Authenticating and Sign On System Information Broker Dealer Using eforms: An Overview eforms is your source for electronic forms and other documents from all lines of business in the MetLife Enterprise. You can search for forms using several search criteria. These include searching by form name, number, and groups of forms with a similar purpose. Don t know the form name or number? Our Smart Search wizards let you drill down to just the forms you need for a particular transaction by answering a few short questions. Once located, forms can be viewed, printed, faxed, or ed. Use the same form(s) frequently? You can create your own customized electronic kits, and can save your most frequently used forms to MyForms, a feature that allows quick retrieval without searching. 2. How Do I Navigate Around the New eforms site? There are many ways to navigate the eforms site to find the forms you are looking for. We ve provided some examples of navigating and searching on the new site vs. the old site. Home Page You will notice a new look on the Home Page of the new redesigned eforms website, but you shouldn t be all too surprised: we now have the same look and feel as other MetLife websites. As you can see, there are some familiar items on the Home Page such as MyForms, What s New, System Information, and the ContactUs links. 1 eforms
2 3. What Happened to Quick Form Number Search? Quick Form Number Search is now called Simple Search: it is on the Home Page. Simple Search now allows users to search across the entire repository by Form Number or Form Name. 4. Navigation How Do I Search? On the old eforms website, you selected your Product Line from the Home Page via the radio buttons. Available products are now selected using the navigation menus from top menu bar. First, select whether you want Individual Product Forms (Individual Insurance and Investment Products and Services), Group Product Forms (Group Insurance and Investments Products and Services) and Administrative Forms (internal forms): Note that there is a 2 nd level drill down navigation menu. This allows you to select a specific product type: 2 eforms
3 5. What Happened to the Search Wizards? Search Wizard is now called Smart Search and opens in the right hand accordion after you have selected a product from the top navigation menus. Just answer the questions in the right pane as you did in the old site and click on Submit to get your forms. The return forms page now allows you to select all or some forms via the checkboxes in the first column. Then use the Selected Item Actions menu in the right hand pane to View, , Fax or add forms to MyForms. Note that selecting multiple forms via the checkboxes automatically creates a kit to View, or Fax, and you can add multiple forms to your MyForms. Note that on the return forms page via a Smart Search, your required forms are automatically checked for 1 click kit creation. You can also Start a New Search or Revise your existing Search using the buttons under the Selected Item Actions menu. IMPORTANT NOTE: The Internet Explorer Back Button does not work in eforms. This is intentional: using the back button does not retain any of the search parameters you specified if you want to revise your search. If you want to change to a different product, select New Search: if you want to search keeping the same product, use Revise Search. You can also click the Home button in the Menu Bar to return to the eforms home page. 3 eforms
4 6. How Do I Search by Name or Search by Number? After drilling down using the navigation menus to the Product group you are interested in, you can search for forms by Name or Number via Simple Search, and your return results will be limited to the Product group you selected. As before, enter some or all of the form name or number: you will receive more results by limiting your search. Note: Forms are not always named intuitively: a limited search will give you more chances to get the form you want. In the picture above, form Name/Number returns would be limited to Long Term Care forms. IMPORTANT: If you use Simple Search from the Home Page and do not select a product or service from the menu bar, you will search the entire eforms repository across all products and your search may return a large number of forms. 4 eforms
5 7. What About Search by Group? Search by Group is now Group Search. Select the Product you are interested in from the top navigation menus. Then simply click on the Group Search bar in the right hand pane to open the Group Search. 8. What Happened to Search by Properties? Search by Properties is no longer available. We are replacing our forms management system that feeds eforms in 2010 and will relaunch an improved Properties search later this year. 9. Give Us Your Feedback! We want your feedback! For your convenience, we ve added a new Feedback link at the top of every page. Clicking this link will open a page to send us your questions or comments. 5 eforms
6 10. How Do I Know What Forms Have Changed? There is now a What's New section on the Home Page. The What's New section contains links for more information regarding any forms that have been updated or changed, and any new site information or features. Click on the scrolling image to open a new page with more What s New information. 11. How Do I Use MyForms? The MyForms feature in eforms allows you to create and maintain a folder containing just the forms you most frequently use. MyForms is available from any page in eforms. To add a form to your MyForms, first find the form you want to add. There is no restriction on how you search for the form. When you have the form(s) you want listed, click the checkbox (es) to the left of the form(s) you want to add, and then click Add to MyForms in the Selected Item Actions in the right hand pane. Once you have added a form to your MyForms, you can easily go to it by clicking the MyForms tab in the top navigation menu from any page in eforms. A listing of your MyForms will be displayed. You can remove forms from your MyForms by clicking the checkbox, then Remove from MyForms in the Selected Item Actions list. 6 eforms
7 Important Note: MyForms does not store copies of the actual forms on your computer. It stores the information about the form you ve saved in a "cookie" that is stored on your computer: the cookie retrieves your form from eforms when you connect to the site and select your form. You must have cookies enabled to use MyForms. In addition, if you delete your cookies without backing them up you will lose your MyForms. Any form that you have saved in MyForms will be automatically updated if a new version comes out, or if the form is obsolete and not replaced. However, in cases where new forms are released but are not immediately available in all states, you will need to check with your marketing area and update your selections when the new form is available in your state. 12. How Do I Use MyKits? Note that MyKits is only available for Individual Life Smart Searches that do not use tele underwriting. To save a commonly used Search return, run your Search and from the Results page, select Add this Search to MyKits from the Selected Item Actions. You will be prompted to enter a name for your Kit, which must not include special characters. The Save Kit dialog will show you the special characters to avoid. To retrieve your saved Kits, simply click MyKits from the top Navigation menu. 7 eforms
8 Use the radio button to Select the Kit that you want to run. You will receive the return forms page from the saved Smart Search that you saved to MyKits. 13. How Do I Fax and E mail? Fax From the Results page, use the checkboxes to select the form or forms that you want to fax then click Fax under the Selected Item Actions. On the Fax information page, please fill in all information including sender and recipient information and fax numbers, then click on Send. Your forms have been kitted and faxed all in 1 step! IMPORTANT NOTE: You must enter the fax number including 1 + the area code. This is necessary because eforms does not fax through your telephone line, but uses a fax server which may or may not be local to where you are faxing to. Also, we recommend that forms be ed whenever possible, rather than faxed. This is because the quality of the forms transmitted is much better with ; also, the forms are received much quicker and at less cost. E mail From the Results page, use the checkboxes to select the form or forms that you want to fax then click under the Selected Item Actions. On the information page, please fill in all information including sender and recipient information and fax numbers, then click on Send. Your forms have been kitted and ed all in 1 step! Note that addresses must be the full Internet e mail address (i.e., jsmith@metlife.com). The forms will be ed as an attachment to the . Only one attachment will be created regardless of the number of forms selected. Whenever possible, forms should be ed rather than faxed. 8 eforms
9 Please be aware that Internet Service Providers (ISPs) have different standards on accepting s; including limiting the size of attachments not accepting attachments at all, and strict "junk mail" policies. If you does not go through, you should check with the recipient's ISP to determine what the problem is. 14. Who Do I Contact to Get Help Using eforms? There is a ContactUs link on the top of every page. The ContactUs page contains phone numbers and addresses for assistance with eforms. General Help, Technical Problems and Assistance For technical problems and assistance, including user ID and password questions, problems with a form opening, missing forms, or problems faxing or e mailing forms, please write down the form(s) you were trying to access or the SmartSearch scenario you were using and call the MetLife IT Service Desk at (800) ASK MET2. For questions about what form(s) to use, please call your Regional Wholesaler. For questions about ordering forms, please contact the Fulfillment Center at (800) MET STUF. MetLife Broker Dealer Administration and Representative Support MetLife Securities Representatives For technical problems and assistance, please call Broker Dealer IT Field Support at (800) , Prompt 6. For site how to help, please call Broker Dealer Customer Service at (800) , Prompts 2, 2: Support is available from 8:30 am 6 pm Eastern Time. New England Securities Representatives For technical problems and assistance, please call Broker Dealer IT Field Support team at , Prompt 7. For site how to help, please call Broker Dealer Customer Service at (800) , Prompts 1, 3, Authenticating and Sign On eforms uses Single SignOn. If you access eforms from a portal you will not be asked to authenticate. If you access eforms from home or by entering eforms.metlife.com in your browser, you can click the Sign In link at the top of the page. You will then be presented with the Sign In screen. Enter your user ID and password, click Enter, and you are authenticated. Authentication allows you to , fax, and complete an eapp (if applicable). Authentication will also know who you are and will reduce the number of questions you need to answer when using the Smart Searches. 9 eforms
10 16. System Information eforms is optimized for viewing with 1280x720 screen resolution and 75% zoom level in Internet Explorer (see View Zoom in Internet Explorer to change). IMPORTANT NOTE: This website is designed in frames, therefore the IE Back button does not work. Please use the Revise Search or New Search buttons to navigate. Adobe version is required to use eforms. To use MyForms and MyKits, cookies must be enabled and pop up blocking software must be turned off. If you delete your cookies without a backup, you will also delete your MyForms and MyKits. This site is designed for use with Microsoft Internet Explorer 6. Other web browsers are not supported at this time. 17. Broker Dealer The eforms Broker Dealer sites are not updated at this time: they will be redesigned later in You can access New England Securities and MetLife Securities from the redesigned eforms site via Investments Mutual Funds and Brokerage. Use the Smart Search to access the current Broker Dealer forms sites. Note that the Simple Search (Form Name/Form Number) does not work on the redesigned site and you must search by Name or Number via the existing MSI and NES sites. For your convenience, Group Search contains searches for all NES and all MSI forms, which you can then add to your MyForms. 10 eforms
