose appointment start date/time is within the selected time interval. For the procedures either ordered (O) status or arrived (A, PST, PSP - without being previously scheduled) that were cancelled, are used the ordered date/time values in the specified time interval are used in the query. 29

32 Cancelled Date will display the cancelled procedures whose cancel date is the within selected time interval; Sort section specifies how to sort the results in the report. Patient Name the results are sorted in ascending order by the Patient Name; Ordering Physician Name the results are sorted in ascending order by the Physician Name 30

33 Manager System Audit Report (Order Activity Audit Report) (Custom - MNAP) Provides a list of the final signed procedures whose completed date/time is within the specified time interval, excludes Cancelled procedures. The results list also displays: Date of study order date Status 1 and Date DT (Dictated date), TR (Transcribed date), FN (Final sign date) Status 2 and Date PR print, FX fax, EM Status 3 and Date date submitted to billing Copy to physician name indicates the physicians that will receive a copy of the study Facility (on which the user has rights), Time Interval (Beginning Date, Ending Date) 31

34 Office Productivity Provides the number of procedures with statuses O, OS, A and CN that were performed by users that belong to the Office Staff (OS) staff type from Other Staff user types. There are counted all procedures made O or OS or A or CN in the selected time interval. Facility (on which the user has rights), Time Interval (Beginning Date, Ending Date), Office Staff list. Option Select all toggle option that allows you to select/unselect all users from the Office Staff list with a single click. 32

35 Open Orders (Open Orders) Provide the end user with a list of all procedures that are in one of the statuses before Completed. Facility (on which the user has rights), Time Interval (Beginning Date, Ending Date), Patient Type. 33

36 Order Activity by Financial Class (Detailed Report by Financial Class, Custom) Provides a list of the procedures completed or final signed within the specified time interval, grouped by financial class. Facility (on which the user has rights), Time Interval (Beginning Date, Ending Date), Financial Class. Options Display completed exams the result will contain only completed exams; Display finalized exams the result will contain only finalized exams; Select all toggle option that allows you to select/unselect all available Financial Class with a single click. 34

37 Order Activity Report Provides a list of all procedures moved to and beyond the S or OS status, excludes the procedures with CN status that have their schedule or forced time within the specified time interval. Criteria settings: Facility (on which the user has rights), Time Interval (Beginning Date, Ending Date) 35

38 Order Activity Report (detailed) (Custom) Provides a list of all procedures scheduled at the selected facility regardless their current status or insurances available and whose schedule date or force date is within the specified date range. The canceled procedures are not displayed. The results are sorted by exam time. Facility (on which the user has rights), Procedure group (Beginning Date, Ending Date) Time Interval of schedule start date/time or arrive date/time for "Walk In" patients Options Select all toggle option that allows you to select/unselect all procedure groups with a single click. Include blocked slots when checked, the system also displays blocked slots. Notes Blocked slots are the Q/ris Client calendar slots reserved by a physician for a future exam. If the patient has more insurances, it is displayed only the primary insurance. 36

39 Order Activity Report Outpatient (detailed) (Custom) Provides a list of outpatient exams containing all procedures scheduled (or forced) at the selected facility within the specified date range. The results are sorted by procedure group. Facility (on which the user has rights), Time Interval (Beginning Date, Ending Date), Procedure Group. Options Select all toggle option that allows you to select/unselect all procedure groups with a single click. Include blocked slots when checked, the system also displays blocked slots. Note Blocked slots are the Q/ris Client calendar slots reserved by a physician for a future exam. 37

40 Ordering Physician Report Provides a list of all Ordering Physicians that ordered procedures from the specified Procedure Group at the selected facility in the specified time interval. The procedures that are counted have moved to and beyond the S or OS status and have their schedule time (time planned to perform the examination) or force time inside the report s selected time interval. The procedures with CN status or those created in the selected time interval but whose scheduled time (time planned to perform the examination) is outside the report s time interval are not counted. Criteria settings: Facility (on which the user has rights), Time Interval (Beginning Date, Ending Date), Procedure Group Options: Select All check this option to automatically check all groups from the Procedure Group list. 38

41 Orders by Insurance Carrier Report Provides the end user with a list of the orders from the selected facility, completed in the specified time interval, whose patients are covered by the selected Insurance Carrier. It is also provided the total number of the orders for the selected Insurance Carrier. The displayed results are sorted either by Patient Name or Procedure Group. Facility (on which the user has rights), Time Interval, (Beginning Date, Ending Date), Insurance Carrier. Options Sort by: Patient Name, Procedure Group 39

42 Patient Account Professional Charges (Patient Account Professional Charges Report) Provides the end user with a list of professional charges for procedures that were charged for the selected patients. Note: Professional charges are the fees charged by radiologists for providing consultations, interpretations, and other professional services. Facility (on which the user has rights), Patient(s). Selecting the patients The patient selection is made in the Select records for query dialog window, available when the Add Patients button is pressed in the Select criteria dialog window. The Select records for query dialog window provides three search criteria, ID, First Name and Last Name, that you can use to find a specific patient. Search results are displayed in the table. (list) To add the patients to query the database, select them in the search result table and click the Select button. 40

43 Patient Account Technical Charges (Patient Account Technical Charges Report) Provides the end user with a list of all technical charges captured for the selected patients from the specified facility. Note: Technical charges are payable to the hospital, or other healthcare institution, for performing the procedure or service. Facility (on which the user has rights), Patient(s). Selecting the patients The patient selection is made in the Select records for query dialog window, available when the Add Patients button is pressed in the Select criteria dialog window. The Select records for query dialog window provides three search criteria, ID, First Name and Last Name, that you can use to find a specific patient. Search results are displayed in the table. To add the patients to query the database, select them in the search result table and click the Select button. 41

44 Patient Schedules Provides a list of the procedures by ordering physician. Time Interval (Beginning Date, Ending Date), Facility (on which the user has rights). The columns indicate: # of Procedures the number of ordered, and not cancelled procedures, within the selected date range by Ordering Physician % of Total the percentage of procedures pertaining to the total number of ordered, and not cancelled, procedures in the specified time interval. Phone Percentage of procedures ordered over the phone Web Percentage of procedures ordered over the web Revenue the revenue obtained by each ordering physician 42

45 Perform Studies Report (only 4.2 line) Provides a list of procedures from the selected facility whose status is in F or AD and have the Date of Service in the selected time interval and belongs to the selected procedure groups. The results are ordered by Patient ID. Facility, Time Interval (Beginning Date, Ending Date), Procedure Group 43

46 Procedure Activity (Procedure Activity) Provides the number of new procedures ordered, and not cancelled by the facility within the selected day, month to date and year to date. The cancelled procedures are not counted. Criteria settings: Facility (on which the user has rights), Date The columns are indicating: Metric Procedure Activity the facility. Procedure Count the number of procedures for the selected day. (Excludes cancelled procedures) Month to Date the number of procedures in the selected month. (Excludes cancelled procedures) Year to Date the number of procedures from the beginning of the year to the selected date. (Excludes cancelled procedures) 44

47 Procedure Statistics Report (Procedure Statistics) Provides statistical information for the procedures with C status and beyond, excludes Cancelled procedures. Criteria settings: Facility (on which the user has rights), Time Interval (Beginning Date, Ending Date) The columns indicate: Count how many times the procedures were used in the specified time interval. YTD Year to Date how many times the procedures were used from the beginning of the year to the selected Ending Date. Percentage of Total is the percentage of the YTD value that makes up the Total 45

48 Procedure Turnaround Statistics (Procedure Turnaround Statistics) This report provides the end user with an analysis of the time it takes to process a procedure from the time of the request to the result distribution. The values displayed in the columns of the result list indicate the hours that have passed from one status to another. Time Interval (Beginning Date, Ending Date) selects the final signed procedures whose A (arrive) date is within the selected interval; Facility (on which the user has rights) the user may select an individual facility or all facilities Procedures/Procedures Group creates the report with the selected procedures or procedure groups. Options Time Range: By Date Range when selected, the report will use only the specified date range; By Date Range and Time Shift when selected, the report will use only the specified time interval of each day for the selected date range. Procedure: Procedure Groups when checked, the displayed results are sorted by Facility and then by procedure groups; Procedures when checked, the displayed results are sorted by Facility, Procedure Group Code and then by Procedure Code. Select All select this option to check all of the items from the Procedure Groups/Procedures list. For the situations where a procedure has been started, dictated or transcribed many times, the report is created using only the first date/time of each status change. The columns in the results table displays the values according to the options selected at time of the query. The letters after the column header name specifies the option explained (PG by procedure groups, P by procedure) Location the Facility Code Procedure Group the Procedure Group of the procedures indicated in the Procedure column. Procedure the Procedure Code and Procedure Name Number indicates the number of the procedures counted on each row (query by PG) or group (query by P) Arrived-Started (P) the amount of time (in hours) used by a procedure to change its status from Arrived to Started; Arrived-Started (PG) the weighted average time (in hours) used by all the procedures from the same group to change their status from Arrived to Started; Started-Completed (P) the amount of time (in hours) used by a procedure to change its status from Started to Completed; 46
