1 Page 1 FHS Online Application Instructions Before you begin: a. Do not attempt to complete this application using a smartphone. b. Make sure you have paper/pen or other means of recording your login name and password. You must record both to complete your application. c. Make sure that you have a copy of your unofficial (or official) transcript from each college or university you have attended to date. You will need to record information about your grades in certain courses as well as other course credit information in the application. To begin and complete your application: Step 1: Step 2: Go to: Do NOT click on the button for Use My Duck ID this option is not available to admission applicants. Instead, click on Admissions and select click here to create your account in the drop down menu.
2 Page 2 Step 3: On the Create Applicant Account page, enter your personal information as requested, create a password, and click Submit. The website will auto-generate a username. Before continuing, write down or otherwise record your username and password in a secure location, as you will need them to log into the system to complete your application and verify receipt of related materials. astudent0000 Record your user name and password! Step 4: Step 5: Click Back to Login. Your username will be auto-filled on the login page as shown below. astudent0000
3 Page 3 Step 6: Step 7: Type in your password and click the Log Into Your Account button. The first part of the application is the Admissions home page. Click the + Create New Application button. HI, ANY Step 8: Select FHS Application Fall 2018 from the Application Type dropdown menu and click the Next button.
4 Page 4 Step 9: A Family and Human Services (FHS) Major Application title will appear in the instructions portion of the page. The application deadline January 23, 2018 at 5:00pm is also listed on this page under the application name. This page details the FBI Criminal Background Check and UO Conduct Check requirement for conditionally admitted students. After reading, click Next. BEFORE YOU PROCEED: Please be aware to save often. It is strongly recommended that you click Save every few minutes to ensure you do not lose information! After saving, you can exit the application and re-enter your partially-completed application on a different computer using the log-in information at any time before the deadline. DO NOT use the back button in your web browser; this may result in loss of data. Instead, use the Prev and Next buttons at the bottom of the application to move through the pages.
5 Page 5 Step 10: You will be prompted to enter your personal information in the PART 1: PERSONAL INFORMATION portion of the application. Once the information has been entered, click the Save button and then the Next button.
6 Page 6 Step 11: Please list your educational history in the PART 2: EDUCATIONAL HISTORY section of the application. To get started, please click the GREEN + SIGN. There is no limit on the number of institutions that you can add, so please be sure to indicate ALL community colleges, universities or other institutions of higher education that you have attended, the years you attended, and any degrees that you received from those institutions. Once you enter the first college, if you need additional spaces to add more colleges, continue to click the GREEN + SIGN. Students transferring to the UO from another institution: You MUST provide the program with an unofficial transcript from EACH current or prior college and university you attended prior to the application deadline. Transcripts may be submitted in person to the FHS Student Services Coordinator (located in the reception area of the third floor of the HEDCO building on the UO campus) or ed to fhs@uoregon.edu. Current UO students: You may be required to provide an unofficial copy of your transcripts to determine your eligibility for admission. You will be notified via if a transcript is needed. Once your information has been entered, click the Save button and then the Next button.
7 Page 7 Step 12: There are three FHS premajor courses: FHS 213, FHS 215, and FHS 216. FHS 215 and FHS 216 have equivalent courses at Lane Community College (LCC) and Umpqua Community College (UCC). On the PART 3:PREMAJOR COURSEWORK portion of the application, please click the GREEN + SIGN to list what FHS premajor core course(s) you have taken under the FHS Course or Equivalent title), where you completed the course(s), (under the Completion Status title), and what grade(s) you received (under If completed, please specify grade title). To add more FHS premajor core courses that you have taken, click the GREEN + SIGN. Once your information has been entered, click the Save button and then the Next button.
8 Page 8 Step 13: On the PART 4: APPLICATION ESSAY portion of the application, you will be uploading your application essay responses. Carefully proof read each of your essay responses for organization, grammar, and spelling, as we will be assessing your writing abilities. Please address each question completely, but stay within the indicated character limits (3000 characters per essay, including spaces; 3000 characters is equal to approximately 500 words or two double-spaced pages of text). You must save each essay in a separate file and upload each separately. Please be sure to label your files using the following convention: YourLastName_YourFirstName_Essay# (e.g., Smith_Jaime_Essay1, Smith_Jaime_Essay2). When you are satisfied with your essay responses, press + Select File, and upload each essay response where indicated. Once you have uploaded both essay responses, click the Save button and then the Next button.
9 Page 9 Step 14: Under the PART 5: RECOMMENDATION section, please download the reference form where indicated. You must obtain one professional reference using this form from someone who can provide an evaluation of you as a volunteer/intern/employee at a community agency or other service-oriented setting with youth, adults, and/or families. Please type of the first and last name of the individual who will be providing your reference, as well as this person s address. It is your responsibility to ensure that your recommendation form is ed to fhs@uoregon.edu by no later than the application deadline! There is also a space provided to tell us anything else you want us to know when evaluating your application. Once you have downloaded the reference form, entered the requested contact information for your reference, and provided any additional information you wish to provide, click the Save button and then the Next button.
10 Page 10 Step 15: On the final page, check the box to attest to the accuracy of your application materials. To sign your application electronically, check the box indicating you are including your electronic signature and enter the date. When you are ready to submit your application, press Submit. Step 16: If you are not ready to submit, click Cancel to review your information. If you are ready to submit your application, click OK, and you re done! When you submit your application, you will receive a confirmation . Your application should also now be listed under Admission Applications on your tk20 home page! FINAL STEP: Within 1 week of the application closing in January, you will receive a follow-up with a hyperlink to complete a Qualtrics demographics survey. This survey is completely anonymous (i.e., it cannot be linked to your application). We request that you complete this survey; however, it is not required. FHS hopes that students voluntary responses to this broader demographic survey will increase our ability to recruit, support, and graduate a diverse group of students, including both more and less represented aspects of your identities.
