5 Screen when you have clicked on the evidence prelog button. Use when submitting evidence for the first time in a case If you have already submitted some evidence in a case

6 Screen that appears when you have clicked on the new case button Please do not type in all caps use proper Please do not type in all caps, use proper punctuation, start with a capital letter and end with a period.

7 If you have worked with a DPS Agency (ISP, DCI Fire Marshal) put yes in this box if you have not put no in this box. Your case number. If your name doesn t appear in this pull down list contact Kathleen at the lab. When you pick your name from the list the BEAST will fill in name, and phone number for you.

8 Based on the Iowa Code can have up to 3 offense types for the case. (List of offense types attached) To be used for information the Lab needs to know about your case to process your submitted evidence. Leave these bottom two boxes empty.

9 When all the information is complete click the save button. Once you have saved this screen you will always be able to find your case. You are now ready to move on to the (2) Tab Names.

10 This screen will appear when you select the names tab. To add a name you will click on the add button. This will open up the fields for information to be added.

11 If you don t have the information necessary to complete a field just leave it blank. Only put hyphens in the SS#. DOB must be entered using the entire year. (1990)

12 When you save the information it will move to the top. If you need to correct any information after you have saved the name - highlight the name, click on edit and the name will appear in the boxes at the bottom so you can make your corrections. You are now ready to move on to the items tab.

13 This is the screen that appears when you click on the items button. This works just like the names tab, add and save the information.

14 This field MUST match the information on your piece of evidence. If you have your evidence marked with the designation of 123 you would put 123 in this box. This field can be numbers or alpha.

15 This field contains the packaging that holds your evidence. If you have a sealed evidence bag that you will be placing in an envelope to mailed to the lab or a box that will hold several cases the packaging is sealed evidence bag. (A list of codes is attached)

16 Try to find the item type that best describes your piece of evidence. (A list of item types is attached)

17 This field is used to describe what is inside your packaging. Cup of urine, two sealed tubes of blood, plastic bag containing white powder. Only describe the evidence here do not add what testing you want performed ed in this field.

18 When you select the item type urine specimen a special DRE When you select the item type urine specimen a special DRE field will come up if you did not do a DRE do not fill in drug type but on a urine specimen please advise if 1 st void or 2 nd void.

19 Just like the names tab when you click on the save button your items move to the top. If you need to make any changes highlight the item you need to change and click edit. You are now ready for the Lab Submission tab.

20 This screen appears when you click the Lab Submission tab.

21 If you have additional info about the case it can be entered here. Used for carrier tracking number only. When you click the add button all of the items you entered will appear in this area. If you have more then 10 items you will find additional page information in this area

23 This screen will come up when you click on the select button. You can have up to 4 tests preformed on any one item of evidence.

24 Highlight the analysis you want for your item. After you have completed as many tests as you want for that item of evidence you will click on the save button.

25 When all of your analysis is selected and you have clicked the save button, you will now be able to click on the submit button.

26 This packing slip needs to be submitted with your evidence. All of the information about your case should appear names, items etc.

29 Click here to find the prior case.

30 Enter the case number you want to find. If you don t know the entire case number you can put a check mark in the partial box. After you have entered your case number click the find button.

31 Your case or a list of cases will appear. Highlight the case Your case or a list of cases will appear. Highlight the case you want to open. Then click open case.

32 Your case information will appear. If all of your case information is If all of your case information is the same you can move onto the items tab to add your additional items.

33 Click the add button to enter your additional items.

34 Items 5 and 6 are the additional items added in this case. When all items have been added click on the lab submission tab at the top

35 It will show submission #1 with the items that have been submitted to the lab. These blue arrows can be used to view previous submissions Click the add button do not edit a previous submission.

36 It will move to the next submission. All of the items will move to this submission but only select analysis on the items you are sending with this submission.

37 When you have selected the analysis you will then click the save button.

39 Next you will click on the submit button.

40 The packing slip with only the additional items needs to be printed and submitted with your items of evidence.
