1 ACE - Online Application Instructions [If technical problems occur specific to this ACE process, please contact Nancy Nelson at nnelson@csus.edu, 916/ ] FOLLOW THESE DIRECTIONS EXACTLY AS INDICATED * {CSUMentor Open 8/15/16 9/15/16} 1. Bring up a web-browser, such as Internet Explorer, Safari, or Firefox. 2. Go to: 3. Select: Apply Online 4. Select: Undergraduate Admission Applications (the top one) 5. Click > Fall 2016 (box on the left) Ignore the messages that say we are not accepting applications. 6. Log On to CSUMentor a. If you already have YOUR OWN Username and Password, enter this information in the Returning user area (right-side box) b. If you do not have an account, you must create one. DO NOT USE SOMEONE ELSE S ACCOUNT. Create Account Go to the create an account button at the bottom of the page. The information you put in for your account will populate fields in the admission application. i. Name: use your legal name. Do not use nicknames. ii. Date of birth: be sure to use the // between month, day and year. iii. address: if you have more than one, use the one you will remember to check. iv. USERNAME AND PASSWORD: Pick one you will remember. v. PASSWORD HINT: Pick something that won t change over time. vi. MAILING ADDRESS: Leave international postal code blank, you are in the USA. vii. Telephone numbers: use numbers you will answer or get your messages. viii. Current grade level: select one that applies. ix. How did you hear about CSUMentor: Just pick one. 7. Once logged into the Undergraduate Application , use the scroll bar in the box to select CSU Sacramento and click start new application. 8. This will bring up a window for the Sac State application with a lot of information. Scroll to the bottom of the page and click > Begin Application for CSU Sacramento. c. Application Information: ONLY enter the below data for these fields. You MUST use this information designated specifically for ACE. i. Term applying for: Fall 2016 (box on the right) ii. Intended major: ZZZ-2* (scroll down to the very bottom of the list) iii. Degree objective: None iv. Credential program: Not interested in a credential program d. Entry Status: i. Graduating high school senior with no college credit. e. Previous Application/Attendance i. Term last applied: Never applied f. Term last attended i. Never attended 1

2 9. NAME AND ADDRESS a. Name: enter your full legal name if it isn t already populated. b. Other name: only enter if your high school records list something other than your legal name. c. Current Mailing Address: update if not correct. d. Permanent Address: Click the blue link (it turns pink) if the address will be the same. e. Contact information: will need to add Home telephone number. f. address: use the one you use and will check often. 10. PERSONAL INFORMATION a. Social Security number: Do Not guess and Do Not use someone else s. You MUST provide this information. b. Student ID: enter only if you are sure you know it. c. Date of birth: be sure to use the // between month, day and year. d. Foster Youth, Orphan or Ward of the court: check all that apply. e. Place of Birth: this is your information. You MUST use this information designated specifically for ACE so additional fees are NOT charged. f. Country of citizenship: enter United States of America (regardless of where you re from) g. Citizenship status: enter US citizen (regardless of your real status) h. Permanent Residence - California county: county where you live i. Permanent Residence - US State: SKIP j. Permanent Residence - Country: SKIP k. Family Education, Income, and Size Information: SKIP l. Miscellaneous Information: SKIP 11. CALIFORNIA STATE RESIDENCY INFORMATION You MUST use this information designated specifically for ACE so additional fees are NOT charged. a. What state do you regard as your permanent home: California b. Do you claim California residency: Yes c. Have you lived in California continuously since birth: Yes d. If no, when did your present stay in California begin: Skip e. Previous residences outside California: Skip 12. DEMOGRAPHIC INFORMATION - a. 3 13. HIGH SCHOOL INFORMATION Last High School Attended a. High school: click the Find button this will bring up another screen. i. US state/territory: select California ii. Select the letter for the name of your school. This will bring up a list of California high schools that begin with that letter. Scroll until you find your school. iii. Select your school. b. Graduation date: enter what it will be even if you aren t a senior. c. Other High School(s) Attended: SKIP d. Test Information: SKIP e. Early Assessment program EAP: SKIP 14. HIGH SCHOOL PREPARATION 1 SKIP You will get an error message because you didn t fill in any coursework..(don t worry - we don t want you to). Click the box = I have read the above and wish to proceed with this application. You will continue to get the message until you get past the pages for high school subject information. 15. HIGH SCHOOL GPA a. Enter your current high school GPA. Do NOT use the GPA calculator link (you haven t entered any grades to calculate) 16. EDUCATIONAL OPPORTUNITY PROGRAM (EOP) a. Are you interested in applying to EOP?: NO b. SKIP the rest of the questions. 17. APPLICATION FEE WAIVER a. Do you want to apply for an application fee waiver: NO Your application fee is already waived by the ACE program. Click > SUBMIT YOUR COMPLETED APPLICATION You will get another error message saying your High School course Information is Incomplete. Click the blue link at the bottom of the message = (yes, I know it s incomplete and I still want to submit it.) 18. CSU SACRAMENTO APPLICATION SUBMITTAL a. Payment options: click > Check or Money Order A waiver will be applied to the $55 CSU Mentor application fee once your hardcopy ACE application is received. DO NOT pay the $55 fee by credit card. DO NOT send in any payment ($55) to the address indicated on your confirmation page. b. Signature: Type in your name Click > SUBMIT NOW 3

4 19. CONFIRMATION PAGE You will not get a confirmation page until you have actually submitted your application. The confirmation page includes important information such as: the name you applied under, the confirmation number and the date and time you submitted your application. Once you receive your confirmation, please print out that single page and submit it along with your hard copy ACE Application and set fee to your course instructor.
