1 Global Employment Screening Country Requirements for new hire candidates Malaysia Guide and country requirements Rev Mar/2017

2 Instructions Review this document in its entirety. Access your First Advantage (FADV) profile via the secure URL provided in the you received from DXC Human Resources and follow the prompts to provide your consent for First Advantage to initiate the screening. Please do your best to complete this action during the next 24 hours to avoid delays in screening and impact to your start date. Please ensure that you have uploaded your current resume via the DXC applicant portal. Any information you had provided at the time of your application via the DXC portal at or was entered on your behalf by DXC, has been transferred to your First Advantage screening profile. Please validate that this information is accurate and add any new information as necessary to avoid delays on the employment screening. Carefully review your screening profile, making edits as needed and providing any missing information as noted in each section of the application. If additional documents are needed, you will be prompted to upload them. Click the Save button to keep your updates. It is recommended that you upload all relevant documentation that could support with validating education and employment. Examples of employment documentation to validate your start and end dates including job title are: Employment contract,, pay slips, employment service letters and tax documents. For educational documents, please provide colour copy of your diploma and official transcript to confirm your highest degree qualification. After verifying that all required fields are accurate, submit your application. You will be able to print it and will receive an electronic copy of the submitted information. Please note that the information you entered into the First Advantage system cannot be modified once submitted. First Advantage will begin processing your screening. If additional information is required, you will be contacted by First Advantage or a DXC representative. Please review your Inbox at the address you provided to DXC on a daily basis and respond to any such inquiries immediately to avoid delays in processing your employment screening. Please refer to section EMPLOYMENT SCREENING REQUIREMENTS CHECKLIST highlighting important points to note when declaring your information onto the portal. Please read and follow the instructions carefully.

Please read carefully. Personal Information Please declare your full legal name as reflected on your government issued identity. Please note that if you furnish your name which differs from your government issued identity, your application will be cancelled and you will be required to re-submit your application.

4 In addition, if you have had a name change, please declare this and provide the necessary legal documentation to substantiate this change. Also please declare all other names ie English Christian names, aliases, maiden, AKA etc under the Aliases section see screenshot below. Candidates from INDIA: Father s Full Name and Full Address: This information is REQUIRED for DXC to proceed with your criminal check. Please ensure that you declare your father s full legal name in the system. Abbreviations will not be accepted. Candidates from CHINA: Name in local language: Please declare your name in local language. Do not enter your mother s or father s name in this field. National ID Please provide a valid clear color copy front and back of your government issued ID. If you do not have a government ID from the country in which you are working, please submit a government ID from your country of residence (i.e. passport or driving license). Education Degree and field of study should be indicated in English (be very careful with translations). INDIA and THE PHILIPPINES: Graduated candidates are required to provide copies of both mark sheets/transcripts and degree certificate. OTHER COUNTRIES: Graduated candidates please provide color copy of diploma/degree certificate.

5 If candidate has not graduated, please state so on the application. Employment If you are employed by an employment/contracting agency, please declare the agency name and contact details as your employer. If you have had multiple roles at the same company, please enter as only one employer and do not break them down by time period. Declare only your last job title. IMPORTANT! If your present and former employers are associated and you do not want your current employer contacted, DO NOT enter former employer information which may result in your declared current employer being contacted by First Advantage. ALL COUNTRIES: Please provide copy of certificate of employment for your last 2 former employers excluding current. If certificate of employment is not available, please provide alternative documents such as employment contracts/offer letter + pay statements, and/or name cards. We need to validate your start date, end date and job title. Please include contact details (HR contact or Supervisor name, and/or company telephone number) refer to screenshot below.). If the employer/contracting agency no longer exists or has closed operations, please obtain documents from that time period and send them to First Advantage. Criminal History If your country requires documents for criminal investigations, ensure you have them on hand. DON T FORGET Ensure you receive a confirmation regarding the submission of your application. Upon successful submission of your consent and screening profile to First Advantage, you will receive an confirmation from DXC Human Resources. Check your inbox and junk daily after the application has been submitted in case First Advantage requires additional clarification and/or documentation. When contacted by FADV, please ensure you provide the necessary requested information/documentation within 24 hours otherwise your screening will be delayed hence impacting your start date.

6 When sending documents to FADV, file size should not exceed 4MB and should preferably be compressed. If file size exceeds 4MB, send documents separately. When uploading documents on the FADV website, file format should preferably be in TFF or jpeg format and file name should not contain any symbols. Required Information for your First Advantage application: In addition to your personal information, additional documentation will be required by First Advantage to process your employment screening. ADDRESS HISTORY: You will be instructed to provide your address history including dates at each residence. o For Executive-level roles (e.g., Director, VP, General Manager, SVP, and EVP): Past 7 years o For all other DXC roles: Past 5 years CRIMINAL CHECK: You will be provided with specific instructions for each one of the countries you listed under your address history. Please follow the instructions to avoid delays on your screening. NATIONAL ID: If you do not have a Social Security Number, please select no and proceed to enter your country of origin government issued ID (such as passport or driver s license from your country of origin). You will be prompted to upload a legible color copy of both sides of the document. EMPLOYMENT: Please list current and 2 most recent previous employments. Previous employers will be contacted to verify your employment history hence please provide full names and addresses where possible including telephone numbers. If you have gaps in-between these employments, please include these and select the corresponding category under the 'Employment Type' field: self-employment, not employed, in school or unpaid internship. You are encouraged to upload your employment documentation ie employment service letters or offer letter + relieving letters/last month payslips that can help us validate your start and end dates including job title. o Name of employer and complete address o Last position title held as it should appear on the company HR system o Employment dates (start and end) o Employer HR contact information (name, phone number / address). First Advantage will only contact official employer s representatives. Note: If you are currently working as a DXC contractor, do NOT list DXC as your current employer; rather, provide the name of your contracting agency as the current employer. If any of your previous positions were through an agency, please ensure that you enter the agency name as the company name and not the details for the customer that you were assigned to. EDUCATION: For your highest completed/graduated University-level /graduate degree please provide: o Name and complete address of educational institution o Attendance dates (start and end) / Graduation date

7 o Name of degree, qualification, or field of study. Note: Neither certifications/university degrees in progress, not exchange programs, will be verified. If included, please make sure that you do not select graduated in the drop down menu. Please upload a copy of your diploma including transcripts/mark sheets.
