1 NHSN Training Session 1: Establishing Access through Secure Access Management Services (SAMS)

2 NHSN Enrollment Who needs to go through the SAMS registration process to get access to NHSN? The person enrolling a facility into NHSN for the first time will go through the SAMS registration process. Once a facility has been enrolled, the NHSN Facility Administrator can add additional users to help enter the data for their facility. Any person planning to help with NHSN data submission or analysis will also need to go through the SAMS registration process. 2

5 Step 1 A. & B. Open and Review Required LTCF Materials A. Open and review the required LTCF overview training slide set: B. Open and review the required enrollment training slide set: NOTE: It is recommended that users print this document as a guide through the enrollment process. Step 1 should take roughly 3 hours to complete. 5

6 STEP 1 C. Facility Contact Information Form Print the Facility Contact Information form. Do not send paper form to NHSN Collects information for staff coordinating NHSN use May need to collect some of the information from others in your facility Be sure you have your Medicare Certification Number (CCN). Select the LTC Facility Component as well as the Healthcare Personnel Safety Component (see arrows) Fill in the NHSN Facility Administrator information You may designate a different point of contact for each component You don t have to include additional people if the NHSN Facility Administrator will be the primary contact for your LTCF 6

7 Step 1 D. & E. Preparing Your Computer for NHSN You may need to change your and Internet security settings to receive communications from NHSN during the enrollment process. Add to trusted sites list and allow pop-ups. In Internet Explorer, open Tools menu, select Internet Options Add trusted sites on the Security tab Allow pop-ups on the Privacy tab Change spam-blocker settings to allow all from nhsn@cdc.gov and SAMS-NO-REPLY@cdc.gov These changes may require assistance from your IT manager or department 7

8 Step 2 A. Register Facility with NHSN Read and agree to NHSN Rules of Behavior by clicking on the Rules of Behavior link from the checklist or this link: Review the terms If you would like a PDF copy of the terms, click the link next to the PDF icon to print it before you click Agree. Click Agree. 8

9 Step 2 B. Register Facility with NHSN Facility Administrator completes. Imperative that the same address is used throughout this process. If your Facility Identifier does not validate, you can request a CDC Registration ID by ing nhsn@cdc.gov. Once you have completed the training, enter the date of completion. Remember to click Submit. 9

10 Step 2 Register Facility with NHSN If your registration was completed, you will soon receive 2 s: o Welcome from NHSN o Invitation to register with SAMS (Secure Access Management Service) *TIP: Create an NHSN folder in your Outlook to save all e- mails from NHSN and SAMS- No-Reply so they are easily accessible. 10

11 Step 3 A. Register Facility with SAMS Follow the steps in the Getting Access to NHSN for your LTCF PDF. It may help to print this out, to follow the step-by-step visuals. Invitation to register with SAMS (Secure Access Management Service) Print the SAMS it has information you will need to register with SAMS. Click on the link at the bottom of the SAMS-No-Reply to begin SAMS registration *TIP: Create an NHSN folder in your Outlook to save all s from NHSN and SAMS-No-Reply so they are easily accessible. 11

12 Step 3 A. Register with SAMS After clicking on the SAMS link provided in the , enter your User Name and Temporary Password in the SAMS Credentials section 12

13 Step 3 A. Register with SAMS Once logged into SAMS, accept the SAMS Rules of Behavior. 13

14 Step 3 A. Register with SAMS Complete the online SAMS registration using the same name and address you will provide in the identity documents that you will submit to SAMS. The process will ask for your home address this is part of the proofing. You will be asked to create your own password, which must o Be eight or more characters long. o Contain at least three of the following: uppercase, lowercase, numeric, and non-numeric character. o Not contain your username or any part of your full name. o When making future changes, be different than your previous 13 passwords. NOTE: Be sure to write this password down as you will need this information again. Click Submit 14

15 Step 3 B. Complete & Notarize the Identity Verification form After completing registration, you will receive a SAMS confirmation that contains all the information and documentation for Identity Verification. Print this out because it contains a document that must be completed Obtain the appropriate identifications (1 unexpired photo ID with current home address from List A and 1 unexpired document from List B) Take the completed application and identity documentation to a notary public. NOTE: Notary should endorse the application using their commission / stamp. 15

Complete & Submit Identity Proofing (Verification) Fax, mail or digitally upload the endorsed Identify Verification Form and a copy of the two forms of identification to CDC Proofing Authority Fax: Toll-Free Number: Mail: Centers for Disease Control & Prevention, Attn: Proofing Authority 1600 Clifton Road N.E. Mailstop K-94 Atlanta, GA **Digitally Upload: Pull up and click on link for SAMS upload (Faster Processing) o Log in. o Upload documents. o You will get an confirmation. *IMPORTANT: Two forms of ID must be submitted with the endorsed Identify Verification Form. o One unexpired document from List A and o One additional unexpired document from List B 16

17 Step 3 D. Registration Approval After your identity documentation has been processed, you will receive confirmation of approval for SAMS access via 2 s: SAMS Account Activation SAMS Activity Authorization You will also be issued an electronic grid card, which is used when logging into the system along with your username and password. This will be delivered via mail to your home address. NOTE: If you do not receive your SAMS grid card within two weeks after receiving your SAMS approval, contact samshelp@cdc.gov or call the toll-free number for assistance. 17
